As leishmanioses são agravos à saúde causados por protozoários do gênero Leishmania Ross, 1903, que se encontram largamente difundidas no mundo. Em Mato Grosso do Sul, a leishmaniose tegumentar se faz presente na maioria dos municípios e em todos os da região sudoeste do Estado e a leishmaniose visceral encontra-se em expansão. Estudos descritivos foram realizados, em 1998 e 1999, sobre a fauna flebotomínea, e em 1999, na população humana e, de maio a dezembro de 1999 de inquérito soroepidemiológico em cães e em equídeos no Assentamento Guaicurus, situado a 20°27'LS, 56°52'LW, na parte central da Serra da Bodoquena, município de Bonito, região sudoeste do Estado de Mato Grosso do Sul, Brasil, com vistas a identificar condicionantes para a ocorrência de leishmanioses naquela localidade. Dos 131 lotes do Assentamento Guaicurus, 74 (56,5%) estão ocupados pelos proprietários, 9 (6,9%), arrendados a terceiros e 48 (36,6%), vazios. As moradias e seus anexos, na grande maioria, são pobres. O grau de escolaridade dos chefes de família é nulo ou restrito ao primeiro grau incompleto e às crianças e adolescentes é oferecido, apenas, o ensino de 1° grau. O saneamento básico é deficiente: a água é escassa, sem tratamento e mal armazenada; os dejetos ficam a céu aberto em 42,6% das casas, enquanto que, o lixo é lançado nos quintais em 45,8% das moradias. A localidade não dispõe de telefone ou de rádio amador. Os chefes de família são pequenos lavradores que vivem da agropecuária tradicional. Não têm cooperativa e não são sindicalizados. No período do estudo, foram notificados sete casos de leishmaniose cutânea em moradores do Assentamento Guaicurus e um caso em índio kadweos do Campo dos Índios. Todos os pacientes eram do sexo masculino, com idades entre 19 a 48 anos. O parasita obtido de biópsia cutânea e inoculado em hamster e meio de cultura 3N-BHI, foi identificado como Leishmania (Viannia) braziliensis pela reação de imunofluorescência indireta, utilizando anticorpos monoclonais. Seis pacientes, quando adquiriram a infecção, trabalhavam como empreiteiros em fazendas fora do Assentamento Guaicurus. Dois pacientes relataram o hábito de caçar em cevas. A transmissão da parasitose na população humana naquela localidade pode ser considerada como decorrência de risco ocupacional e cultural, em que aos determinantes biológicos somam e interagem os ecológicos, os econômicos, os sociais e os culturais. As capturas de flebotomíneos resultaram na identificação de 9 gêneros e 14 espécies. No Assentamento Guaicurus, houve nítido predomínio de Lutzomyia longipalpis que se fez presente em quase todos os ambientes estudados e mostrou freqüência mais expressiva em abrigos de animais domésticos. No Campo dos Índios, reserva indígena dos kadwcos c no retiro da Fazenda Salobrão, localidades limítrofes incluídas no inquérito sobre flebotomíneos, a espécie mais freqüente foi Nyssomyia whitmani. Foram examinados 65 eqüídeos; em nenhum foi constatada lesão cutânea compatível com leishmaniose tegumentar e todos apresentaram-se não reagentes à reação de imunofluorescência indireta para leishmaniose. Em um total de 97 cães examinados, a maioria apresentou estado nutricional bom e aparência sadia, exceto, pela presença de ectoparasitas e alterações no tegumento. Dentre esses animais, 23 (23,7%) foram reagentes à reação de imunofluorescência indireta para leishmaniose com títulos variando de 20 a 640. Amostras do parasita, isoladas a partir de aspirado medular, foram identificadas, pela reação de imunofluorescência indireta utilizando anticorpos monoclonais, como Leishmania (Leishmania) chagasi. À luz dos fatores investigados, a veiculação de leishmaniose visceral entre a população canina do Assentamento Guaicurus pode ser atribuída à L. longipalpis, enquanto que, N. whitmani parece ser a responsável pela transmissão de casos humanos de leishmaniose tegumentar no Barreirão, no Campo dos Índios. A alta densidade populacional de Lutzomyia almerioi em cavernas, não só no Assentamento Guaicurus, como ao longo do Planalto da Bodoquena, seu elevado grau de antropofilia e atividade diurna, somados à implementação do ecoturismo para a região sudoeste de Mato Grosso do Sul, recomendam um planejamento cuidadoso das atividades de turismo para a área.

Leishmaniasis are diseases caused by protozoans of the genus Leishmania Ross, 1903 which exist throughout the world. In Mato Grosso do Sul State cutaneous leishmaniasis is present throughout the southwestern region of the state and in the majority of other counties and the visceral leishmaniasis is spreading. The following descriptive studies: of the phlebotomine fauna in 1998-1999, of the human population in 1999, and a seroepidemiological survey of dogs and equine species from May to December 1999, were carried out on the Guaicurus Settlement, located at 20° 27' S and 56° 52" W, in the central section of the Bodoquena Range, Bonito county, in the southwestern region of the Mato Grosso do Sul State, Brazil, to identify determinants for the occurence of leishmaniasis in that locality. Of the 131 allotments ofthe Guaicurus Settlement, 74 (56.5%) are occupied by the owners, 9 (6 9%) are rented out to other occupants and 48 (36.6%) are unoccupied. The great majority of the dwellings as well as of their outbuildings are poor. The level of schooling of the heads of families is non-existent or extremely elementary and only basic schooling is offered to the children and adolescents. The sewage system is inadequate water, untreated and inadequately stored, is scarce; excrement is left in the open air near 42.6% of the dwellings and garbage is just left lying around of 45.8% of the same. The neighborhood has no telephone or amateur radio services. The heads of families are classified as small farmers who live by farming the land and cattle. There is no cooperative and they belong to no labor union. During the period ofthis study, seven cases of cutaneous leishmaniasis were reported among residents of the Guaicurus Settlement and one case among the Kadweos Indians in the "Indian Encampment". All the patients were men, of ages varying from 19 to 48 years. The parasite isolated was identified as Leishmania (Viannia) braziliensis. Six of the patients, when they acquired the infection, were working under contract on ranches outside the Guaicurus Settlement. Two of them reported the habit of hunting at watering places where food was left as a decoy to attract animals. The transmission of the parasite disease in the human population of the locality may be understood in the light ofthe epidemiological pattern of occupational and cultural risk in which the biological determining factors reinforce one another and interact with the ecological economic, social and cultural factors. The sandfly captures rendered specimens of 9 genera and 14 species. On the Guaicurus Settlement there was a clear predominance of Lutzomyia longipalpis which was present in almost all the environments studied and presented the greatest frequency in the shelters used by domestic animais. On the "Indian farm," on the Kadweos Indian reserve, and on the Salobrão Ranch, areas just within the boundaries of the sandfly study area, the most frequent species was Nyssomyia whitmani. Sixty-five specimens of equine species were examined but in none of them was any skin lesion compatible with cutaneous leishmaniasis to be found and none of them presented any reaction to anti-leishmania sorology. Of a total of 97 dogs examined, the majority were in good nutritional condition and were of healthy appearance, except for the presence of ectoparasites and changes in the skin. Of these animais, 23 (23.7%) reacted to sorology by indirect immunotluorescence with titers ranging from 20 to 640 Specimens of the parasite were identified as Leishmania (Leishmania) chagasi. In the light of the factors investigated, it was concluded that the transmission of visceral leishmaniasis among the dog population of the Guaicurus Settlement may be attributed to L. longipalpis, whereas N. whitmani seems to be responsible for the transmission among human cases of cutaneous leishmaniasis in Barreirão, on the "Indian farm." The high density o f the Lutzomyia almerioi population in caves, not only on the Guaicurus Settlement, but also throughout the length of the Bodoquena Range, together with the sandtly's high degree of anthropophily and day-time activity call for the careful planning of tourist activities in the region, especially in view of the increase of ecotourism in the southwestern region of Mato Grosso do Sul.
