O presente trabalho tem como objetivo o estudo das relações entre as fontes de financiamento e a eficiência em dois hospitais gerais paulistas, privados filantrópicos e de médio porte, e com diferentes fontes de financiamento, sendo que um deles atende quase que exclusivamente a pacientes do Sistema Único de Saúde - SUS e o outro a pacientes de uma importante indústria nacional. Para isso, optou-se por uma amostra intencional (estudo de caso) com a utilização de dados obtidos através dos registros financeiros, contábeis, do serviço de arquivo médico e de um inquérito hospitalar mediante entrevista com diretores dos hospitais. O período escolhido foi o de 1990 a 1994, já que apenas a partir dessa data existiam dados organizados, registrados e confiáveis e que permitiram uma análise à nível de tendência. Considerou-se como váriavel independente o financiamento e como variável dependente a eficiência dos hospitais expressa nos indicadores de produção, recursos humanos, gastos, e de produtividade. A eficiência foi definida de acordo com a literatura vigente, ou seja, a adequação dos recursos utilizados (input, gastos) com relação aos benefícios obtidos (output, desempenho hospitalar). Não foi verificada nenhuma relação entre as fontes de financiamento e a eficiência dos hospitais, no período pesquisado. Apesar de uma situação econômico-financeira desfavorável, com déficits contínuos, o hospital com financiamento público (Hospital A - HA) apresentou uma melhor performance do que o hospital com financiamento privado (Hospital B - HB). Diante disso foi constatada a necessidade de readequação das fontes de financiamento dos hospitais pesquisados já que a longo-prazo a manutenção de um canal quase que exclusivo de recursos poderá significar a piora da situação financeira dos hospitais, com reflexos imediatos na eficiência dessas instituições. O atendimento a pacientes de convênios com empresas e particulares tem sido incrementado recentemente pela direção dos hospitais, mas os resultados ainda são inconclusivos quanto aos ganhos de eficiência.

The objective of the present study is to investigate the relationship between financial sources and efficiency in two private, philanthropic and medium-size general hospitals in São Paulo with two different money resources; one assisting almost exclusively patients from the SUS-Sistema Único de Saúde (Public Health System) and the other with patients from one important national industry. For this purpose, an intentional sample was chosen (study-case) using data obtained from financial and accounting records, from a medical register and nosocomial inquiry by means of interviews with hospital directors. The period selected was from 1990 to 1994 since only from this date on there were organized, registered and trustful data and allowed an analysis at the level of trends. Financing was considered an independent variable and efficiency of the hospitals expressed by production indicators, human resources, expenses and productivity were considered dependent variables. Efficiency was defined according to the current literatura, that is, adequation of the resources used (input, expenses) to benefits obtained (output, nosocomial performance). No relationship between the financial resources and the efficiency of the hospitals was found in the assessed period. Despite an unfavorable economic-financial situation with continuous deficits, the hospital supported by public funds (Hospital A - HA) presented a better performance than by private financing (Hospital B - HB). Thus, a need for re-adequation of financing sources of the investigated hospitals was observed since a long-term maintenance of an almost exclusive resource channel may mean the worsening of a financial situation with immediate reflexes in the efficiency of these institutions. The assistance to patients associated with enterprises and private institutions has been lately incremented by the hospital boards but the results are still inconclusive regarding efficiency gains.
