A disfunção erétil (DE) é definida como uma incapacidade constante de alcançar e manter a ereção suficiente para uma relação sexual satisfatória. Vários fatores estão envolvidos no risco e progressão da DE, como aumento do estresse oxidativo e redução da produção de óxido nítrico (NO). O eixo PI3K-AKT pode afetar várias vias de sinalização. Destacamos a fosforilação da eNOS-nNOS, ativando essas duas enzimas e aumentando a produção de NO. Estudos mostraram o envolvimento da PI3K em doenças cardiovasculares. Como o papel do óxido nítrico nessas doenças é claro, nos faz pensar que a via da PI3K pode ter grande relevância celular e fisiológica no contexto da DE. Portanto, o objetivo do estudo foi verificar a associação dos polimorfismos genéticos nos genes PIK3CA (rs6443624, rs9838411, rs2699887) PIK3CG (rs342286, rs17398575 e rs1129293) com a disfunção erétil e com os níveis plasmáticos de nitrito. Foram incluídos 288 indivíduos (139 pacientes com DEC e 149 voluntários saudáveis) do ambulatório de urologia da Faculdade de Medicina de Ribeirão Preto. Todos os sujeitos tiveram sangue coletado para testes genéticos e foram entrevistados. A função erétil foi avaliada pelo Questionário do Índice Internacional de Função Erétil (IIEF). A genotipagem foi realizada pela Reação em Cadeia da Polimerase em Tempo Real. A análise estatística foi realizada por meio de testes qui-quadrado ou teste de Kruskall-Wallis, seguido pelo teste post-hoc de Dunn. Para o gene PIK3CA, o haplótipo AGG (P=0,004) apresentou aumento de risco para DE. No rs9838411, os pacientes com DEC portadores do genótipo GG apresentaram melhora no score FE do IIEF (P=0,022), resultado confirmado na análise de regressão linear. No rs6443624, também foi possível observar que os pacientes DEC com a presença do genótipo CC apresentaram uma melhora no score FE do IIEF (P=0,027). No gene PIK3CG, apenas o haplótipo AAC (PNC=<0,001) os pacientes DEC apresentou um maior risco para DE. No rs17398575 (P=0,026), os pacientes DEC portadores dos genótipos GG/GA apresentaram valores menores de nitrito, resultados confirmados nas análises de regressão linear multivariada. Ainda para o gene PIK3CG no rs1129293 (P = 0,062), o grupo controle, os indivíduos saudáveis portadores do genótipo CC/CT apresentaram valores maiores de nitrito. Foi possível concluir que polimorfismos genéticos no gene PIK3CA (rs6443624 e rs9838411) estão associados a função erétil, enquanto polimorfismos genéticos no gene PIK3CG (rs17398575 e rs1129293) estão associados aos níveis plasmáticos de nitrito.

Erectile dysfunction (ED) is defined as a constant inability to achieve and maintain an erection sufficient for satisfactory sexual intercourse. Several factors are involved in the risk and progression of ED, such as increased oxidative stress and reduced production of nitric oxide (NO). The PI3K-AKT axis can affect multiple signaling pathways. We highlight the phosphorylation of eNOS-nNOS, activating these two enzymes and increasing NO production. Studies have shown the involvement of PI3K in cardiovascular disease. As the role of nitric oxide in these diseases is clear, it makes us think that the PI3K pathway may have great cellular and physiological relevance in the context of ED. Therefore, the aim of the study was to verify the association of genetic polymorphisms in the PIK3CA genes (rs6443624, rs9838411, rs2699887) PIK3CG (rs342286, rs17398575 and rs1129293) with erectile dysfunction and with plasma levels of nitrite. It included 288 subjects (139 patients and 149 healthy volunteers DEC) of the urology clinic at the Ribeirão Preto Medical School. All subjects had blood drawn for genetic testing and were interviewed. Erectile function was assessed using the International Index of Erectile Function Questionnaire (IIEF). Genotyping was performed by Polymerase Chain Reaction in Real Time. Statistical analysis was performed using the chi-square test or the Kruskall-Wallis test, followed by Dunn's post-hoc test. For the PIK3CA gene, the AGG haplotype showed an increased risk for ED(P=0.004). Regarding rs9838411, patients with CED with the GG genotype showed an improvement in the IIEF FE score (P=0.022), a result confirmed in the linear regression analysis. The rs6443624 CC genotype was also associated with an improvement in the IIEF FE score(P=0.027),. In the PIK3CG gene, only the AAC haplotype (PNC=<0.001) CED patients presented a higher risk for ED. Regarding rs17398575, DEC patients carrying the GG/GA genotypes had lower nitrite values(P=0.026), results confirmed in multivariate linear regression analyses. Also for the PIK3CG gene in rs1129293 (P = 0.062), the control group, healthy individuals carrying the CC/CT genotype had higher nitrite values. It was possible to conclude that genetic polymorphisms in the PIK3CA gene (rs6443624 and rs9838411) are associated with erectile function, while genetic polymorphisms in the PIK3CG gene (rs17398575 and rs1129293) are associated with plasma nitrite levels.
