INTRODUÇÃO: O tratamento endovascular está associado a menor morbimortalidade que o tratamento convencional para o tratamento do AAA, no entanto, necessita de seguimento com exames de imagem para confirmação da exclusão do saco aneurismático ao longo do tempo. A melhoria das técnicas de imagens, principalmente tomografia, ressonância magnética e PET CT, possibilitou a obtenção de maior quantidade de informações obtidas com essas imagens. Isso levou ao entendimento das imagens médicas como repositórios de informações de alta dimensionalidade (higher-dimensional data), criando um novo conceito definido por Radiomics. Um dos parâmetros utilizados nesses processos é a análise da textura pelo método de matriz de co-ocorrência de nível de cinza. OBJETIVO: Avaliação da textura dos trombos dos AAAs em pacientes submetidos à correção endovascular correlacionando os dados como possíveis preditores de evolução clínica. MATERIAL E MÉTODOS: Vinte (20) pacientes tratados consecutivamente do ambulatório de Cirurgia Vascular e Endovascular do HCFMRP-USP foram submetidos à correção endovascular de AAA conforme indicação clínica. Estes pacientes realizaram exames de vigilância (tomografia computadorizada) em três momentos: pré-operatório e após o procedimento cirúrgico (um e seis meses), objetivando a investigação de vazamentos e/ou alterações na morfometria do saco aneurismático. Foram analisadas as texturas dos trombos nas tomografias de seis meses desses pacientes, correlacionando-a com as alterações na morfometria e evolução clínica de cada paciente. RESULTADO: Dentre aqueles que apresentaram crescimento, um (11%) era do sexo feminino e oito (89%) do sexo masculino. Os diâmetros dos aneurismas destes, antes da cirurgia, variavam entre 56.1 a 97,5 milímetros, com média de 67,7 milímetros. Os diâmetros dos aneurismas após a cirurgia variavam entre 59,4 a 110,3 milímetros, com média de 74,6 milímetros. O aumento do diâmetro do aneurisma entre a cirurgia e a tomografia de seis meses foi 2,6 a 19,1 milímetros, com média de 6,9 milímetros. Obteve-se então uma sensibilidade de 77%, especificidade 63% e acurácia de 70% através da analise da textura. CONCLUSÃO: O método proposto de análise das imagens mostrou-se promissor com uma correlação satisfatória entre a textura e a evolução do saco aneurismático, porém mais estudos com maior número de casos são necessários antes que se possa recomenda-lo na incorporação desta variável nas analises de imagem das angiotomografias em pós-operatório do tratamento endovascular do AAA.

INTRODUCTION:. Endovascular treatment is associated with lower morbidity and mortality than conventional treatment; however, it requires follow-up with imaging studies to confirm the exclusion of the aneurysmal sac over time. The improvement of imaging techniques, especially tomography, magnetic resonance imaging and PET CT, made it possible to obtain more information obtained from these images. This led to the understanding of medical images as repositories of higher-dimensional data, creating a new concept that defines itself as Radiomics. One of the parameters used in these processes is texture analysis by the gray level co-occurrence matrix method. OBJECTIVE: To evaluate the texture of AAA thrombi in patients undergoing endovascular correction, correlating the data as possible predictors of clinical evolution. MATERIAL AND METHODS: Twenty (20) consecutive patients treated at the HCFMRP-USP Vascular and Endovascular Surgery Outpatient Clinic and submitted to endovascular AAA correction as clinically indicated. These patients underwent surveillance examinations (computed tomography) at three times: preoperatively and after the surgical procedure (one and six months), aiming to investigate leakage and / or changes in aneurysmal sac morphometry. Thrombus textures in the six-month CT scans of these patients were analyzed, correlating it with changes in morphometry and clinical evolution of each patient. RESULTS: Among those with growth, one (11%) was female and eight (89%) male. The aneurysm diameters before surgery, of these, ranged from 56.1 to 97.5 mm, with a mean of 67.7 mm. Aneurysm diameters after surgery ranged from 59.4 to 110.3 mm, with an average of 74.6 mm. The increase in aneurysm diameter between surgery and sixmonth tomography was 2.6 to 19.1 mm, with an average of 6.9 mm. A sensitivity of 77%, specificity 63% and accuracy of 70% were obtained. CONCLUSION: The proposed method of image analysis has shown promise with a satisfactory correlation between texture and evolution of the aneurysmal sac, but more studies with a larger number of cases are needed before it can be recommended to incorporate this variable in the analyzes. image of postoperative angiotomographs of endovascular treatment of AAA.
