A avaliação biatrial é parte integrante da caracterização do Coração de Atleta. A utilização cada vez mais frequente da ressonância magnética de coração (RMC) implica em reconhecimento de padrões de normalidade para essas câmaras cardíacas, bem como a necessidade de estabelecimento de correlação clínica com as medidas encontradas. Para tal fim, objetivou-se realizar a avaliação morfofuncional biatrial em atletas e esportistas corredores de provas de fundo e compará-las com indivíduos sedentários e hígidos da mesma faixa etária e gênero. Foi realizado estudo observacional e retrospectivo, no Instituto Dante Pazzanese de Cardiologia (IDPC) e analisadas ressonâncias magnéticas de coração de 25 atletas e esportistas, do gênero masculino, com idade entre 30 e 60 anos e comparadas com a de 10 sedentários, de mesmo gênero e faixa etária. Para ambos os átrios foram avaliados volumes diastólico final (VDF) e sistólico final (VSF) e fração de ejeção, além das medidas de espessura de septo interatrial (SIA) e diâmetro anteroposterior do átrio esquerdo (AE). A análise da RMC resultou na identificação de relevante alteração das câmaras cardíacas estudadas na comparação entre atletas e sedentários. Em detalhes, foram encontradas diferenças estatísticas nos seguintes parâmetros: VDF de átrio direito (AD) de 105,56 mL ±24,86 para atletas e 69,90 mL ±8,9 para sedentários (p valor<0,001), VSF de AD de 62,72 mL ±21,18 para atletas e 39,10 mL ±7,46 para sedentários (p valor<0,001), VDF de átrio esquerdo (AE) de 82,96mL ±33,25 para atletas e 63,4 mL ±14,15 para sedentários (p valor<0,03) e menores valores de SIA sendo 2,42mm ±0,74 para atletas e 3,21mm ±1,24 para sedentários (p valor<0,04). Não houve diferença na avaliação das demais medidas de câmaras, de função atrial através da fração de ejeção e não foi possível a correlação com dados clínicos. Dessa forma, o presente estudo demonstra importante dilatação biatrial, sendo mais expressiva para o átrio direito na avaliação do coração de atleta pela ressonância magnética de coração, sem presença de prejuízo na função biatrial avaliada pela fração de ejeção.

The biatrial evaluation is a part of the characterization of the athlete's heart. The increasingly frequent use of cardiac magnetic resonance (CMR) involves the recognition of normality patterns for these cardiac chambers, as well as the need to establish a clinical correlation with the measurements found. For this purpose, the main objective was to perform a biatrial morphofunctional evaluation of endurance athletes and compare them with sedentary and healthy individuals of the same age and gender. An observational and retrospective study was conducted at the Dante Pazzanese Institute of Cardiology (DPIC). Cardiac magnetic resonance of 25 male athletes, aged 30 to 60 years, were analyzed and compared to 10 sedentary subjects of the same gender and age. end-diastolic volume (EDV), end systolic (ESV) and ejection fraction were measured for both atria, in addition to measurements of the atrial septal thickness (AST) and anteroposterior left atria (LA) diameter. The CMR indicates a relevant alteration in heart chambers features among athletes compared to sedentary individuals. In details, statistically significant differences were observed in four parameters, as follows: EDV of Right Atria (RA) (105.56±24.86 mL for athletes vs. 69.90±8.9 mL for sedentary subjects (p<0.001)), ESV of RA (62.72±21.18 mL athletes vs. 39.10±7.46 mL sedentary (p<0.001)), EDV of LA (82.96±33.25 mL athletes vs and 63.4±14.15 mL sedentary (p <0.03)), and lower AST values comparison (2.42±0.74 mm for athletes vs 3.21±1.24 mm sedentary (p <0.04)). There was no difference in the evaluation of the other chambers measurements and atrial function, and the markers for biatrial dilatation does not correlate to clinical data. The present study results demonstrate an essential biatrial dilation in athletes detected by CMR. There is a right atrium significant alteration, without the impairment of biatrial function as measured by the ejection fraction parameter. Taken together, the observed alterations do not cause real dysfunction, and can be interpreted as a physiological accommodation of the athlete's heart.
