A doença falciforme (DF) cursa com múltiplas complicações clínicas, destacando-se manifestações crônicas na medula óssea secundárias à hemólise, com persistência da medula hematopoiética nas crianças e reconversão medular nos adultos, além de eventos ósseos vaso-oclusivos. A ressonância magnética (RM) permite boa avaliação destas alterações, destacando-se o estudo da medula óssea, com análise qualitativa e recursos quantitativos. Dentre eles, ressalta-se a técnica DIXON pela rapidez de execução e facilidade de processamento. Assim, diante da necessidade de investigar potenciais parâmetros que permitam aferir a gravidade da DF, o presente estudo pretende avaliar a medida da fração de gordura na medula óssea pela técnica DIXON de seis pontos como potencial biomarcador de gravidade clínica. Trata-se de estudo de corte transversal com grupos de comparação (DF e controle) em que todos os participantes realizaram RM da coluna lombar e da bacia com imagens na sequência IDEAL-IQ (técnica DIXON de seis pontos) para quantificação da fração de gordura (FGdix). Utilizamos regiões de interesse nos corpos vertebrais lombares e na primeira vértebra sacral, bem como nos ilíacos, cabeças femorais, trocanteres maiores dos fêmures e colos femorais. Os pacientes com DF preencheram questionário clínico-demográfico e realizaram alguns exames laboratoriais. A ferramenta "Sickle Cell Disease Severity Calculator" foi utilizada para o cálculo dos escores de gravidade da DF. As alterações ósseas secundárias a fenômenos vaso-oclusivos foram agrupadas na variável complicações ósseas e os dados de dosagem de hemoglobina sérica Hb ≤ 9,5 g/dl e LDH > 300 U/L permitiram a criação da variável hemólise. Foram 96 participantes, pareados por sexo, peso e idade, sendo 48 com DF, destes, 26 eram do subtipo HbSS e 22 HbSC. Os achados mais frequentes nos pacientes do grupo DF, em ordem decrescente, foram necrose avascular da cabeça femoral (75%), infartos ósseos (58,3%), vértebras com morfologia em "H" (58,3%) e necrose da medula óssea (8,3%). Além disso, foi observada uma maior espessura da cortical óssea nos pacientes com DF em relação ao grupo controle e identificamos frequência não desprezível de necrose da medula óssea no grupo DF. A espessura da cortical óssea nas porções proximais das diáfises femorais nos pacientes com DF apresentou moderada correlação negativa com a medida da FGdix nas vértebras lombares, ilíacos, colos femorais e na primeira vértebra sacral. Além disso, a FGdix foi menor nos pacientes com DF em relação ao grupo controle, apresentando valores ainda mais baixos no subtipo HbSS (mais graves) e nos pacientes com maior grau de hemólise, bem como nos que apresentaram mais complicações ósseas por eventos vaso-oclusivos. Assim, a FGdix demonstrou ser um parâmetro com potencial utilidade no acompanhamento do tratamento, bem como na avaliação da gravidade da DF, destacando-se os ilíacos e as cabeças femorais como melhores locais para aferição da FGdix.

Sickle cell disease (SCD) is associated with multiple clinical complications, especially chronic manifestations in the bone marrow secondary to hemolysis, with persistence of the hematopoietic marrow in children and bone marrow reconversion in adults, in addition to vaso-occlusive bone events. Magnetic resonance imaging (MRI) allows a good evaluation of these changes, with emphasis on the study of bone marrow, with qualitative analysis and quantitative resources. Among them, the DIXON technique stands out for its speed of execution and ease of processing. Thus, in view of the need to investigate potential parameters to assess the severity of SCD, the present study intends to evaluate the measurement of the fat fraction in the bone marrow by the six-point DIXON technique as a potential biomarker of clinical severity. This is a cross-sectional study with comparison groups (SCD and control) in which all participants underwent MRI of the lumbar spine and pelvis with images in the IDEAL-IQ sequence (six-point DIXON technique) to quantify the fat fraction (FFdix). We used regions of interest in the lumbar vertebral bodies and the first sacral vertebra, as well as in the iliacs, femoral heads, greater trochanters of the femurs and femoral necks. SCD patients completed a clinical-demographic questionnaire and underwent some laboratory tests. The tool "Sickle Cell Disease Severity Calculator" was used to calculate the severity scores of SCD. Bone changes secondary to vaso-occlusive phenomena were grouped into the variable named bone complications; the serum hemoglobin dosage data ≤ 9.5 g/dl and lactate dehydrogenase > 300 U/L allowed the creation of the hemolysis variable. There were 96 participants, matched for sex, weight and age, 48 with SCD, of these, 26 were of the HbSS subtype and 22 were HbSC subtype. The most frequent findings in patients of SCD group, in decreasing order, were avascular necrosis of the femoral head (75%), bone infarctions (58.3%), H-shaped vertebra (58.3%) and necrosis of the bone marrow (8.3%). In addition, a greater thickness of the cortical bone was observed in patients with SCD compared to the control group and we identified a non-negligible frequency of bone marrow necrosis in the SCD group. The thickness of the cortical bone in the proximal portions of the femoral diaphysis in patients with SCD presented a moderate negative correlation with the measurement of FGdix in the lumbar, iliac, femoral necks and in the first sacral vertebrae. In addition, FFdix was lower in patients with SCD compared to the control group, showing even lower values in the HbSS subtype (more severe) and in patients with a higher degree of hemolysis, as well as in those who had more bone complications due to vaso-occlusive events. Thus, FFdix proved to be a parameter with potential usefulness in monitoring treatment, as well as in assessing the severity of SCD, highlighting the iliac and femoral heads as the best places for measuring FFdix.
