Introdução: A hérnia diafragmática congênita (HDC) consiste em um defeito na formação do diafragma que resulta na passagem de vísceras abdominais para a cavidade torácica. A incidência é de 1:2500-4000 nascidos vivos e sua elevada morbimortalidade está relacionada à hipoplasia e à hipertensão arterial pulmonar, cujos mecanismos ainda são pouco compreendidos e tornam o cuidado neonatal um grande desafio. Portadores de HDC apresentam um curto período de estabilidade ventilatória pós-natal, com oxigenação sistêmica adequada, chamado de período de lua de mel (honeymoon). Em seguida, há deterioração progressiva da oxigenação com piora da hipertensão pulmonar. Acredita-se que alterações da sinalização mediadas pelo VEGF, VEGFR-1/2 e pela eNOS levam à adaptação errática da circulação pulmonar neonatal e resultam em alterações da reatividade vascular, com consequente hipertensão pulmonar. Objetivo: Avaliar o efeito da ventilação mecânica durante o período de lua de mel na morfometria e na expressão de VEGF, VEGFR-1/2 e eNOS na HDC experimental em ratos. Material e Métodos: Filhotes de ratas Sprague Dawley foram separados em cinco grupos: controle (C), controle respirando (CR), controle ventilado (CV), hérnia diafragmática congênita (HDC) e hérnia diafragmática congênita ventilado (HDCV). Para os grupos com HDC, as ratas prenhes foram expostas a 100 mg de nitrofen no dia gestacional (DG) 9,5. Os grupos CR, CV e HDCV foram subdivididos em tempos ventilatórios de 30, 60 e 90 minutos (n=7-8 cada). No DG 21,5, os neonatos foram coletados e, após a ventilação (grupos ventilados) e a pesagem (grupos não ventilados), foram sacrificados. Os pulmões de n=4 neonatos de cada grupo foram submetidos à análise do parênquima (fração de espaço aéreo) e da vasculatura pulmonar (espessura da camada média), além da avaliação imuno-histoquímica (IHQ) (n=4) e por western blotting (WB) (n=3-4) da expressão de VEGF, VEGFR-1/2 e eNOS. A análise estatística foi realizada com diferenças significativas para p<0,05 usando ANOVA one-way com pós-teste de Tukey. Resultados: Os grupos com HDC apresentaram menores parâmetros biométricos que os grupos controle. A fração de espaço aéreo do grupo HDC foi menor do que todos os demais grupos, e os grupos ventilados apresentaram valores semelhantes independente do tempo ventilatório. Quanto à vasculatura pulmonar, os controles apresentaram redução da espessura da camada média, enquanto os neonatos com HDC tiveram aumento desse parâmetro com a progressão do tempo ventilatório (período de lua de mel). Os grupos controle e os grupos com HDC apresentaram diferentes padrões de expressão de VEGF, VEGFR-1/2 e eNOS. Os achados de expressão dos receptores e eNOS foram significativos à IHQ mas não ao WB, enquanto o VEGF foi significativo ao WB mas não à IHQ. Conclusão: Houve alteração da reatividade vascular pulmonar e da expressão de VEGF, VEGFR-1/2 e eNOS na HDC em ratos durante o período de lua de mel, o que sugere um papel importante da via VEGF-NO nesse período.

Introduction: Congenital diaphragmatic hernia (CDH) consists of a defect in the diaphragm formation that results in abdominal viscera passing into the chest cavity. The incidence is 1:2500-4000 live births and its high morbidity and mortality are related to lung hypoplasia and pulmonary arterial hypertension. The mechanisms implicated in their pathogenesis are still poorly understood, which results in a significant challenge for neonatal care. CDH patients have a short postnatal ventilatory stability period with adequate systemic oxygenation, called the honeymoon period. Afterward, there is a progressive deterioration of oxygenation with worsening pulmonary hypertension. Alterations in signaling mediated by VEGF, VEGFR-1/2 and eNOS might lead to erratic adaptation of the neonatal pulmonary circulation and result in vascular reactivity changes, with consequent pulmonary hypertension. Objective: To evaluate mechanical ventilation's effect on the morphometry and VEGF, VEGFR-1/2 and eNOS expressions during the honeymoon period in experimental HDC. Material and Methods: Sprague Dawley neonates were separated into five groups: control (C), breathing control (BC), ventilated control (VC), congenital diaphragmatic hernia (CDH) and ventilated congenital diaphragmatic hernia (VCDH). For the CDH groups, pregnant rats were exposed to 100 mg of nitrofen on the gestational day (GD) 9.5. BC, VC and VCDH groups were subdivided into ventilatory times of 30, 60 and 90 minutes (n=7-8 each). In GD 21.5, neonates were harvested and, after ventilation (ventilated groups) and weighing (non-ventilated groups), they were sacrificed. The lungs of n=4 neonates from each group were submitted to analysis of lung parenchyma (airspace fraction) and pulmonary vasculature (media wall thickness), in addition to immunohistochemical evaluation (IHC) (n=4) and western blotting (WB) (n=3-4) of VEGF, VEGFR-1/2 and eNOS expressions. Statistical analysis was performed with significant differences for p<0.05 using one-way ANOVA with Tukey's post-test. Results: The groups with CDH showed lower biometric parameters than the control groups. The airspace fraction in the CDH group was lower than all the other groups, and the ventilated groups showed similar values regardless of ventilatory time. As for the pulmonary vasculature, the controls showed a reduction of the media wall thickness, while neonates with CDH presented an increase of this parameter with the progression of ventilatory time (honeymoon period). Control groups and groups with CDH showed different expression patterns of VEGF, VEGFR-1/2 and eNOS. The findings regarding receptors and eNOS were significant for IHC but not for WB, while VEGF was significant for WB but not for IHC. Conclusion: There was a change in pulmonary vascular reactivity and in the expression of VEGF, VEGFR-1/2 and eNOS in experimental CDH during the honeymoon period, which suggests an important role of the VEGF-NO pathway during this period.
