Desconhece-se, até o momento, o efeito funcional de medicamentos anticolinérgicos de longa ação, em pacientes com fibrose cística (FC), apesar do seu uso habitual, com segurança e eficácia, em pacientes com doença pulmonar obstrutiva crônica (DPOC). OBJETIVO: Este estudo teve por objetivo avaliar a resposta funcional aguda ao brometo de tiotrópio em crianças e adolescentes com fibrose cística. MÉTODOS: O desenho da pesquisa foi transversal, prospectivo, duplo cego, randomizado, cruzado e placebo controlado. Foram incluídos 20 pacientes, 10 do sexo masculino, com doença estável, na faixa etária dos 6 aos 20 anos (média: 12,7anos), fora de agudização pulmonar, com VEF1 acima de 40% na condição basal (média de 70,4% ± 14,4 % do previsto), aptos para inalar medicamentos na forma de pó, randomizados através de uma tábua de randomização aleatória para receber brometo de tiotrópio (A) ou placebo(B). Foram obtidos parâmetros espirométricos e pletismográficos, seguindo as normas da American Thoracic Society: capacidade vital forçada (CVF), volume expiratório forçado no primeiro segundo (VEF1), relação VEF1/CVF, fluxo médio expiratório entre os 25 e 75% da CVF (FEF 25-75%), capacidade pulmonar total (CPT), volume residual (VR), relação VR/CPT, condutância e resistência das vias aéreas e capacidade inspiratória (CI). Todos os parâmetros foram medidos na condição basal e 30, 60, 180 minutos e 24h após a inalação da droga ou placebo. Após sete dias, os medicamentos foram invertidos (se o paciente recebia A recebia B e vice-versa) e foram executados e repetidos os mesmos parâmetros funcionais basais e nos mesmos momentos que os testes anteriores. Como desfecho principal foram analisadas as mudanças significativas nos parâmetros espirométricos e de pletismografia em cada tempo observado a partir dos valores basais no grupo tiotrópio e as diferenças significativas, nos diferentes momentos, em relação ao grupo placebo. A análise estatística foi realizada pelo teste de análise de variância para medidas repetidas (ANOVA), teste de Tukey- Krumer e teste de T de Student. RESULTADOS: Com relação à comparação das médias percentuais dos valores basais em relação ao previsto com as medidas sucessivas nos diferentes momentos da avaliação, houve aumento significante do FEF 25-75% no momento 24h pós tiotrópio (p = 0,0397); não houve variação significantes nos parâmetros CVF ,VEF1, relação VEF1/CVF e na CI. Houve redução significante do VR nos momentos 180 min (p = 0,0122) e 24h (p = 0,0229), da CPT no momento 24h (p = 0,0468) e da relação VR/CPT nos momentos 180 min (p = 0,0119) e 24h (p = 0,0395). Houve um aumento significante da condutância das vias aéreas em todos os momentos da avaliação (p < 0,05) e redução significante da resistência nos momentos 180 min (p = 0,0261) e 24h (p = 0,049). Na comparação das médias percentuais dos valores previstos do grupo tiotrópio em relação ao placebo, nos diferentes momentos, houve aumento significante na CVF (p = 0,0469); VEF1 (p = 0,0462) e no FEF 25-75% (p = 0,007); redução no VR (p < 0,0001), na CPT (p = 0,0468) e na relação VR/CPT (p = 0,0005); aumento da condutância das vias aéreas nos momentos 180 min (p = 0,0495) e 24h (p = 0,0013) e redução da resistência no momento 24h (p = 0,0213). Não houve efeitos adversos significantes pelo uso do tiotrópio. CONCLUSÕES: O uso do tiotrópio, nesta casuística, melhorou agudamente a obstrução de pequenas e grandes vias aéreas, aumentou a condutância e reduziu significativamente a resistência, aprisionamento aéreo e a hiperinsuflação pulmonar. São necessários novos estudos para avaliar o impacto do tiotrópio em longo prazo, na função pulmonar e na melhora da dispnéia, no consumo de oxigênio e na qualidade de vida de pacientes com fibrose cística.

The functional effect of long-acting anticholinergic agents in patients with cystic fibrosis (CF) is still unknown, despite the fact that this drug is frequently used, safe and effective, in patients with chronic obstructive pulmonary disease (COPD). OBJECTIVE: The objective of this study is to evaluate the acute functional response to tiotropium bromide in children and teenagers with cystic fibrosis. METHODS: The study design was transversal, prospective, double-blind, randomized, cross-over and placebo-controlled. Twenty CF patients were included, 10 male patients, with stable disease, age range 6 to 20 years (average: 12.7 years) without pulmonary exacerbations, with an FEV1 above 40% in the baseline condition (average of 70.4% ± 14.4% of the predicted), able to inhale medication in powder form, randomized using a random board to receive tiotropium bromide (A) or placebo (B). The spirometric and pletismographic parameters were obtained following the American Thoracic Society recommendations: forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1/CVF ratio, average peak expiratory flow between 25 and 75% of the FVC (FEF 25-75%), total lung capacity (TLC), residual volume (RV), RV/TLC ratio, conductance and resistance of the airways and inspiratory capacity (IC). All the parameters were measured in the basal condition and at 30, 60, 180 minute intervals and 24 hours after the inhalation of the drug or placebo. After seven days, the medications were inverted (if the patient was receiving A, they then received B, and vice-versa) and the same basal and functional parameters were performed and repeated at the same time intervals as in the previous tests. The main outcome measures were the analysis of the significant changes in the spirometric and pletismographic parameters at each time intervals observed compared to the basal values in the tiotropium group and also the significant differences, at the different time intervals, in relation to the placebo group. The statistical analysis was performed using the test of variance analysis for repeated measures (ANOVA), the Tukey- Krumer test and the T-Student test. RESULTS: Regarding the comparison of the percentile averages of the basal values in relation to the predicted results with the successive measurements at the different time intervals of the evaluation, there was a significant increase of FEF 25-75% at the 24-hour time interval after the tiotropium (p = 0.0397); there was no significant variation in the parameters FVC, FEV1, FEV1 / FVC ratio and the IC. There was a significant reduction of RV at the time intervals: 180 min (p = 0.0122) and 24 hours (p = 0.0229), of the TLC at the 24-hour time interval (p = 0.0468) and of the RV/TPC ratio at 180 min (p = 0.0119) and 24 hours (p = 0.0395). There was a significant increase of the conductance of the airway in all the time intervals of the evaluation (p <0.05) and a significant reduction of the resistance at 180min (p = 0.0261) and 24 hours (p = 0.049). The mean percentage’s values in relation to predicted for the tiotropium compared to the placebo group, at different time intervals, showed a significant increase in the FVC (p = 0.0469); FEV1 (p = 0.0462) and in FEF 25-75% (p = 0.007); reduction in RV (p <0.0001), in TLC (p = 0.0468) and in the RV/TLC ratio (p = 0.0005); increase of the conductance of the airways at 180min (p = 0.0495) and 24 hours (p = 0.0013) and reduction of resistance at the 24-hour time interval (p = 0.0213). There were no significant side-effects using tiotropium. CONCLUSIONS: The use of tiotropium, in this casuistic, improved the obstruction of small and large airways acutely, increased the conductance and reduced the resistance and significantly reduced the air trapping and the pulmonary hiperinsuflation. New studies are required to evaluate the impact of long-term tiotropium use in lung function and in the improvement of the dyspnea, oxygen consumption and the quality of life of patients with cystic fibrosis.
