OBJETIVO: O uso racional de medicamentos é essencial para se atingir níveis adequados de saúde. Objetivou-se, descrever as práticas terapêuticas, e avaliar a prestação dos serviços em 20 unidades de saúde de Ribeirão Preto, São Paulo, Brasil. Utilizou-se os indicadores de uso de medicamentos da OMS: indicadores de prescrição (IP), de assistência ao paciente (IAP) e do serviço de saúde (ISS). MÉTODOS. Trabalhou-se com clínicos e pediatras, utilizando para o IP 6692 receitas, 30 amostras por unidade para o IAP e 69 a 71 medicamentos para o ISS. RESULTADOS. O número médio de medicamentos por receita é 2,4; das prescrições, 30,5% são pela denominação genérica. A prescrição de antibióticos ocorre em 21,3% das receitas, e em 8,2% há prescrição de injetável. Em 80,1 % das prescrições os medicamentos constam da Lista de Medicamentos Padronizados (LMP). O tempo médio de consulta é de 9,2 minutos, e o de dispensação de 19 segundos. Em 70,0% dos casos a população sabe como tomar o medicamento. Do total de medicamentos prescritos, forneceu-se a população 60,3%; para a presença dos medicamentos chaves o índicador foi de 98,4%. CONCLUSÕES: a prescrição pela denominação genérica é baixa, o conhecimento da dose, a prescrição pela LMP e a disponibilidade dos medicamentos é boa. O tempo de dispensação é insuficiente. Para avaliação dos outros indicadores faltam parâmetros.

Objectives: The reasonable use of medicinas is essential to achieve an adequated health standard. The therapeutic measurements to be adopted and the quality of the services rendered to population in 20 Health Units in Ribeirão Preto, São Paulo, Brazil, was studied. This evaluation was based on the list of indicators in the use of medicinas published by WHO: prescription indicator (PI), assistance patient indicator (API) and health service indicator (HSI).Methods: Generalist and pediatric physicians' presciptions were used in this study, in a total of 6692 samples for (PI), 30 per unit for (API) and 69 to 71 medicinas for (HSI). Results: The mean average of medicinas used per presciption was 2.4%, and 30.5% were written using generic or commercial names. The avarage of prescripted oral antibiotics was 21.3% and for parenteral 8.2%. In 80.1% of the prescriptions, the requested medicinas belong to the Standardized Medicina List (SML). The mean time used for the doctor during medicai appointment is 9.2 minutes and to release patient 19 seconds. Only 70% of the attended patients were able to take medicines by themselves. An avarage of 60.3% of the patients received free medicines provided by the Institution. For the main prescripted medicines, the indicator level was 98.4%. Conclusions: The prescriptions using generic or commercial medicine names are low. The accuracy of the users' dosage, the requested medicines available on the SML and medicine disponibility are good. The requested time for dispensation is insuficient. Some other investigations and standards to evaluate alternative indicators are needed.
