Introdução: A Obesidade está relacionada ao risco aumentado de numerosas doenças, que podem levar à alteracão da função endotelial, do perfil inflamatório e de aspectos estruturais do coração. O tratamento cirúrgico tem sido a única alternativa para muitos pacientes obesos. Objetivo: Determinar o impacto da perda de peso observada após a cirurgia bariátrica na função endotelial, perfil inflamatório e variáveis ecocardiográficas. Métodos: Estudamos a função endotelial por meio da ultrassonografia de alta resolução, o perfil inflamatório mensurado pela PCR-us e alterações cardíacas avaliadas pelo ecocardiograma com Doppler em 47 pacientes antes e depois de serem submetidos à cirurgia bariátrica. Resultados: O grupo foi contituído por 93,8% de mulheres, com média de 41 anos. O tempo entre a cirurgia e o retorno para reavaliação foi de 10 meses, com perda de 33% de peso neste período (p<0,001). O índice de massa corpórea mediano era de 46,87 e passou para 31,6 (p<0,001). A vasodilatação endotélio-dependente avaliada por meio do ultrassom de alta resolução, mostrou melhora significativa após a cirurgia em população de não diabéticos e sem distúrbios hormonais, que apresentavam valores de média de 7,4% antes da cirurgia, para 18% após o procedimento (p<0,001), com diferença absoluta de 11,4%, mostrando a importância da perda de peso de forma rápida após a cirurgia bariátrica. Houve melhora da função diastólica avaliada pela ecocardiografia em 18 pacientes (72%) que apresentavam disfunção diastólica grau I antes da cirurgia, passaram a ter função diastólica normal no pós-operatório. Ocorreu redução da mediana no espessamento médio-intimal das artérias carótidas de 0,8mm para 0,5mm (p<0,001) após a cirurgia, e também das taxas séricas de PCR-us que passaram da mediana de 0,83mg/dL para 0,18mg/dL (p<0,001).

Introduction: Obesity is related to a high risk of several diseases, which might lead to endothelial dysfunction, inflammatory profile dysfunction as well as structural heart alterations. Surgery has been the only treatment for such patients. Aims: To determine the impact of weight loss after bariatric surgery over the endothelium function, inflammatory profile, and echocardiography parameters. Methods: Endothelial function was assessed by high-resolution ultrasound, inflammatory profile was measured by PCR-us, and heart changes were assessed by Doppler echocardiography in 47 patients before and after bariatric surgery. Results: Forty-seven obese adults were examined before and 10 months after bariatric surgery having lost 33% of their original weight (p<0.001). The BMI (body mass index) was 46.87 (kg/m2) and turned to 31.6 (kg/m2) (p<0.001). Median age of the study population was 41 years old, most of which were women (93%). The flow-mediated dilation assessed by high-resolution ultrasound showed significant improvement after surgery in this non-diabetic population. Measures of FMD (flow-mediated dilation) changed from 7.4% to 18% (p<0.001) after bariatric surgery. The diastolic function was improved in 18 patients (72%), who had level I diastolic dysfunction, as assessed by Doppler echocardiography. There was regression of CIMT (carotid artery intima-media thickness) from 0.8 mm to 0.5mm (p<0.001). The PCR-us rate changed from 0.83mg/dL to 0.18mg/dL (p<0.001).
