Introdução - Fenômeno crescente, que envolve alta morbidade e mortalidade e consequentemente altos custos para os sistemas de saúde, a alta prevalência de obesidade está sendo encontrada também entre a população pediátrica e é considerado atualmente um problema de saúde pública. O aleitamento materno tem sido relacionado como fator protetor contra o desenvolvimento da obesidade na infância. Objetivo - Avaliar o estado nutricional de lactentes e crianças no início da idade pré-escolar e analisar a presença de excesso de peso e sua relação com o tempo de aleitamento materno e a idade de introdução de outros alimentos. Métodos - Estudo de coorte histórica no qual foram coletados dados antropométricos (peso e estatura) de crianças de 16 a 37 meses de idade de pré-escolas públicas no município de Taubaté- SP para a classificação de seu estado nutricional. Foram coletados, também, por meio de questionário respondido pelos pais ou responsáveis, o peso de nascimento e tipo de parto da criança e dados sobre a alimentação no primeiro ano de vida: tempo de aleitamento materno exclusivo ou não, idade de introdução leite não materno, água e/ou chá, papa de fruta, papa de vegetais e de guloseimas. O escore z do índice de massa corpórea para a idade (zIMC/I) foi considerada a variável desfecho. As análises entre a variável dependente e as variáveis independentes foram realizadas por meio de métodos estatísticos de correlação e de múltiplas variáveis. Resultados A frequência de crianças com excesso de peso foi de 27,4%. O tempo médio de aleitamento materno exclusivo e total foi respectivamente de 3 e 10 meses. As variáveis sexo, peso ao nascer e tempo total de aleitamento materno foram as que mostraram ter alguma correlação, significativa estatisticamente, com o escore z de IMC para a idade. Conclusões Observou-se uma elevada frequência de excesso de peso. O peso de nascimento elevado é fator de risco e os meninos apresentam menor risco de desenvolver excesso de peso no início da idade pré- 5 escolar. A duração total do aleitamento materno é fator de proteção contra o desenvolvimento precoce de excesso de peso.

Introduction - Growing phenomenon, which involves high morbidity and mortality and consequently high costs for health systems, the high prevalence of obesity has been found also among pediatric population and is currently considered a public health problem. Breastfeeding has been linked as a protective factor against the development of obesity in childhood. Objective - To assess the nutritional status of infants and children in early preschool age and examine the presence of excess weight and its relation to the duration of breastfeeding and age of introduction of other foods. Methods - Historical cohort study in which we collected anthropometric data (weight and height) of children 16-37 months of age in public preschools in the city of Taubaté- SP for the classification of nutritional status. Were also collected through a questionnaire answered by parents or guardians information about the children, such as birth weight and type of delivery, and the food in the first year of life: breastfeeding duration and age of introduction of milk not maternal, water and/or tea, fruit pope, pope of vegetables and of treats. The z-score of body mass index for age (zIMC/I) was considered the outcome variable. The analysis between the dependent variable and the independent variables were performed by statistical correlation methods and multiple variables. Results - The prevalence of children with overweight was 27.4 per cent . The mean duration of exclusive and full breastfeeding was 3 and 10 months, respectively. The variables sex, birth weight and total duration of breastfeeding were the ones that were shown to have some correlation, statistically significant, with BMI z score for age. Conclusions - The study population has a high prevalence of overweight. The high birth weight is a risk factor and the boys have a 7 lower risk of developing overweight at the start of pre- school age. The total duration of breastfeeding is a protective factor against early development of overweight.
