Introdução: O objeto deste estudo é a percepção materna do estado nutricional do filho, pois há evidências de que as mães apresentam dificuldade em reconhecer o estado nutricional do filho, especialmente nos casos de excesso de peso. A prevenção da obesidade requer detecção precoce e a não percepção representa obstáculo importante, tanto para o cuidado materno, quanto para a busca de assistência profissional. Estudos que abordam essa temática ainda são poucos no país, o que torna relevante o desenvolvimento desta pesquisa. Objetivo geral: Avaliar a percepção materna do estado nutricional do filho. Objetivos específicos: Comparar a percepção materna do estado nutricional do filho com o real estado nutricional da criança; identificar as variáveis associadas ao não reconhecimento materno do real estado nutricional do filho; e analisar a satisfação materna com a imagem corporal do filho. Métodos: Estudo transversal, descritivo-analítico, que integra investigação mais ampla desenvolvida em Itupeva, SP. A amostra foi proporcional ao número de crianças menores de três anos de idade matriculadas nas 12 Unidades Básicas de Saúde e foi constituída por 357 pares mães-crianças que buscaram atendimento de fevereiro a maio de 2013. Os dados foram coletados por entrevista realizada com a mãe. A percepção foi avaliada com aplicação de escala de descritores verbais do estado nutricional (de muito magro a muito gordo) para todas as crianças, e escala de silhuetas da imagem corporal, validada para crianças maiores de um ano de idade. Verificou-se peso e estatura das crianças e o estado nutricional foi avaliado pelo Índice de Massa Corporal-para-idade com uso do programa Anthro versão 3.2.2 da Organização Mundial de Saúde. Classificou-se o estado nutricional segundo pontos de corte para percentil (<0,1= magreza acentuada; 0,1 e <3= magreza; 3 e 85= peso adequado; >85 e 97= risco de sobrepeso; >97 e 99,9= sobrepeso; >99,9= obesidade). O banco de dados, construído no software Epi-Info versão 3.5.1, foi confrontado por dupla digitação e as análises foram processadas no STATA (Statistics/Data Analysis) versão 13.1. Utilizou-se teste qui quadrado e regressão logística, com nível de significância de 5%. O estudo foi aprovado por Comitê de Ética em Pesquisa, autorizado pela Diretoria de Saúde do município e todas as mães assinaram Termo de Consentimento Livre e Esclarecido. Resultados: Para 69,6% (n=238) das mães, o filho tinha peso adequado e 44,7% (n=153) não reconheceram o real estado nutricional do filho pela escala de descritores verbais. Dentre essas, 76,5% (n=117) referiram um descritor verbal inferior ao real estado nutricional, ou seja, subestimaram o peso do filho. Com a escala de silhuetas, 80,1% (n=108) indicaram imagem corporal de peso adequado e 34,8% (n=47) não reconheceram o real estado nutricional do filho, sendo que dessas, 91,5% (n=43) subestimaram o peso, com indicação de silhuetas mais magras. A percepção materna associou-se estatisticamente com o real estado nutricional (p<0,01), com maior percentual de não reconhecimento de crianças com excesso de peso, tanto para os descritores verbais (85,2%; n=23), quanto para a escala de silhuetas (61,5%; n=8). Análise de regressão logística múltipla para escala de descritores verbais mostrou que mães de crianças com excesso de peso (ORa=11,1; IC 95% 6,0-20,2) e com baixo peso (ORa=5,8; IC95% 2,0-16,9) apresentaram maior chance de não reconhecer o real estado nutricional do filho, da mesma forma que mães de crianças com mais idade (ORa=3,1; IC95% 1,5-6,3) e do sexo feminino (ORa=1,7; IC95% 1,0-2,8). Para a escala de silhuetas, apenas mães de crianças com excesso de peso apresentaram maior chance de não reconhecer o real estado nutricional do filho (ORa=3,9; IC95% 1,8-8,4). Em relação à satisfação materna com a imagem corporal do filho, 51,2% das mães de crianças com peso adequado queriam o filho mais gordo. Também estavam satisfeitas, 68,7% e 38,5% das mães de crianças com risco de sobrepeso e excesso de peso, respectivamente, sendo que entre as últimas, 23,0% queriam que os mesmos fossem ainda mais gordos e apenas 38,5% queriam que os filhos fossem mais magros. Conclusões: Reiterou-se que as mães não reconhecem o real estado nutricional do filho. A maioria o considera com peso adequado e prefere crianças mais gordas. Mães de crianças com excesso de peso, com mais idade e do sexo feminino apresentam maior chance de não reconhecer o real estado nutricional do filho. Os resultados evidenciam a importância dos profissionais de saúde avaliarem a percepção e auxiliarem as mães a reconhecerem o real estado nutricional do filho para uma efetiva assistência à saúde nutricional na infância, pois a detecção precoce é o primeiro passo para a prevenção ou recuperação do excesso de peso e obesidade.

Introduction: The purpose of this study is the maternal perception about their childs nutritional status, because there is evidence that mothers do not recognize the weight status of children, especially in cases of overweight. Obesity prevention requires early detection and lack of awareness represents an important obstacle for maternal care and for seeking professional assistance. Studies about this topic are few in the country, which makes it important to develop this research. The main objective: To evaluate maternal perception about their childs nutritional status. The intermediary objectives: To compare the maternal perception of the nutritional status of the child with the actual nutritional status of the child; identify the variables associated with non-maternal recognition of the actual nutritional status of the child; and analyze maternal satisfaction with body image of the son with the actual nutritional status of children. Methods: Cross-sectional, descriptive-analytical, integrating broader research developed in Itupeva, SP. The sample was proportional to the number of children under three years of age enrolled in Basic Health Units 12 and consisted of 357 pairs mothers-children who sought care from February to May 2013. Data were collected from interviews with the mother. The perception was evaluated with application of range of verbal descriptors of nutritional status (from "too thin" to "too fat") for all children, and silhouettes scale of body image, validated for more than one year of age. There was weight and height of children and nutritional status was assessed by Body Mass Index-for-age using the Anthro version 3.2.2 program of the World Health Organization was classified nutritional status according to percentile cutoffs (<=0.1 strong thinness; 0.1 and <3 = thinness; 3 an 85 = normal weight,>85 and 97 = risk of overweight,>97 and 99.9 = overweight,>99. 9 = obesity). The database was built using Epi-Info version 3.5.1 software was confronted by double entry and analyzes were processed in STATA (Statistics/Data Analysis) version 13.1. We used chi square test and logistic regression, with significance level of 5%. The study was approved by the Research Ethics Committee, authorized by the Board of Health of the municipality and all mothers signed a consent form. Results: In 69.6% (n=238) of the mothers, the child had "normal weight" and 44.7% (n=153) did not recognize the actual nutritional status of the child by verbal descriptors scale. Among these, 76.5% (n=117) reported a lower verbal descriptor to the actual nutritional status and underestimated the weight of the child. With silhouettes scale of body image, 80.1% (n=108) reported body image of "normal weight" and 34.8% (n=47) did not recognize the actual nutritional status of the child, and these, 91.5% (n=43) underestimated their weight, indicating leaner silhouettes. Maternal perception was associated statistically with the actual nutritional status (p<0.01), with the highest percentage of non-recognition of children overweight for both verbal descriptors (85.2%, n=23), as to silhouettes scale (61.5%, n=8). Multivariate logistic regression to scale descriptors showed that mothers of children with overweight (OR=11.1, 95% CI 6.0 to 20.2) and low birth weight (OR = 5.8, 95% CI 2.0 to 16.9) were more likely to not recognize the actual nutritional status of the child, just as mothers of older children (OR=3.1, 95% CI 1.5 to 6.3) and female (OR=1.7, 95% CI 1.0-2.8). For silhouettes scale, only mothers of children with overweight were more likely to not recognize the actual nutritional status of the child (OR=3.9, 95% CI 1.8 to 8.4). Regarding maternal satisfaction with body image of the child, 51.2% of mothers of children with normal weight wanted the fattest child. Were also satisfied, 68.7% and 38.5% of mothers of children at risk for overweight and overweight, respectively, and among the latter, 23.0% wanted them to be fatter and still only 38, 5% wanted their children to be thinner. Conclusions: If proves that mothers do not recognize the actual nutritional status of the child. The majority considers their child with normal weight and prefers fatter children. Mothers of children with overweight, over age and females have a greater chance of not recognize the actual nutritional status of the child. The results show the importance of health professionals assess the perception and assist mothers to recognize the actual nutritional status of the child for effective assistance to nutritional health in childhood, because detect early is the first step to prevention or recovery of excess weight and obesity.
