O estudo avaliou os efeitos de um programa de educação nutricional participante no controle metabólico e qualidade de vida de mulheres com Diabetes mellitus Tipo 2, mediante a realização de um "quase-experimento" com grupo controle, desenvolvido em três etapas: caracterização biopsicossocial da população, planejamento e execução do programa e avaliação da sua eficácia. Os procedimentos de pesquisa envolveram técnicas quantitativas e qualitativas. Os resultados evidenciam que as características psicossociais da população constituem importantes fatores na determinação e controle do diabetes. A adesão à participação no programa educativo foi significativa, facilitada pelos incentivos motivacionais inerentes à metodologia proposta, especialmente a atenção às necessidades de apoio emocional e social, prazer alimentar, valorização pessoal e manejo do stress. A análise da evolução clínica e da qualidade de vida das participantes dos dois grupos - experimental e controle -, mostrou que apenas no primeiro ocorreram alterações estatisticamente significantes, como redução no peso corporal, diminuição da hemoglobina glicosilada, aumento da HDL-colesterol, redução na ingestão calórica e no total de lipídios, especialmente os saturados. Outros resultados indicativos da eficácia do programa educativo foram o maior aumento na adesão à prática da atividade física, redução no uso de hipoglicemiantes, na freqüência de sintomas associados ao diabetes e aumento subjetivo do bem-estar, entre as mulheres do grupo experimental. Conclui-se que a educação nutricional participante é proposta válida para a melhoria do controle metabólico e qualidade de vida de mulheres com Diabetes mellitus Tipo 2.

This study evaluated the effects of a participating nutritional education program on clinical control and quality of life of women with Diabetes mellitus Type 2, through a closely controlled trial, developed at three steps: biological and psychosocial characterization of the people, educational program planning and development, and assessment of its efficacy. Research procedures included quantitative and qualitative techniques. Results showed that psychosocial characteristics of the observed population are important factors on diabetes determination and control. The adherence to the education program was meaningful, facilitated by motivational incentives inherent to the proposed methodology, particularly attention to emotional and social support needs, food pleasure, personal valorization, and stress coping. The clinical evolution and quality of life analysis of the two observed groups - experimental and control - showed that significant changes occured only in the first group: weight and glycosylated hemoglobin reductions, HDL-cholesterol increase and reductions of energy and total fat intakes, especially of saturated fats. Other results that indicate the efficiency of the education program were: higher increase in adherence to physical exercise, reduction in the use of oral hypoglycemic agents, decreased frequency of diabetes associated symptoms, and increased subjective well-being feeling among women ofthe experimental group. We conclude that the participating nutritional education is a valid proposal for the improvement ofm metabolic control and quality of life of women with Diabetes mellitus Type 2.
