Trata-se de um estudo de caso da gestão em saúde do município de Diadema/São Paulo, no período de 2000 a 2011. Para tanto, relaciona os principais enfrentamentos percorridos neste período no tocante à manutenção da sustentabilidade do sistema público de saúde municipal diademense. O conceito de sustentabilidade empregado abrange duas dimensões principais: a de estrutura e a de processo. Diadema constitui-se como um fiel seguidor dos princípios do SUS, aderindo desde sua implantação às políticas de gestão e assistência implementadas nestes mais de 20 anos de sua construção. Por tanto, o grande objetivo desta análise é o de ampliar a discussão acerca dos rumos trilhados pelo SUS e seu possível destino turbulento, e inferir os principais limites da construção do sistema a partir da ótica municipal. Diadema destaca-se pelos crescentes investimentos na área de saúde: desde 2000 aplica, em média, 31 por cento da sua receita de impostos (compreendidas as transferências constitucionais) em ações de saúde, segundo dados do Sistema de Orçamentos Públicos em Saúde (SIOPS). Isso representa mais do que o dobro do percentual mínimo (15 por cento) regulamentado pela Emenda Constitucional 29 e Lei Federal 141 de janeiro de 2012. Mais além, custeia hoje cerca de 70 por cento de suas despesas com saúde com recursos do orçamento próprio. A análise das políticas implementadas, os principais objetivos perseguidos pela gestão, as principais dificuldades enfrentadas, os principais resultados obtidos, o cenário de financiamento e o quadro de restrições econômico e financeiras da Saúde municipal de Diadema no período, constituíram a base para a caracterização dos três campos de enfrentamentos que descrevem o objetivo de desenhar os limites da construção do SUS: enfrentamento à Estrutura do Federalismo Brasileiro e Princípio de Descentralização do SUS; enfrentamento à Estrutura de Financiamento do SUS; e, Enfrentamento ao Modelo de Atenção. Para cada campo de enfrentamento foram sugeridas propostas, com base na realidade de produção de saúde sob a ótica municipal, em que se descreveram alternativas para desviar o SUS municipal de seu possível destino turbulento na busca de dias de sol na condução das políticas municipais de saúde

It is a case study about health management of the city of Diadema/São Paulo, between 2000 and 2011. Therefore, relates the major obstacles faced in this period as regards the maintenance of the sustainability of the public health system of Diadema. The established concept of sustainability comprehends two main dimensions: the structure one and the process one. The city settles down as a loyal follower of the principles of SUS by the adhesion of its management policies and assistance implemented in these more than 20 years of its construction. Thereby, the major objective of this analysis is to amplify the discussion about the course taken by SUS and its possible turbulent destiny, and infer the main limits of the construction of the system from the municipal optics. Diadema stands out for the growing investments in health section: since 2000, applies, on an average, 31 per cent of its taxes (the constitutional transfers comprehended) in health actions, according to System of Public Budgets in Health (SIOPS). It represents more than the double of the minimum percentage (15 per cent) regulated by the Constitutional Amendment 29 and Federal Law 141 of January 2012. Furthermore, it defrays about 70 per cent of the expenses with health with its own budgets resources. The analysis of the implemented policies, the main persuaded objectives by the management, the leading confronted difficulties, the major obtained results, the financial scenario and the conjuncture of economic and financial restrictions of the municipal Health of Diadema in the period composed the basis for the characterization of the 3 confronting areas that describe the objective to delineate the limits of the construction of SUS: confrontation to the Structure of the Brazilian Federalism and Principle of the Decentralization of SUS; confrontation to the Structure of SUS Financing; and confrontation to the Attention Model. For every area of confrontation, proposals have been suggested, based in the reality of health production under the municipal optics, in which were described alternatives to divert the municipal SUS of its possible turbulent destiny in the pursuit of sunny days in the leading municipal policies of health
