No presente estudo, foi investigado o efeito do polimorfismo do gene da apolipoproteína E (APOE) sobre a expressão de RNAm e a resposta a terapia de reposição hormonal combinada ou não com atorvastatina, em 87 mulheres brancas sem vínculo genético em pós-menopausa com hipercolesterolemia. O polimorfismo Hhal (exon 4) do gene APOE foi analisado por PCR-RFLP e a quantificação do RNAm do gene APOE foi medida em células mononucleares do sangue periférico por RT-PCR duplex. As pacientes foram divididas em cinco grupos e tratadas pelo período de três meses com atorvastatina (10 mg/d); estradiol (2 mg/d); atorvastatina e estradiol; e estradiol e acetato de noretisterona (1 mg/d); e atorvastatina, estradiol e acetato de noretisterona. O polimorfismo do gene APOE não foi associado com variações no perfil lipídico sérico e na expressão de RNAm basais. Todos tratamentos reduziram significativamente as concentrações séricas de colesterol total e LDL-colesterol, independentemente do polimorfismo do gene APOE. A expressão do RNAm do gene APOE foi reduzida após os tratamentos com atorvastatina e com atorvastatina e estradiol. Esse efeito foi significativo nas mulheres portadoras de genótipo E3/E3 (p=0,025). Foi observada correlação positiva entre as variações de expressão do RNAm do gene APOE e de concentrações de colesterol total e LDL-colesterol após o tratamento com estradiol. Em conclusão, a expressão de RNAm do gene APOE em células mononucleares do sangue periférico foi reduzida pelo tratamento com atorvastatina e/ou estradiol. Esse efeito foi associado ao polimorfismo do gene APOE e à diminuição do colesterol sérico durante a terapia de reposição hormonal.

In the present study, the effect of the polymorphism at the apolipoprotein E gene (APOE) on mRNA expression and response to hormone replacement therapy (HTR) and atorvastatin was investigated in 87 unrelated postmenopausal women with hypercholesterolemia. The APOE Hhal (exon 4) polymorphism was analyzed by PCR-RFLP and mRNA expression was evaluated in mononuclear leukocytes by duplex RT-PCR. The patients were divided into five groups and treated during three months with atorvastatin (10 mg/day); estradiol (2 mg/day); atorvastatin and estradiol; estradiol and norethisterone acetate (1 mg/day); e atorvastatin, estradiol and norethisterone acetate. The APOE polymorphism was not associated with variations on basal lipids and mRNA expression levels. The treatments reduced significantly serum total and LDL cholesterol levels, without relation to the APOE polymorphism. The APOE mRNA expression was reduced after treatment with atorvastatin and atorvastatin combined with estradiol. This effect was significant in women carrying E3/E3 genotype (p=0,025). There was a positive correlation between the APOE mRNA expression and serum total and LDL cholesterol levels after treatment with estradiol. In conclusion, APOE mRNA expression on mononuclear leukocytes was reduced by atorvastatin and/or estradiol treatments. This effect was associated with the APOE polymorphism and with the reduction of the serum cholesterol during the HRT.
