A estimulação elétrica neuromuscular (EENM) tem sido considerada uma modalidade coadjuvante no tratamento das disfagias orofaríngeas. Muitas questões existem a esse respeito, incluindo candidatos apropriados, localização dos eletrodos, amplitude de estimulação e mudanças fisiológicas resultantes da aplicação. Este trabalho teve por objetivo verificar o efeito imediato de distintas amplitudes de EENM na fisiologia da deglutição em indivíduos saudáveis, considerando diferentes volumes e consistências de alimentos, bem como o processo de envelhecimento. Para isso foi aplicada estimulação sensorial, motora e ausente, comparando jovens e idosos. Foram avaliados 34 indivíduos, sendo 20 jovens (20 a 30 anos) e 14 idosos (60 a 79 anos). Cada participante foi solicitado a deglutir líquido fino, líquido engrossado e pudim em volumes de 5ml, 10ml e 20ml em cada um dos três níveis de EENM. Medidas fisiológicas incluíram pressão da língua contra o palato duro, manometria faríngea e função respiratória. Os dados quantitativos foram submetidos à análise multivariada a quatro critérios, bem como à análise de variância a dois critérios, seguidas do teste de Tukey, enquanto às variáveis qualitativas foi aplicado o teste qui-quadrado. Os resultados demostraram que diferentes níveis de EENM diminuíram as pressões da região anterior da língua para jovens e idosos, enquanto as pressões medidas em região média e posterior foram reduzidas no grupo de jovens. Para os idosos a função em orofaringe e hipofaringe foi influenciada pelo nível motor de EENM, enquanto o esfíncter esofágico superior respondeu à estimulação sensorial. Por fim, o nível motor resultou em aumento da duração da apnéia da deglutição em ambos os grupos. Estes achados indicaram que diferentes níveis de EENM influenciaram de forma distinta as fases oral e faríngea da deglutição. A influência da idade dos participantes, volume e consistência do alimento na fisiologia da deglutição também foi demonstrada.

The neuromuscular electrical stimulation (NMES) has been considered an adjunctive modality in the treatment of oropharyngeal dysphagia. There are many questions in this regard, including appropriate candidates, location of electrodes, amplitude of stimulation and physiological changes resulting from NMES application. This study aimed to verify the immediate effect of distinct amplitudes of NMES in the physiology of swallowing in healthy subjects, considering different volumes and consistencies of foods, as well as the aging process. For this purpose, sensory, motor and no stimulation were applied, comparing young and old volunteers. A total of 34 subjects were evaluated, being 20 young (20 to 30 years) and 14 elderly (60 to 79 years). Each participant was asked to swallow thin, thickened liquid, and pudding in volumes of 5ml, 10ml, and 20ml in each of the three levels of NMES. Physiological measurements included tongue pressure against the hard palate, pharyngeal manometry and respiratory function. Quantitative data were submitted to four-way multivariate analysis, as well as two-way analysis of variance, followed by Tukey test, while the qualitative variables were analyzed by the chi-square test. The results showed that different levels of NMES decreased anterior tongue pressures in young and old participants, while the pressures measured on middle and posterior tongue regions were reduced in the young group. For elderly participants, the function in the oropharynx and hypopharynx was influenced by the motor level of NMES, while the upper esophageal sphincter responded to sensory stimulation. Finally, the motor level increased the duration of swallowing apnea in both groups. These findings showed that different levels of NMES influenced the oral and pharyngeal phases of swallowing differently. The influence of age of the participants, volume and consistency of food on the physiology of swallowing was also demonstrated.
