Foi realizado estudo em 299 indivíduos de meia-idade e idosos institucionalizados, na cidade de São Paulo. Pesquisaram-se alterações oculares, relacionando-as com as alterações sistêmicas em 25 diabéticos, 84 hipertensos, 5 desnutridos e em 185 pessoas do grupo controle, segundo grupos etários de 50 a 64 anos, 65 a 79 anos e de 80 anos de idade e mais, e sexo. Para caracterizar a. doença sistêmica, verificou-se o prontuário e a história pessoal de cada indivíduo e realizaram-se medidas da pressão arterial e hemoglicoteste. O exame oftalmológico constou de: - exame externo de órbitas, pálpebras, cílios, supercílios, conjuntivas e córneas; - biomicroscopia de cílios, conjuntivas, escleras, córneas, câmaras anteriores, íris e cristalinos; - exame refratométrico; - tonometria de aplanação; - oftalmoscopia monocular direta e binocular indireta. As doenças oculares responsáveis por maior diminuição da acuidade visual foram: 1. catarata senil madura, a maior responsável pela perda visual, aparecendo nos quatro grupos estudados, mas com incidência maior nos idosos; sua distribuição foi semelhante em ambos os sexos; 2. glaucoma, a segunda maior causa de perda visual, encontrando-se em todos os grupos estudados, com exceção dos desnutridos; 3. degeneração senil de mácula, responsável nos quatro grupos pela perda da acuidade visual; 4. outras causas de perda visual: retinopatia diabética proliferativa, leucoma de córnea, estafiloma corneal, trombose venosa, placa de córiorretinite atrófica macular e degeneração miópica.

A study was undertaken with a sample of 299 middle aged and aged individuals, living in institutions in the town of São Paulo. The ocular alterations were examined for and the systhemic alterations were classified in 25 diabethics; 84 high blood pressure (hypertensives); 5 undernourished; 185control group. All the patients were classified according to sex and age as follows: 50 to 64 years, 65 to 79 years, 80 years and over. To characterize the systhemic illness, the Medical Report and Personal History of each individual was verified and examinations for blood pressure and hemoglicotest were also undertaken. The ophthalmological examination comprised: - External examination of orbits, eye lashes, eye lids, conjunctiva and cornea. - Biomicroscopy of eye lashes, conjunctiva, sclera, cornea, anterior chamber, íris, lens and vitreous. - Refraction examination. - Applanation tonometry. - Direct monocular and indirect binocular ophthalmoscopy. The ocular diseases which most altered the visual acuity were: 1. Mature cataract senilis in the 4 studied groups, with similar distribution in both sexes, but with higher prevalence in the aged. Among the ocular diseases this one was the main factor for loss of vision. 2. Glaucoma was found in all studied groups, except the undernourished. 3. Age-related macular degeneration (AMO) occurred in all groups, resulting always in visual acuity loss. 4. Other causes of loss of vision were: proliferative diabetic rethinopathy, corneal leucoma, corneal staphilome, venous trombose, macular atrophical corio-retinitical scar and myopical degeneration.
