A rotina de exames para o diagnóstico de infertilidade masculina não sofreu alterações nos últimos vinte anos e os atuais testes genéticos disponíveis são capazes de determinar a etiologia de apenas 4% dos pacientes inférteis não selecionados, portanto, para criar novos testes diagnósticos é necessária a melhor compreensão dos mecanismos moleculares e genéticos da infertilidade masculina. Embora o sequenciamente de nova geração tenha permitido a análise simultânea de centenas de genes, poucos genes candidatos possuem evidência suficiente para serem definitivamente associados à infertilidade masculina. Esse trabalho teve como objetivo realizar o sequenciamento completo do exoma de 16 pacientes brasileiros portadores de azoospermia não obstrutiva e analisar um conjunto de genes previamente associados à infertilidade masculina, em busca de variantes causais. Foram selecionados 37 genes associados aos fenótipos apresentandos de azoospermia não obstrutiva, aplasia de células germinativas e parada de maturação de células germinativas. As variantes encontradas foram confirmadas pelo sequenciamento direto de Sanger e a sua patogenicidade foi prevista por programas de análise in silico. A seleção racional de genes permitiu a detecção de variantes raras e potencialmente patogênicas em seis pacientes. Duas variantes, c.671A> G em DMRT1 e c.91C> T no gene REC8, foram previamente descritas em pacientes azoospérmicos. Também encontramos novas variantes em genes com evidência moderada de associação a defeitos na espermatogênese (TEX15, KLHL10); em genes com evidência limitada (DNMT3B, TEX14) e em um gene ainda sem evidência de associação à infertilidade (SYCE1L). Adicionalmente este trabalho visou estabelecer um protocolo para realização do estudo funcional de variantes identificadas nos genes NR5A1 e DMRT1. Demonstramos que o ensaio da luciferase, tal como descrito para o gene NR5A1, pode ser aplicado para análise do gene DMRT1 humano, apesar de ainda não ser possível tirar conclusões sobre o efeito das variantes selecionados sobre a expressão dos genes analisados.

Understanding molecular and genetic mechanisms of male infertility is urgent, in order to create new diagnostic assays, once the routine diagnostic work-up have not changed over the last twenty years and the current available genetic tests are able to determine the etiology of only 4% of unselected infertile patients. The aim of this study was to perform the whole exome sequencing of 16 Brazilian patients with non-obstructive azoospermia searching for causal variants in a set of genes previously associated with male infertility. We selected 37 genes associated with non-obstructive azoospermia, germ cell aplasia and maturation arrest of germ cells. The variants we found were confirmed by Sanger sequencing and their pathogenicity was predicted by in silico programs. The rational selection of genes allowed the detection of rare and potentially pathogenic variants is six patients. Two variants, c.671A> G in DMRT1 and c.91C> T in the REC8 gene, were previously described in azoospermic patients. We also found new variants in genes with moderate evidence of association with defects in spermatogenesis (TEX15, KLHL10); in genes with limited evidence (DNMT3B, TEX14) and in a gene without evidence so far (SYCE1L). In addition, this work aimed to establish a protocol to perform the functional study of variants identified in the NR5A1 and DMRT1 genes. We demonstrated that the luciferase assay, as it is described for the NR5A1 gene, can be applied for analysis of the human DMRT1 gene, although it was not possible to draw conclusions about the effect of the selected variants on the expression of the genes analysed.
