Introdução: O transplante hepático é uma alternativa de tratamento que tem como principal objetivo o reestabelecimento das condições de saúde que foram prejudicadas em decorrência da doença hepática, com consequente melhoria da qualidade de vida e aumento da sobrevida do paciente. A vivência do processo do transplante envolve situações contraditórias (perdas e ganhos) que geram implicações psicológicas demandando o desenvolvimento de estratégias de enfrentamento. Objetivos: Caracterizar as estratégias de enfrentamento de pacientes avaliados no pré e pós-operatório de transplante hepático e correlacionar essas estratégias com a presença de sintomas de ansiedade e/ou depressão. Métodos: Participaram do estudo pacientes inscritos na lista de espera para transplante hepático, sendo avaliados 80 pacientes no pré-transplante e 28 pacientes em três períodos do pós-operatório: imediato, três meses e seis. Foram avaliadas as características sociodemográficas e clínicas dos pacientes por meio de questionário semi-estruturado, os aspectos psicológicos (ansiedade e depressão) pela escala de depressão e ansiedade hospitalar (HAD) e as estratégias de enfrentamento através da escala de modos de enfrentamento do problema (EMEP). Resultado e Discussão: A maioria dos pacientes avaliados no pré-operatório foram do sexo masculino (73,8%), com média de idade de 53,8 anos, predomínio de pacientes com baixa renda (61,3%), vínculo afetivo com uma companheira (72,5%), tendo como principal fator etiológico da doença hepática o alcoolismo (37,5%), seguido das hepatites virais (20%), dados que corroboram com a literatura. A maioria estava afastada do trabalho em decorrência da doença hepática. A maior parte dos pacientes pré (73%) e pós (82,1%) transplante não apresentaram sintomas de ansiedade e depressão, índices abaixo dos encontrados na literatura. A estratégia de enfrentamento focada no problema foi a mais prevalente tanto no pré como no pós-transplante, média de pontuação de 3,99 e 4,01 respectivamente. Houve maior utilização da estratégia de enfrentamento focada na emoção quando presentes sintomas de ansiedade e depressão. Na comparação entre os grupos nos momentos pré e pós-transplante, não houve diferença entre os sintomas de ansiedade e depressão e as estratégias de enfrentamento. Conclusão: Houve um predomínio de pacientes de meia idade, do sexo masculino com nível médio de escolaridade e afastados do trabalho em decorrência da doença hepática crônica e suas complicações. A maioria dos pacientes, tanto no pré como no pós-transplante hepático não apresentaram sintomas de ansiedade e depressão. Houve maior utilização da estratégia de enfrentamento focada no problema tanto no pré como no pós-transplante hepático. A estratégia de enfrentamento focada no suporte social foi mais utilizada no pós-transplante imediato em relação ao pré-transplante. Houve correlação positiva entre a presença de sintomas de ansiedade e depressão nos pacientes pré-transplante hepático e a utilização da estratégia centrada na emoção, assim como houve correlação negativa entre a presença de sintomas de depressão e a utilização do enfrentamento focado no problema. Os resultados contribuíram para um melhor entendimento estratégias de enfrentamento utilizadas pelos pacientes que vivenciam o processo do transplante hepático, o que pode auxiliar no planejamento de ações de prevenção e intervenção em relação à saúde mental.

Background: Liver transplantation is an alternative treatment that has as its main goal the reestablishment of the health conditions damaged due to a disease of the liver, with consequent improvement in the quality of life and increased of the survival rate of patients. The experience of transplantation involves contradictory situations (losses and gains) that generate psychological implications, which require the development of coping strategies. Aims: To characterize the coping strategies of patients evaluated in the pre and post-operative period of liver transplantation, and to correlate the coping strategies with the presence of anxiety and/or depression symptoms. Methods: Patients enrolled on the liver transplant waiting list participated in the study, with 80 patients pre-transplant and 28 patients evaluated in three postoperative periods: immediate, three months and six. Patients' sociodemographic and clinical characteristics were assessed using a semi-structured questionnaire, psychological aspects (anxiety and depression) using the hospital anxiety and depression scale (HAD) and coping strategies using the problem coping scale (EMEP). Results and Discussion: Most of the patients evaluated in the preoperative period were male (73,8%), with a mean age of 53.8 years, predominance of patients with low income (61.3%), affective bond with a partner (72,5%), with alcoholism (37.5%) as the main etiological factor of liver disease, followed by viral hepatitis (20%), data that corroborate the literature. Most were away from work due to liver disease. Most pre (73%) and post (82.1%) transplant patients did not show symptoms of anxiety and depression, rates below those found in the literature. The coping strategy focused on the problem was the most prevalent in both pre and post-transplantation, with an average score of 3.99 and 4.01 respectively. There was greater use of the coping strategy focused on emotion when symptoms of anxiety and depression were present. In the comparison between the groups in the pre and post-transplant moments, there was no difference between the symptoms of anxiety and depression and the coping strategies. Conclusion: There was a predominance of middle-aged male patients with a medium level of education and away from work due to chronic liver disease and its complications. Most patients, both before and after liver transplantation, did not show symptoms of anxiety and depression. There was greater use of the coping strategy focused on the problem both before and after liver transplantation. The coping strategy focused on social support was more used in the immediate post-transplant than in the pre-transplant. There was a positive correlation between the presence of symptoms of anxiety and depression in pre-liver transplant patients and the use of the emotion-centered strategy, as well as a negative correlation between the presence of symptoms of depression and the use of coping focused on the problem. The results contributed to a better understanding of coping strategies used by patients who experience the liver transplantation process, which can assist in the planning of prevention and intervention actions in relation to mental health.
