Introdução: Entre os principais fatores que contribuem para o surgimento da Síndrome metabólica (SM) estão a predisposição genética, inatividade física, distúrbios da homeostase da glicose, hipertensão arterial, obesidade centralizada e dislipidemias. Por outro lado, o stress tem sido apontado como fator de risco para a obesidade centralizada e para a hipertensão arterial. A SM e o stress tem sido pouco avaliados em estudos populacionais. Objetivo: Verificar a prevalência de síndrome metabólica e do stress e a relação de ambos com o nível socioeconômico, hábitos comportamentais, condições de saúde e áreas de residência em usuários de Unidades Básicas de Saúde da zona sul de São Paulo. Métodos: Trata-se de um estudo de corte transversal, cuja amostra aleatória foi constituída por usuários, maiores de 20 anos, de Unidades Básicas de Saúde, situadas no Jardim Germânia, 187 usuários (com predominância de classe média) e no Jardim Comercial, 265 usuários (com predominância de classe pobre), totalizando 452 usuários. Os indivíduos que concordaram em participar do estudo assinaram o Termo de Consentimento Livre e Esclarecido. Para detectar a presença de stress foi utilizado o Inventário de Sintomas de Stress para Adultos de Lipp (ISSL). Para o diagnóstico da SM utilizou-se o critério proposto pela International Diabetes Federation (circunferência da cintura com valores étnicos especificados, sendo para os sul americanos, cintura abdominal 80 cm para mulheres e 90 cm para homens), mais dois quaisquer dos quatro seguintes critérios: glicemia de jejum 100 mg/dL; triglicerídeos 150 mg/dL; colesterol HDL <40 mg/dL em homens e <50 mg/dL em mulheres; pressão arterial com valores de corte considerando 130/85 mmHg ou tratamento de hipertensão previamente diagnosticada. Para efeitos comparativos utilizou-se também, o critério do NCEP ATP III que define como SM a presença de 3 a 5 fatores de risco metabólicos em um único paciente. Os componentes e os valores seriam os mesmos recomendados pelo IDF, exceto para a circunferência da cintura, respectivamente para homens > 102 cm e para mulheres > 88 cm e glicemia de jejum 110 mg/dL. A avaliação laboratorial e clínica constou dos seguintes exames: glicemia de jejum, colesterol total e frações, triglicerídeos e mensuração da pressão arterial. Na avaliação antropométrica foram aferidas medidas de peso, estatura, circunferências abdominal e do quadril. Foi utilizado questionário geral para obtenção de dados sociodemográficos, socioeconômicos, antecedentes familiares e pessoais de morbidades, hábitos comportamentais como tabagismo, etilismo e atividade física. Para avaliação da atividade física foram utilizados o Questionário Internacional de Atividade Física (IPAQ) versão longa modificada junto com o Questionário do Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (VIGITEL) e o método preconizado pela OMS 1985. Análise Estatística: Foi realizada análise exploratória dos dados visando o conhecimento da amostra. Efetuou-se cálculo da prevalência bruta da SM e stress por sexo e por local de residência. Neste trabalho, o nível de significância adotado foi de 5 por cento. Foi estabelecida associação entre as variáveis explicativas de interesse e SM e stress, empregando-se a regressão logística multivariada. Na regressão logística, a verificação do ajuste dos modelos foi realizada por meio do teste do quiquadrado e teste de Hosmer Lemeshow. A presença ou não de multicolinearidade (alta correlação entre as variáveis independentes) foi testada em cada modelo, empregando-se critérios para diagnóstico de multicolinearidade. Resultados: A maioria dos participantes do estudo era do sexo feminino, UBS Jardim Comercial 59,2 por cento e UBS Jardim Germânia 67,4 por cento, a faixa etária com maior representatividade foi 40 49 anos para ambos os sexos nas duas UBS, respectivamente 26,4 por cento e 21,4 por cento. No Jardim Germânia, os percentuais de SM para os homens foram 18 por cento e 11,5 por cento, pelos critérios do IDF e do NCEP ATP III, respectivamente; para as mulheres, por esses critérios, foram de 25.4 por cento e 19 por cento. No Jardim Comercial, os percentuais para os homens foram 27,8 por cento (IDF) e 20,4 por cento (NCEP ATP III). Para as mulheres, 24,2 por cento (IDF) e 19,7 por cento (NCEP ATP III). No Jardim Germânia, o percentual de stress para os homens foi de 60,7 por cento e para as mulheres 84,1 por cento. No Jardim Comercial, o percentual para os homens foi de 43,5 por cento e para as mulheres de 69,4 por cento. A maioria dos participantes do estudo que apresentaram stress, se encontravam na fase de resistência. A fase de quase-exaustão foi encontrada principalmente entre as mulheres: 13,4 por cento no Jardim Comercial e 26,2 por cento no Jardim Germânia. Quanto à associação entre Stress e variáveis nas duas UBS, o sexo feminino apresentou 3 vezes mais chance (OR=3,275; p<0,001) de ter stress do que o sexo masculino. A escolaridade foi analisada como variável contínua, mostrando relação direta com o stress. Não houve relação significativa entre o tabagismo e o stress e o etilista tem o triplo de chance (OR=3,007; p<0,001), comparado ao não-etilista. O sobrepeso teve relação direta com o stress (OR=1,596; p<0,001) e a obesidade (OR=1,478; p<0,001) também. O bairro de Jardim Comercial apresentou relação inversa com o stress. Os níveis de atividade física sedentário e leve, comparados aos níveis de atividade moderada e intensa, aumentam a chance de ter stress. Em relação à associação entre SM segundo critérios do IDF e variáveis em ambas as UBS as mulheres têm o dobro de chance de SM (OR=2,055; p< 0,001) do que os homens. A escolaridade apresentou relação inversa com a SM. Com relação aos hábitos comportamentais, o tabagismo foi positivamente associado a SM, apresentando o dobro de chance(OR=2,553; p< 0,001) comparado ao não tabagismo. O etilismo não apresentou relação significativa. Indivíduos que residem no bairro Jardim Comercial tem mais que o dobro de chance de ter SM (OR=2,399; p< 0,001). Os sedentários (OR=2,407; p< 0,001) e os que tem um nível de atividade física leve (OR=2,076; p< 0,001) têm mais que o dobro de chance de ter SM em relação aos ativos e com nível moderado de atividade física. O stress apresentou uma relação altamente significativa com a SM, (OR=1,690; p< 0,001). Antecedentes familiares de Hipertensão, Diabetes e Doenças Cardíacas foram significativamente relacionados com a chance de SM. Sobre a associação entre SM segundo critério do NCEP ATP III e variáveis nas duas UBS, o sexo feminino apresentou 1,8 vezes mais chance de SM comparado ao masculino. A escolaridade apresentou relação inversa. Quanto aos hábitos comportamentais, os fumantes apresentaram quase 4 vezes mais chance de SM comparados aos nãofumantes. Os etilistas apresentaram mais chance (OR=1,459; p< 0,001; ) que os nãoetilistas. Residir no bairro Jardim Comercial significa ter 2,3 vezes chance de SM. Possuir antecedentes familiares de hipertensão e diabetes foram significativos para ter SM na população de estudo. O sedentarismo e baixo nível de atividade física aumentam significativamente a chance de a população ter SM comparados aos níveis de atividade física moderada e intensa. Conclusões: Os diversos resultados sugerem que as morbidades que compõem a SM são associadas ao stress, e apontam o stress como um grave problema de Saúde Pública nessa população. Medidas preventivas e educativas devem ser consideradas para contribuir com uma melhor qualidade de vida para esta população; bem como a definição de políticas públicas para a promoção da saúde, principalmente na área de saúde mental.

Introduction: Among the principal factors which contribute to the appearance of the Metabolic syndrome (MS) are genetic disposition, physical sedentarism, glucose homeostasis, arterial hypertension, centralized obesity and dyslipidemias. On the other hand, stress has been indicated as a risk factor for centralized obesity and arterial hypertension. MS and stress have rarely been assessed in population studies. Objective: To ascertain the prevalence of metabolic syndrome and of stress and the relation of both to socioeconomic level, behavioral habits, health status and area of residence of users of Basic Health Units in the southern zone of São Paulo City. Methods: This is a crosssectional study the random sample for which was constituted of users of more than 20 years of age of the Basic Health Units situated in Jardim Germânia, 187 users (predominantly middle class) and Jardim Comercial, 265 users (predominantly lower class), totaling up 452 users. The individuals who agreed to participate in the study signed the term of Free and Enlightened Consent. Lipp´s Inventory of Stress Symptoms for Adults (LISS) was used to detect the presence of stress. The criterion proposed by the International Diabetes Federation (waist circumference with specified ethnic values, those for South Americans being; abdominal circumference 80 cm for women and 90 cm for men) was used to diagnose MS, plus any two others of the following four criteria: glycemia in fasting 100 mg/dL; triglycerides 150 mg/dL; cholesterol HDL <40 mg/dL in men and <50 mg/dL in women, arterial pressure with cut-off values considered being 130/85 mmHg or treatment of previously diagnosed hypertension. For purposes of comparison the NCEP ATP III criterion, which defines as MS the presence of from 3 to 5 metabolic risk factors in any one patient, was also used. The components and the values would be the same as those recommended by the IDF, except for waist circumference, respectively > 102 cm for men and > 88 cm for women and glycemia in fasting 110 mg/dL. The laboratory and clinical assessment consisted of the following exams: glycemia in fasting, total cholesterol and fractions, triglycerides and the measurement of arterial pressure. In the anthropometric assessment the following were also measured: weight, height, abdominal circumference and that of the thigh. A questionnaire was used to obtain sociodemographic and socioeconomic data, family and personal morbidity antecedents, and behavioral habits such as smoking, consumption of alcohol, and physical activity. For the assessment of physical activity the long, modified version of the International Questionnaire of Physical Activity (IQPA) together with the Surveillance System Questionnaire for Risk Factors for and Protection against Chronic Diseases by Telephone Survey (VIGITEL) and the method foreseen by WHO 1985, were used. Statistical Analysis: An exploratory analysis of the data for the purpose of getting to know the sample population was undertaken. The calculation of the gross prevalence of MS and stress by sex and place of residence was made. The significance level adopted in this study was of 5 per cent. An association was established between the explanatory variables of interest and MS and stress using multivariate logistic regression, the verification of the adequacy of the models was undertaken by means of the chi-squared test and that of Hosmer Lemeshow. The presence or otherwise of multicolinearity (high degree of correlation among independent variables) was tested for each model, employing criteria for the diagnosis of multicolinearity. Results: The majority of those who participated in the study were women, BHU Jardim Comercial 59.2 per cent and BHU Jardim Germânia 67.4 per cent, the most numerous age group being that of 40 49 years for both the sexes at both the BHUs, 26.4 per cent and 21.4 per cent, respectively. In Jardim Germânia the percentages of MS for men were 18 per cent and 11,5 per cent, respectively, by the IDF and NCEP ATP III criteria; for the women, by the same criteria, they were 25,4 per cent and 19 per cent. In Jardim Comercial the percentages for the men were 27,8 per cent (IDF) and 20,4 per cent (NCEP ATP III). For the women they were 24,2 per cent (IDF) and 19,7 per cent (NCEP ATP III). In Jardim Germânia the percentage of stress, for the men, was 60.7 per cent and for the women 84.1 per cent. In Jardim Comercial, the percentage for the men was 43.5 per centand for the women 69.4 per cent. The majority of those who participated in the study who presented stress were in the resistance phase. The phase of almost total exhaustion was found mainly among the women: 13.4 per cent in Jardim Comercial and 26.2 per cent in Jardim Germânia. As for the association between stress and the variables at the two BHUs, the feminine sex presented a three times greater possibility (OR=3.275; p<0.001) of suffering from stress than did the masculine sex. Schooling was analyzed as a continuous variable, and showed a direct relation to stress. There was no significant relationship between smoking and stress but the consumption of alcohol led to a three times greater possibility of such a relationship (OR=3.007; p<0.001) than did abstention. Overweight was directly related to stress (OR=1.596; p<0.001) and to obesity also (OR=1.478; p<0.001). The suburb of Jardim Comercial presented an inverse relationship to stress. The sedentary and light levels of physical activity, as compared with the moderate and intense levels, increased the possibility of stress. As regards the association between MS according to the IDF criteria and the variables of both the BHUs, the women had twice the possibility of having MS (OR=2.055; p<0.001) than the men. Schooling presented an inverse relationship to MS. Concerning behavioral habits, smoking had a positive association with MS, presenting double the possibility of suffering from MS (OR=2.553; p<0.001) than that of non-smokers. The consumption of alcohol did not present any significant relationship. Individuals who live in the suburb of Jardim Comercial have more than double the chance of having MS (OR=2.399; p<0.001). The sedentary (OR=2.407; p<0.001) and those who engage in light physical activity (OR=2.076; p<0.001) have more than double the chance of having MS than do those of intense or moderate level of physical activity. Stress presented a highly significant relationship with MS (OR=1.690; p<0.001). Family antecedents of hypertension, diabetes and heart diseases were significantly related to the chance of suffering from MS. As to the relationship between MS according to the NCEP ATP III criterion and variables of the two BHUs, the feminine sex presented 1.8 times more chance of MS than the masculine. Schooling presented an inverse relationship. As for behavioral habits, smokers presented almost 4 times more probability the MS than nonsmokers. Those who consumed alcohol presented a greater probability (OR=1.459; p<0.001) than those who did not. Living in the Jardim Comercial suburb meant a 2.3 times greater chance of MS. Having family antecedents of hypertension and diabetes were significant for MS in the population studied. Sedentarism and low level of physical activity increased the chance of having MS as compared with the moderate and intense levels of activity. Conclusions: The various results suggest that the morbidities that compose the MS are associated with stress and indicate stress as a serious Public Health problem for the population studied. Preventive and educational measures should be considered as a contribution to the better quality of life for this population; as also the definition of public policies for health promotion, especially in the field of mental health.
