Esta pesquisa teve como objetivo caracterizar conhecimentos e práticas de trabalhadores de Centro Infantil (CI ou creche) relativos ao aleitamento materno, por meio de um estudo de caso realizado num serviço vinculado a um hospital escola no município de São Paulo. Realizou-se identificação das rotinas relativas ao aleitamento e entrevista com 10 trabalhadores que atuaram em situações de aleitamento materno na creche com crianças menores de um ano, nos últimos cinco anos (2003 2007). Tendo em vista as funções dos trabalhadores do CI, participaram do estudo 7 auxiliares de desenvolvimento infantil, a auxiliar de enfermagem, o auxiliar administrativo e a diretora, uma vez que todos estes profissionais potencialmente executam ações que podem influenciar o aleitamento materno naquele local. Os resultados foram agrupados em quatro grandes temas: A Importância do Aleitamento Materno na Creche, A Prática do Aleitamento Materno Depende das Mães, Facilidades Relacionadas ao Aleitamento Materno na Creche e Dificuldades Relacionadas ao Aleitamento Materno na Creche. Os respondentes manifestam que o leite materno é importante para o desenvolvimento da criança, todavia, com comentários genéricos, uma vez que seus conhecimentos são provenientes de sua prática diária, de suas vivências enquanto mãe/pai, avó, educador, ou transmitidos pela mídia. Tais conhecimentos empíricos são passados às mães no momento da matrícula e podem ser decisivos na manutenção ou não do aleitamento materno. Eles relatam também que a prática do aleitamento materno depende do interesse das mães, uma vez que a instituição permite seu livre acesso à creche para amamentar. No tema facilidades relacionadas ao aleitamento materno na creche, os respondentes sinalizam o apoio oferecido pela instituição, a garantia ao aleitamento prevista pela legislação e as práticas dos funcionários na creche, como não oferecer outros alimentos que atrapalhariam a amamentação, e reforçar sua importância para as mães. Por fim, as dificuldades relacionadas ao aleitamento materno na creche incluem a distância entre o local de trabalho e o CI e as dificuldades enfrentadas pelos funcionários que cuidam das crianças em aleitamento, uma vez que nem sempre as mães conseguem chegar ao local nos horários das mamadas devido as demandas do serviço. A formação de uma rede de apoio à amamentação, com acompanhamento pelas instituições de saúde, seja a unidade básica de referência, seja o próprio hospital de ensino ao qual o CI se vincula, seria um meio para promover e incentivar o aleitamento materno. Essa parceria possibilitaria preparar adequadamente os trabalhadores do CI não somente quanto aos benefícios do aleitamento materno, mas também quanto a estratégias de ação para que fossem agentes promotores dessa prática. Assim, a creche poderia contribuir ainda mais com a promoção do aleitamento atuando no sentido de prevenir o desmame precoce por meio de outros incentivos a essa prática. O incentivo incluiria ações tais como estímulo à retirada do leite e seu oferecimento no copinho, esclarecimento às mães sobre seus direitos e apoio, entendendo e ajudando as mães a lidar com os conflitos que vivenciam decorrentes das exigências das demandas do trabalho e do cuidado da criança, incluindo o aleitamento

This research aimed to distinguish the knowledge and practices of workers at a Child Center (CC or Day Care Center) regarding to breastfeeding, through a study of case made at a service associated to a school hospital in the city of Sao Paulo. It was made an identification of daily practices related to breastfeeding and interview with 10 workers who engaged to breastfeeding practices at the Day Care Center with infants under one year old in the previous five years (2003 2007). Keeping in mind the function of the workers at the CC, 7 nursing auxiliaries of child development, a nursing auxiliary, an administrative auxiliary and the director attended the study, once all these professionals potentially perform actions that may influence on breastfeeding in that setting. The results were grouped into four broader themes: The Importance of Breastfeeding at the Day Care Center, Breastfeeding Practice Depends on the Mothers, Easiness Related to Breastfeeding at the Day Care Center, and Difficulties Related to Breastfeeding at the Day Care Center. The respondents expressed that breast milk is important for the child development; however, making generic comments, once their knowledge came from either their daily practice, their experiences as parents, grandmother, tutor, or as transmitted by media. This anecdotal knowledge is passed to the mothers at the childs enrollment and they may be decisive in keeping or not the breastfeeding engagement. They also reported that the breastfeeding practice depends on the mothers interest, once the facility allows free access to the Day Care Center to breastfeed. In the theme Easiness related to breastfeeding at the Day Care Center, respondents pointed out the support offered by the facility, the guarantee of breastfeeding due to the law and the practices of the Day Care Center workers, such as not offering other foods which would disturb breastfeeding, and reinforcing its importance for the mothers. Finally, the difficulties related to breastfeeding at the Day Care Center include the distance between workplace and the CC and the difficulties faced by workers who take care of the breastfed infants, once their mothers cant always get to the place in time for breastfeeding due to their service demand. The development of a breastfeeding support network accompanied by the healthcare facilities, be the referential basic unit or the teaching hospital itself in which the CC is affiliated, would be a way to promote and encourage breastfeeding. This partnership made it possible to prepare adequately the CC workers not only regarding to the benefits of breastfeeding, but also the action strategies for them to be promotion agents of this practice. Thus, the Day Care Center could contribute even more to promote breastfeeding acting in the sense of preventing early weaning by other encouragements on this practice. The encouragement would include such actions as the stimulation for breast milk removal and its offering in a cup, informing the mothers about their rights and support, understanding and assisting the mothers to deal with the conflicts they experienced from their work requirement and care of the child demands, including breastfeeding
