A deficiência transversal de maxila caracteriza-se por mordida cruzada posterior uni ou bilateral, palato ogival, dentes apinhados e rotacionados. Seu tratamento, em indivíduos que atingiram a maturidade esquelética, é a expansão de maxila cirurgicamente assistida. Tal procedimento não possui efeitos somente esqueléticos, mas também dentários (especialmente nos dentes pilares do aparelho expansor), cavidade e espaço aéreo nasais, lábio e demais tecidos moles circunjacentes. O objetivo deste trabalho foi avaliar, por meio do método de elementos finitos, a distribuição das tensões que incidem nos dentes suportes do aparelho expansor dento-muco-suportado, produzidas pela expansão de maxila cirurgicamente assistida, quando variou-se as osteotomias. Foram construídos virtualmente 5 modelos de maxila: M1 - modelo hígido; M2 - Osteotomia Le Fort I com degrau no pilar zigomático-maxilar; M3 - Osteotomia Le Fort I com degrau no pilar zigomático-maxilar e disjunção ptérigo-maxilar; M4 - Osteotomia Le Fort I descendente sem degrau e M5 – Osteotomia Le Fort I descendente sem degrau e com disjunção ptérigo-maxilar. Em todos os modelos, com exceção do M1, foram simuladas osteotomias da sutura intermaxilar, realizando a disjunção intermaxilar. Os resultados mostraram que, mesmo com a utilização do aparelho dento-mucosuportado, ocorreu dissipação de tensões de tração principalmente nas regiões palatinas dos dentes e osso alveolar, e de compressão predominantemente na vestibular do osso alveolar e dos dentes. A realização de osteotomia Le fort I subtotal associada a osteotomia da sutura intermaxilar, pareceu reduzir essa dissipação de forças. Concluiu-se que a osteotomia Le Fort I subtotal associada aosteotomia da sutura intermaxilar e com a disjunção do processo ptérigo-maxilar, deve ser a osteotomia de escolha quando se busca reduzir tensões aos dentes, e que existe tendência dos dentes pilares do aparelho deslocarem-se para mesiovestibular (pré-molar) e disto-vestibular (molar).

The transverse maxillary deficiency is characterized by a posterior crossbite, higharched palate, crowded and rotated teeth. To treat individuals who had reached skeletal maturity, surgically assisted maxillary expansion is necessary.This procedure has not only skeletal effects, but also in the teeth (especially the pillars of the expander), nasal cavity and airspace, lip and other surrounding soft tissues. The purpose of this study was to evaluate, through means of finite element analysis, the distribution of stresses that affect the teeth holders of the expander appliance. Five models were virtually built: M1-healthy model, M2-Le Fort I osteotomy with step in the zygomaticmaxillary buttres, M3-Le Fort I osteotomy with step in the zygomaticmaxillary buttres and pterygomaxillar disjunction, M4-Le Fort I osteotomy without step and M5 - Le Fort I osteotomy without step and pterygomaxillar disjunction. In all models, except for M1, simulated osteotomy of the intermaxillary suture and intermaxillary disjunction were performed. The results showed occurrence of tensile forces in the teeth’s palatine side and alveolar bone and compression on the teeth’s buccal side of the teeth and alveolar bone. The subtotal Le Fort I osteotomy combined with osteotomy of the intermaxilarry suture seemed to reduce the dissipation of tensions. Therefore, subtotal Le Fort I osteotomy combined with osteotomy of the intermaxillary suture and the disjunction of the pterygoid process of maxillary tuberosity, may be the osteotomy of choice when searching to reduce tensions on the teeth. It was a tendency of the abutment teeth of the device move mesio-vestibular (premolar) and disto-buccal (molar).
