São raros os estudos epidemiológicos sobre neuropatias periféricas crônicas em crianças e adolescentes. Nesse estudo realizamos análise clínica, eletrofisiológica e molecular de 149 pacientes portadores de neuropatia periférica crônica, com início antes dos 18 anos, todos pertencentes a diferentes famílias, encaminhados ao HCFMRP-USP nos últimos 15 anos. Um total de 77.2% dos casos foi classificado como de origem hereditária, incluindo 66% de CMT, 23% de complexas, 6% de HNPP e 5% de NHSA; 12.1% dos casos como de origem adquirida, incluindo 61.1% de PDIC, 27.8% de mononeurite múltipla e 11.1% de neuropatia associada à diabete melitus; 9.4% dos casos como de origem indeterminada; e em 1.3% dos casos observou-se uma coexistência de CMT e PDIC. Os pacientes com CMT foram estratificados conforme a herança (dominante, recessivo, ligada ao sexo ou esporádico) e subestratificados conforme a velocidade de condução dos nervos nos membros superiores (desmielinizante se a velocidade de condução era ≤30m/s, intermediário se a velocidade de condução era entre 30-45m/s, e axonal se a velocidade de condução era ≤45m/s). Realizaram-se testes moleculares de acordo com o grupo em que o paciente foi subestratificado (duplicação ou deleção da região 17p11.2-p12 por PCR e/ou sequenciamento dos genes PMP22, MPO, LITAF, EGR2, GJB1 e MFN2). Assim, entre os 75 pacientes com CMT, 54.7% apresentavam CMTI, incluindo 34 com CMT1A (32 duplicação e 2 mutação ponto na Ser 72) e 2 CMT1B (mutação ponto no gene MPZ); 5.3% apresentavam CMT4; 1.3% apresentavam uma neuropatia desmielinizante esporádica; 8% apresentavam CMT2, incluindo 2 pacientes com CMT2A (mutação no gene MFN2); 9.4% apresentavam ARCMT2; 6.6% apresentavam CMT2 esporádica; 1,3% apresentavam uma neuropatia intermediária dominante, que apresentou uma mutação digênica, ou seja, mutação ponto na ser72 associado à duplicação na região 17p11.2-p12.; 2,7% apresentavam ARCMTI; 8% apresentavam CMTI esporádica, incluindo 1 paciente com mutação no LITAF; e 2.7% apresentavam CMTIX (ligada ao sexo), ambos com mutação no gene GJB 1. Analisou-se a velocidade de progressão da neuropatia em cada grupo de CMT através do cálculo da média do CMTNS e da média da duração da neuropatia. Observou-se uma evolução mais lenta nos pacientes com CMT1A, CMT axonal (exceto CMT2A) e CMT intermediária, diferente dos pacientes com CMTI B, CMT4 e CMT2A, nos quais se observou um quadro mais agressivo. Entre os 7 pacientes com HNPP, 6 apresentaram a característica deleção na região 17p11.2-p12 do gene PMP22. Em um paciente não foi possível a realização do teste molecular. Entre os 27 pacientes com neuropatias complexas, inclusas entre as hereditárias, estabeleceu-se um diagnóstico definitivo em 7 pacientes (25,9%). Os diagnósticos definidos em pacientes com neuropatia axonal foram 2 casos de Ataxia de Friedreich e 1 caso de Neurofibromatose tipo 1; nos pacientes com neuropatia intermediária foram 1 caso de Encefalopatia NeuroGastrolntestinal Mitocondrial e caso de 1 Ataxia com Apraxia Oculomotora tipo 1; nos pacientes com neuropatia desmielinizante foi 1 caso de Doença de Krabbe; e nos pacientes com neuronopatia sensitiva foi 1 caso de Neuropatia, Ataxia e Retinite Pigmentar. Dos pacientes com mononeurite múltipla, 1 era portador de Lúpus Eritematoso Sistêmico, 1 de Hanseníase, 1 de Poliangiíte Microscópica e 2 da Síndrome de Lewis-Sumner. Concluindo, nesse estudo descrevemos os aspectos clínicos, eletrofisiológicos e moleculares de uma população de crianças e adolescentes com neuropatia periférica crônica que frequentam um centro de doenças neuromusculares terciário, e relatamos as características interessantes de cada caso.

Epidemiological studies about chronic peripheral neuropathies are very rare. In this study we analyzed 149 unrelated patients refereed to the HCFMRP-USP with a suspect neuropathy in the last 15 years. A total of 77.2% of the cases were classified as hereditary, including 66% with CMT, 23% with a complex neuropathy, 6% with HNPP and 5% with HSAN; 12.1% had an acquired neuropathy, including 61.1 of them with CIDP, 27.8% multiplex mononeuritis and 11.1% a neuropathy associated to diabetes melitus; 9.4% had an undefined neuropathy; and 1.3% had a coexistence of CIDP and CMT, like the series performed in other countries. CMT patients were classified based in inheritance (dominant, recessive, X-linked and sporadic) and each group was sub classified according to upper limb conduction velocity (demyelinating if the conduction velocity ≤30m/s, intermediary if the conduction velocity was between 30-45m/s, and axonal if the conduction velocity was ≤45m/s). We performed molecular tests based in the group in that the patient was classified (PCR for duplication or deletion of the 17p11.2-p12 region, and/or PMP22, MP0, LITAF, EGR2, GJB1 and MFN2 genes sequenciament). Then, among the 75 CMT patients, 54.7% presented CMT1, including 34 with CMT1A (32 duplication and 2 Ser72 point mutation) and 2 CMT1B (MPZ gene mutation), 5.3% presented CMT4; 1.3% presented a sporadic demyelinating neuropathy; 8% presented CMT2, including 2 patients with CMT2A (MFN2 gene mutation); 9.4% presented ARCMT2; 6.6% presented sporadic CMT2; 1.3% presented an intermediary dominant neuropathy, that showed a compound mutation characterized by a Ser72 point mutation associated to a 17p11.2-p12 region duplication; 2,7% presented ARCMTI; 8% presented sporadic CMTI, including 1 patient with LITAF mutation; and 2.7% presented CMTIX (sex-linked), both with GJB1 gene mutation. We analyze the neuropathy progression in each CMT group by estimating the mean of CMTNS and mean duration of the disease. CMT1A, axonal CMT (except CMT2A) and intermediary CMT patients presented a benign evolution when compared to CMT1B, CMT4 and CMT2A, which presented a more aggressive disease evolution. Among the HNPP patients, six presented the characteristic 17p11.2-p12 region deletion. In one patient it was not possible to perform the molecular test. Among the 27 patients with a complex hereditary neuropathy, the specific diagnosis was performed in 7 (25,9%). In the axonal neuropathy group there were two cases of Friedreich Ataxia and one case of neurofibromatosis type 1; In the intermediary neuropathy group there were one case of MNGIE and one probable case of AOA1; In the demyelinating group there was one case of Krabbe disease; In the sensory axonal neuropathy group there was one case of Neuropathy, Ataxia and Retinitis Pigmentosa. Among the multiple mononeuritis neuropathy group one had neuropathy associated to Systemic Lupus Erythematosus, one neuropathy associated to Mycobacterium Leprae, one Microscopic Polyangiitis with associated CNS manifestations and two had Lewis-Sumner Syndrome. In conclusion, in this study we describe the clinical, electrophysiological and molecular characteristics of a children and adolescent population with a chronic peripheral neuropathy that frequent a tertiary neuromuscular disease center, and relate the interesting aspects of each case.
