Introdução: Existem inconsistências ao estudar os achados eletrocardiográficos relacionados à predição de eventos cardíacos. No cenário da fibrilação atrial (FA), a maior duração da onda P, bem como a sua dispersão e variabilidade, estão associados ao maior risco para esta arritmia. Retardos da condução do impulso elétrico atrial favorecem mecanismos reentrantes atriais traduzidos por ondas P de longa duração visualizadas em diferentes derivações eletrocardiográficas. Além das características desta onda, várias publicações associam o bloqueio atrioventricular de primeiro grau com maior risco de FA; porém, sem esclarecimento do seu mecanismo envolvido, e ainda com informações contraditórias. Alguns autores ainda discutem o real valor do aumento do intervalo PR como fator de risco para a FA. Suspeitamos, então, que o percentual da ocupação da onda P dentro desse intervalo poderia ser um fator identificador para o surgimento dessa arritmia. Em estudo recente, foi demonstrado que, quando esse percentual superava 70%, associava-se a uma maior taxa de mortalidade, independente da duração do intervalo PR. Contudo, a causa responsável por este aumento não foi esclarecida. Nos últmos anos, observamos que pacientes com FA paroxística ao Holter de 24 horas apresentavam encurtamento do segmento PR, ou seja, com o final da onda P muito próximo ao início do QRS. Esse fato despertou nosso interesse em avaliar se o percentual da onda P ocupado dentro do intervalo PR poderia ser uma variável útil para predição de risco para a FA. A confirmação desse achado nos daria suporte para o entendimento da maior mortalidade observada no estudo citado anteriormente. Objetivo: O objetivo desse estudo é avaliar se a relação entre a duração da onda P e o intervalo PR (índice P/PRi) seria um preditor de risco para a FA documentada em pacientes submetidos ao Holter de 24 horas. De modo conjunto, avaliar outras variáveis clínicas, eletrocardiográficas e ecocardiográficas, descritas na literatura como preditores de risco para FA. Métodos: Foram incluídos neste estudo 223 pacientes submetidos ao Holter de 24 horas, dos quais 110 apresentavam FA paroxística. De modo padronizado, foram realizadas as aferições das durações da onda P e intervalo PR no canal 2 dos traçados do Holter. As ondas P e o intervalo PR desse canal apresentam cartacterísticas similares à derivação D2 do eletrocardiograma. Com esses dados, foi calculado o índice P/PRi em ambos os grupos. Variáveis clínicas, eletrocardiográficas e ecocardiográficas foram obtidas a partir da análise dos prontuários, e, conjuntamente a este índice, foram utilizadas para caracterizar pacientes com maior risco de apresentar FA. O índice P/PRi variou entre 0,57 e 0,92 (média 0,71), e entre 0,43 e 0,78 (média 0,62) nos grupos FA e controle, respectivamente. Ao se estabelecer um valor de corte do índice P/PRi de 0,69, a sensibilidade e especificidade foi de 52% e 85%, respectivamente. O índice P/PRi pela análise multivariada mostrou-se um bom preditor independente para a FA, com um poder discriminativo de 81%. Quando associado a outras variáveis estudadas (idade, sexo, volume atrial e ectopias supraventriculares por hora), aumentou seu poder discriminativo para 91%. Conclusões: O índice P/PRi apresenta-se como um importante preditor independente para a FA. O poder discriminativo dessa variável aumenta quando associado às variáveis como sexo, idade, volume atrial e densidade de ectopias supraventriculares.

Introduction: There are inconsistencies in studying the electrocardiographic findings related to the prediction of cardiac events. In the scenario of atrial fibrillation (AF), the longer duration of the P wave, as well as its dispersion and variability, are associated with a higher risk for this arrhythmia. Delays in conduction of the atrial electrical impulse favor atrial reentrant mechanisms translated by long-term P waves visualized in different electrocardiographic leads. In addition to the characteristics of this wave, several publications associate first-degree atrioventricular block with an increased risk of AF; however, without clarifying its mechanism involved, and with contradictory information. Some authors still discuss the real value of increased PR interval as a risk factor for AF. We suspect that the percentage of P wave occupancy within this interval could be an identifiable factor for the onset of this arrhythmia. In a recent study, it was shown that, when this percentage exceeded 70%, it was associated with a higher mortality rate, regardless of the duration of the PR interval. However, the cause responsible for this increase has not been clarified. In recent years, we observed that patients with paroxysmal AF subjected to 24-hour Holter monitoring presented shortening of the PR segment, that is, with the end of the P wave very close to the beginning of the QRS. This fact aroused our interest in evaluating whether the percentage of the P wave occupancy within the PR interval could be a useful variable to predict risk for AF. Confirmation of this finding would support the understanding of the higher mortality observed in the study cited previously. Objective: The objective of this study was to evaluate whether the relationship between the duration of the P wave and the PR interval (P/PRi index) would be a predictor of risk for AF documented in patients subjected to 24-hour Holter monitoring. Jointly, to evaluate other clinical, electrocardiographic and echocardiographic variables, which are described in the literature as predictors of risk for AF. Methods: A total of 223 patients subjected to 24-hour Holter monitoring were included in the study, of whom 110 had paroxysmal AF. The P waves and the PR interval of this channel have similar characteristics to the D2 lead of the electrocardiogram. With these data, the P/PRi index was calculated in both groups. Clinical, electrocardiographic and echocardiographic variables were obtained from the analysis of the medical records and, together with this index, were used to characterize patients at higher risk of having AF. The P / PRi index ranged from 0.57 to 0.92 (mean 0.71), and from 0.43 to 0.78 (mean 0.63) in the AF and control groups, respectively. When a P/PRi index cutoff value of 0.69 was established, the sensitivity and specificity were 52% and 85%, respectively. The P/PRi index by multivariate analysis proved to be a good independent predictor of AF, with a discriminative power of 81%. When associated with other variables studied (age, sex, atrial volume and supraventricular ectopies per hour), it increased its discriminative power to 91%. Conclusions: The P/PRi index is an important independent predictor of AF. The discriminative power of this variable increases when associated with variables such as sex, age, atrial volume and density of supraventricular ectopies.
