We investigated the effects of different fitness levels on the autonomic modulation of heart rate variability (HRV) in healthy volunteers and the difference in response between genders. Ninety-six healthy volunteers, aged between 18 and 45 years (48 men and 48women) underwent cardiopulmonary exercise test and divided into three groups according to the response of VO2peak; group low performance (LP= VO2: 22-38 ml kg-1 min-1, n=16 men and n=16 women), mean performance (MP= VO2: 38-48 ml kg-1 min-1, n=16 men and n=16 women) and high performance (HP= VO2: > 48 ml kg-1 min-1, n=16 men and n=16 women). The experimental protocol used to evaluate the HRV was the spectral analysis of R-R interval time series derived from the record the electrocardiogram at rest in the supine position (baseline) and during the tilt test. The results showed that the fitness level did not promote changes in spectral parameters of HRV in the supine position when the genders were compared separately. However, when genders were compared, there were differences between each grade fitness study. Women had lower blood pressure values and the LF oscillations, and higher values of HF oscillations in relation to their groups formed by men, regardless of fitness level. In the tilt test, all groups made up of men had similar responses. On the other hand, when the tilt test was administered to groups of women, we observed that the high performance group had smaller changes in LF and HF oscillations. In conclusion, our results suggest that men have a modulating cardiac autonomic balance more favorable to the LF oscillations, whereas in women the HF oscillations are more deterministic. In addition, women's group of highperformance seem less reactive to orthostatic stress
