Introdução: Os profissionais de enfermagem se deparam, no ambiente de trabalho, com uma variedade de estressores. Diante da exposição repetitiva e prolongada e da ausência de recursos individuais de adaptação, estes estressores podem ocasionar estresse crônico e comprometer a saúde física e mental, principalmente quanto às algumas funções cognitivas, como atenção, memória e função executiva, que são essenciais para prática da enfermagem segura e com qualidade. Objetivos: Identificar a frequência de profissionais de enfermagem com indicadores biológicos e psicológicos de estresse crônico e se a distribuição varia conforme categoria profissional (auxiliar, técnico de enfermagem e enfermeiro) e unidade de atendimento (crítica e não crítica); analisar a correlação entre indicadores biológicos e psicológicos de estresse e se estes se associam com pior desempenho cognitivo nos trabalhadores de enfermagem. Método: Foram incluídos 142 profissionais de enfermagem do turno diurno, randomicamente selecionados, alocados em diferentes setores do Hospital Universitário. Para análise dos indicadores biológicos de estresse crônico, os participantes foram submetidos à coleta de cortisol salivar nos momentos: ao acordar, 30 minutos após acordar, às 14h, às 16h e antes de dormir, durante dois dias consecutivos de trabalho; coleta de sangue em único dia e mensuração de pressão arterial e medidas antropométricas. Para avaliação dos indicadores psicológicos foram aplicadas as escalas de estresse percebido; de depressão de Beck; de sintomas físicos e mentais de estresse (SRQ-20) e de desequilíbrio esforço e recompensa no trabalho. A avaliação cognitiva foi realizada através dos testes extensão de dígitos ordem direta e inversa, trilhas A e B e Aprendizado Auditivo Verbal de Rey (RAVLT). Os dados foram armazenados e analisados estatisticamente através do programa SPSS14, com o nível de significância de 5%. Resultados: Foi observada resposta baixa de cortisol ao acordar e carga alostática elevada em 47,2% dos profissionais. Quase metade dos profissionais apresentaram sintomas de transtorno mental comum. Em quase 90% foi evidenciado desequilíbrio entre as demandas do trabalho e as recompensas obtidas, sendo que 30,4% apresentou comprometimento excessivo às exigências e às obrigações do trabalho. A concentração de cortisol pela manhã se associou com pior desempenho na memória operacional e na memória declarativa evocação tardia. Os profissionais com alteração na resposta de cortisol ao acordar apresentaram maior intensidade de sintomas de transtorno mental comum. Ademais quanto maior o comprometimento excessivo do profissional maior foi o índice de carga alostática. Conclusão: Os profissionais de enfermagem apresentam indicadores biológicos e psicológicos de estresse crônico e estes repercutem negativamente nas habilidades cognitivas, que são essenciais para o planejamento e execução da assistência. Portanto, é imprescindível que as instituições de saúde adotem medidas para minimizar os riscos ocupacionais, preservar a saúde física e mental dos trabalhadores e proporcionar a prática da enfermagem segura e com qualidade.

Introduction: Nurses face a variety of stressors in the work environment. Given the repetitive and prolonged exposure and lack of individual adaptive resources, these stressors can cause chronic stress and compromise physical and mental health, especially in relation to some cognitive functions, such as attention, memory and executive function, which are essential for the practice of safe and quality nursing. Objectives: To identify the frequency of nursing professionals with biological and psychological indicators of chronic stress and whether the distribution varies according to the professional category (auxiliary, nursing technician and nurse) and service unit (critical and non-critical); Analyze the correlation between biological and psychological indicators of stress and if these are associated with worse cognitive performance in nursing workers. Method: We included 142 randomly selected day shift nurses assigned to different sectors of the University Hospital. In order to analyze the biological indicators of chronic stress, participants were submitted to salivary cortisol collection at the moments: upon waking, 30 minutes after waking up, at 2:00 p.m., at 4:00 p.m., and before bedtime, during two consecutive days of work; Blood collection on a single day and blood pressure measurement and anthropometric measurements. To evaluate the psychological indicators were applied the perceived stress scales; Of Beck's depression; Of physical and mental stress symptoms (SRQ-20) and imbalance effort and reward at work. Cognitive assessment was performed through the direct and inverse order digit extension tests, A and B trails and Rey Verbal Auditory Learning (RAVLT). The data were stored and analyzed statistically through the SPSS program, with a significance level of 5%. Results: A low cortisol response on awakening and elevated allostatic load was observed in 47.2% of the professionals. Almost half of the professionals presented symptoms of common mental disorder. Almost 90% showed an imbalance between the demands of the work and the rewards obtained, with 30.4% showing an excessive commitment to the demands and the obligations of the work. The cortisol concentration in the morning was associated with worse performance in operational memory and in declarative memory late recall. Professionals with altered cortisol response on waking had a higher intensity of symptoms of common mental disorder. In addition, the greater the excessive commitment of the professional, the greater the index of allostatic load. Conclusion: Nursing professionals present biological and psychological indicators of chronic stress and these negatively affect cognitive abilities, which are essential for the planning and execution of care. Therefore, it is imperative that health institutions adopt measures to minimize occupational risks, preserve workers' physical and mental health, and provide safe and quality nursing practice.
