Introdução: O exame nasoendoscópico permite a visualização das estruturas da velofaringe em repouso e durante a fala e por isso pode ajudar a definir o diagnóstico e a melhor conduta de tratamento para a disfunção velofaríngea (DVF). Uma vez que a técnica de Furlow (F) preconiza uma maior extensão do véu palatino por meio do reposicionamento das fibras dos músculos do palato mole, este trabalho faz a seguinte pergunta: Será que os pacientes operados pela técnica de F que permaneceram com insuficiência velofaríngea (IVF) após a cirurgia de palato apresentam menor gap velofaríngeo (VF) do que aqueles operados pela de von Langenbeck (vL)? Objetivo: Comparar os achados nasoendoscópicos entre pacientes com fissura labiopalatina operados do palato pela técnica de F e aqueles operados pela de vL. Material e Métodos: Os exames analisados neste estudo foram coletados do banco de gravações de exames de nasoendoscopia do Projeto Florida do Hospital de Reabilitação de Anomalias Craniofaciais da USP. A amostra foi constituída por 70 gravações de nasoendoscopia obtidas de 22 pacientes que receberam o procedimento de F e 48 que receberam o procedimento de vL durante palatoplastia primária. Os pacientes, de ambos os sexos, permaneceram com IVF após a palatoplastia e realizaram o exame de nasoendoscopia para definição da melhor conduta para corrigir IVF, entre as idades de 5 e 15 anos (média 8 anos). As imagens foram editadas em sequência aleatória em um DVD e julgadas por três fonoaudiólogas experientes, quanto: ao movimento e deslocamento do véu palatino; ao movimento e descolamento das paredes laterais da faringe, ao movimento e deslocamento da parede posterior da faringe, à ocorrência da prega de Passavant, e ao tamanho e tipo do gap VF. Resultados: Os resultados mostraram uma concordância geral quase perfeita intrajuízas com uma média 86% (Teste Kappa) para a maioria dos parâmetros julgados. Quanto à comparação das medidas entre as técnicas cirúrgicas (F vs. vL) nenhum resultado foi estatisticamente significante. Conclusão: A técnica cirúrgica utilizada na palatoplastia primária não foi relevante para determinar diferença no tamanho do gap VF para os pacientes que ficaram com IVF.

Introduction: The nasoendoscopic exam allows for visualization of the structures of the velopharynx during rest and during speech and can help the diagnostic and the identification of the best treatment approach to correct velopharyngeal dysfunction (VPD). Since the Furlow technique can lead to a longer velum by means of repositioning the muscles fibers of the soft palate, this study investigates the following question: Do patients who have undergone palatal surgery with the Furlow technique, who remained with velopharyngeal insufficiency (VPI), present smaller velopharyngeal gap than those operated with the von Langenbeck technique? Objective: To compare the nasoendoscopic findings between patients who received the Furlow technique and those who received the von Langenbeck technique for the primary palatal repair. Material and Methods: The nasoendoscopic exams analyzed in this study were retrieved from a bank of nasoendoscopic exams of the Projeto Florida/HRAC/USP. The sample consisted of 70 recorded nasoendoscopic exams obtained from 22 patients who received the F and 48 who received the vL during primary palatoplasty. The patients, both genders, presented with VPI after palatoplasty and were submitted to nasoendoscopic examination, between the ages of 5y and 15 y (mean=8y), for definition of the best treatment approach to correct VPI. The images were edited into a DVD in a randomized sequence to be judged by three experienced speech-language pathologists (SLP), who rated: displacement and excursion of the soft palate; maximum displacement and excursion of lateral pharyngeal walls, displacement and excursion of the posterior pharyngeal wall, presence of Passavant´s pad, and size and type of velopharyngeal gap. Results: Mean intrajudge reliability was almost perfect and found at a 86% level (Kappa) for most parameters rated. Comparison of ratings between F and vL groups revealed a difference that was not statistically significant. Conclusion: The surgical technique used in primary palatoplasty was not relevant to determining difference in the size of the velopharyngeal gap for patients who remained with VPI after primary surgery.
