Introdução: A resposta metabólica secundária ao trauma térmico agudo acarreta estresse inflamatório com resposta inflamatória sistêmica (SIRS), sepse, choque e síndrome da disfunção de múltiplos órgãos e sistemas (SDMOS). A diminuição dos lipídeos séricos secundária à queimadura implica em pior prognóstico, e embora seja recomendado de suplementar lipídeos e ácidos graxos ômega 3 (n-3), são escassos os estudos avaliando suas respectivas ações sobre o estresse inflamatório e no metabolismo lipídico nas vítimas de trauma térmico agudo. Objetivos: Em vítimas de queimaduras agudas, avaliar indicadores bioquímicos nutrológicos e metabólicos, a resposta inflamatória e os níveis de lipídeos após suplementação endovenosa de emulsão lipídica padrão e de ácidos graxos n-3. Casuística e métodos: Conduzido na Unidade de Queimados do HCFMRP-USP, de acordo com as normas éticas, em 18 pacientes, ambos os gêneros, idade ≥16 e ≤70 anos, tempo de internação ≤48 horas do trauma térmico agudo, superfície corporal queimada (SCQ) ≥15% e ≤40%, alocados em três grupos de estudo. No Grupo Controle (GC/n=8) os pacientes não receberam intervenção clínica. No Grupo Lipídeo (GL/n=4), os voluntários receberam uma única infusão endovenosa (EV) de 400mL de emulsão lipídica padrão (ELP), e os voluntários do Grupo Ômega (GO/n=6) receberam 400mL EV de emulsão lipídica padrão acrescida de 100mL de emulsão lipídica de ômega-3 (ELO-3), totalizando a oferta de 500mL de lipídeo por infusão. Esta foi realizada na noite do D5 para o D6 e as análises laboratoriais para avaliação geral, nutrológica e inflamatória foram realizadas nas manhãs do D3, D5, D7 e D10. Para tratamento estatístico foi utilizado o software "SPSS v.17.0. Considerada significância estatística quando valores de p ≤0,05. Resultados: A análise das variáveis não apresentou diferença estatística entre os grupos no D3 e D5. A análise longitudinal evidenciou alterações significativas nos grupos GC e GO-3, sendo que no GC houve aumento de proteínas totais (PT) (p=0,003*) , de albumina (p=0,03*) e de α-1 glicoproteína ácida (α-1 glic.) (p=0,01*) a partir do D3. No GO-3 os níveis de PT foram maiores no D10 (p=0,03*), com aumento de α-1 glic. (p=0,007*) e de ferritina a partir do D7 (p=0,02*). Em todos os grupos houve tendência ao aumento de α-1 glic., sem tendência a alteração nos níveis de glicemia, cortisol, colesterol total, HDL-c, LDL-c e Triglicerídeos. O GC apresentou tendência ao aumento dos níveis de ferritina (p=0,01*); o GO-3 tendência ao aumento de PT (p=0,003*) e albumina (p=0,02*), além de diminuição nos níveis de PCR (p=0,01*). Conclusões: Os dados obtidos evidenciam o estresse inflamatório decorrente do trauma térmico, independente da suplementação EV de lipídeos com ou sem ácidos graxos n-3, mostrando uma tendência ao aumento de alfa-1 glicoproteína ácida em todos os grupos de estudo, porém com tendência ao aumento da ferritina apenas no Grupo Controle. O Grupo Ômega apresentou tendência ao aumento de PT e albumina, além da diminuição da PCR. Tais dados indicam que há evidências benéficas da suplementação de ácidos graxos n-3 em pacientes vítimas de queimadura aguda. Porém, não são suficientes para apontar a necessidade da suplementação EV de ácidos graxos n-3 da forma isolada. São necessários mais estudos que determinem as necessidades de ácidos graxos ômega-3 em pacientes queimados.

Introduction: The metabolic stress response secondary to acute heat trauma causes inflammation with Systemic Inflammatory Response Syndrome (SIRS), sepsis, and shock and Multiple Organ and Systems Dysfunction Sys (SDMOS). The decrease of serum lipids secondary to burn implies a worse prognosis, and although it is recommended to supplement lipids and omega 3 fatty acids (n-3), there are few studies evaluating their respective actions on inflammatory stress and lipid metabolism in victims of acute thermal trauma. Objectives: To evaluate, in acute burn victims, the laboratory evaluation of nutritional and metabolic status, the markers of inflammatory activity and lipid levels after supplementation of intravenous standard lipid emulsion and the lipid n-3 fatty acid emulsion. Methods: Conducted at the Burns Unit of HCFMRP-USP, in accordance with the ethical standards, in 18 patients of both genders, age ≥16 e ≤70 years, lengh of hospital ≤48 hours from acute thermal trauma, Total Burn Surface (TBS) ≥15% and ≤ 40% were divided into three study groups. In the control group (GC/n=8) patients did not receive clinical intervention. In the Lipid Group (GL/n=4), volunteers received a single intravenous infusion (IV) a of standard lipid emulsion (ELP) 400mL. Volunteers from the Omega Group (GO / n = 6) received 400mL IV of a lipid emulsion standard plus 100mL of lipid emulsion of omega-3 fatty acids (ELO-3). This was held on the evening of D5 to D6 and laboratory testing for general assessment and nutritional-inflammatory analysis were held on the mornings of D3, D5, D7 and D10. For statistical analysis we used the software "SPSS v.17.0. Considered statistically significant when p ≤ 0.05. Results: The analysis showed no statistical difference between the groups from D3 and D5. The longitudinal analysis showed significant changes in GC and GO-3, whereas in the control group there was an increase of total protein (TP) (p=0.003*), albumin (p=0.03*) and α-1 acid glycoprotein (α-1 glic.) (p=0.01*) from D3. In GO-3 group, PT levels were higher at D10 (p=0.03*), and there was an increase of α1-gluc. (* P = 0.007) and serum ferritin levels from D7 (p = 0.02*). In all groups there was a tendency to increased α 1-gluc., without changing tendency in blood glucose levels, cortisol, total cholesterol, HDL-C, LDL-C and triglycerides. GC showed a tendency to higher ferritin levels (p=0.01*); GO-3 tendency to PT (p=0.003) and albumin (p=0.02*) increase, and decreased tendency levels of CRP (p=0.01*). Conclusions: The data evidenced the inflammatory stress resulting from thermal injury, regardless of EV lipid supplementation with or without n-3 fatty acids, resulting in higher alpha-1 acid glycoprotein in all study groups, but with a tendency to increased ferritin only in the control group. The Omega Group showed the tendency to increase PT and albumin, and to the decrease the CRP levels. These data indicate that there is evidence of the benefits of supplemental n-3 fatty acids in patients suffering from acute burn. The data obtained in this study are not sufficient to point out the need for supplementation EV fatty acids n-3 alone. However, studies are needed to determine the needs of omega-3 for burn patients.
