A DTM agrupa um número de sinais e sintomas envolvendo os músculos da mastigação, a ATM e estruturas associadas. Estudos epidemiológicos relatam que tais sinais e sintomas podem ser encontrados em todas as faixas etárias, inclusive na infância e na adolescência. A hipertonicidade muscular, assim como a fraqueza dos músculos mastigatórios, estão associadas à DTM, e seu monitoramento é uma forma de verificar as condições do sistema estomatognático. Considerando-se que qualquer desequilíbrio entre os elementos do sistema estomatognático pode repercutir na harmonia morfofuncional de uma criança ou adolescente, o objetivo do presente trabalho foi analisar a atividade eletromiográfica e a espessura muscular dos músculos masseter e temporal e a força de mordida molar máxima de crianças com diferentes graus de severidade de DTM, e comparar com as avaliações resultantes de crianças que não apresentam sinais e sintomas desta disfunção. Foram avaliados 93 indivíduos, entre 7 e 11 anos de idade, em atendimento na FORP-USP. Com base no Índice de Disfunção Clínica de Helkimo e nos critérios de inclusão e exclusão, foram constituídos 4 grupos, (G1 controle, G2 DTM leve, G3 DTM moderada, G4 DTM severa), somando um total de 45 crianças. Os registros eletromiográficos foram realizados durante o repouso e atividades que envolveram a participação ativa da musculatura mastigatória, por meio de um eletromiógrafo MyoSystem-I. Para a análise da espessura muscular, foi utilizado o equipamento de ultrassonografia SonoSite Titan Nacionalizado com transdutor linear 56 mm de 10 MHz. A força de mordida foi obtida por meio de um dinamômetro modelo IDDK, com capacidade de 100 Kgf, adaptado para a condição bucal. Os dados eletromiográficos normalizados e os dados ultrassonográficos e da força de mordida foram tabelados e analisados estatisticamente pela ANOVA. Para a força de mordida, utilizou-se o teste t para estabelecer possíveis diferenças entre os lados direito e esquerdo (SPSS - p<0,05). Os resultados mostraram que a atividade eletromiográfica de crianças com sinais e sintomas de DTM foi menor que em crianças sem sinais e sintomas desta disfunção. Nas condições clínicas de repouso e lateralidades direita e esquerda houve diferença estatisticamente significante (p<0,05), entre os grupos analisados, para o músculo temporal esquerdo, e na MIH para os músculos masseter esquerdo e temporal esquerdo. A espessura dos músculos avaliados por meio da ultrassonografia não apresentou diferenças estatisticamente significantes entre os grupos. A força de mordida molar máxima foi maior nos grupos que apresentaram DTM leve e moderada e menor no grupo com DTM severa. Os valores obtidos para a força de mordida molar máxima esquerda foram significantes (p<0,05) entre os grupos analisados, mas, na comparação entre os lados, os dados não foram estatisticamente significantes. Assim, conclui-se que a severidade dos sinais e sintomas de DTM refletiu na atividade eletromiográfica, mas não gerou mudanças estruturais na musculatura mastigatória. Nos períodos iniciais da DTM, os indivíduos apresentaram uma força de mordida relativamente maior que os indivíduos normais durante a mastigação e houve uma diminuição da magnitude desta força, resultante da sintomatologia dolorosa, nos casos mais severos.

The temporomandibular dysfunction (TMD) includes a number of signs and symptoms involving the masticatory muscles, the temporomandibular joint (TMJ) and associated structures. Epidemiological studies report that such signs and symptoms can be found in all ages, even in childhood and adolescence. The muscular hypertonicity, as well as the weakness of the masticatory muscles are associated with TMD, and their monitoring is a way to verify the conditions of the stomatognathic system. Considering that any imbalance among the elements of the stomatognathic system can affect the morphofunctional harmony in a child or adolescent, the aim of this study was to analyze the electromyographic activity and muscle thickness of the masseter and temporal muscles, as well as the maximum molar bite force in children with different degrees of severity of TMD, and to compare with the results in children without signs or symptoms of this disorder. Nine three (93) individuals, between 7 and 11 years of age, followed at FORP-USP were evaluated. Based on the Helkimos Clinical Dysfunction Index and the inclusion and exclusion criteria, 4 groups were composed (G1-control, G2- mild TMD, G3-moderate TMD, G4- severe TMD), in a total of 45 children. The electromyographic records were performed during rest and activity that involved the active participation of the masticatory muscles, by an electromyograph MyoSystem-I. The SonoSite Titan Nationalized ultrasonography equipment, with 56 mm linear transducer of 10 MHz was used to analyze the muscle thickness. The bite force was obtained by a dynamometer IDDK model, with a capacity of 100 Kgf, adapted for oral conditions. The standardized electromyographic data, the ultrasound findings and the bite force results were tabled and statistically analyzed by ANOVA. The t- test was utilized to determine possible differences in bite force between right and left sides (SPSS - p<0.05). The results showed that the electromyographic activity in children with signs and symptoms of TMD was lower than in children without signs or symptoms of this dysfunction. In clinical conditions of resting and right and left laterality there was a statistically significant difference (p<0.05) between the analyzed groups, for the left temporal muscle, and maximum habitual intercuspation (MHI) for the left masseter and left temporal muscles. The muscle thickness evaluation by ultrasound showed no statistically significant differences between the groups. The maximum molar bite force was higher in the groups that had mild and moderate TMD, and lower in the group with severe TMD. The values obtained for the maximum molar bite force on the left side were significant (p<0.05) among the analyzed groups, but the comparison between sides, the data were not statistically significant. Therefore, it is concluded that the severity of signs and symptoms of TMD was reflected in the electromyographic activity but it did not generate structural changes in the masticatory muscles. In the initial phases of TMD, individuals had a relatively stronger bite force than normal subjects during mastication and there was a decrease in the magnitude of this force secondary to painful symptoms in more severe cases.
