INTRODUÇÃO: O diagnóstico radiológico da compressão das veias ilíacas e ainda motivo de debate. O exame mais utilizado atualmente (flebografia transfemoral) não permite quantificação da estenose, enquanto o ultrassom intravascular, atual padrão ouro para quantificar a estenose, além de caro e mais invasivo, não permite avaliar a presença de colaterais e do fluxo venoso na pelve. A evolução tecnológica dos aparelhos de angiografia permite a realização de angiografia rotacional tridimensional (tomografia de feixes cônicos) que possibilita a avaliação angiográfica da pelve e reconstrução de um molde tridimensional capaz de definir, com mais detalhes, estenoses e dilatações vasculares. Assim, nosso principal objetivo foi comparar a angiografia rotacional tridimensional com a flebografia transfemoral para o diagnóstico da compressão da veia ilíaca comum esquerda. MÉTODO: Os pacientes com suspeita de compressão das veias ilíacas que foram encaminhados para a realização de flebografia transfemoral no Hospital Nove de Julho, foram convidados para participar do estudo, no período de 2013 a 2017. Os que aceitaram, foram também submetidos a angiografia rotacional tridimensional. Uma vez realizados os exames, os filmes foram analisados por 4 examinadores independentes e o resultado comparado com o resultado da angiografia rotacional tridimensional. Foi considerado positivo quando estenose maior ou igual a 50%. RESULTADOS: Foram incluídos 96 pacientes na analise, 69 mulheres e 27 homens, que realizaram ambos os exames. A flebografia transfemoral foi positiva em 34,30% dos casos e a angiografia tridimensional em 80% dos pacientes (p < 0,001). Foi calculado curva ROC para avaliar o melhor ponto de corte para o diagnóstico de estenose para a angiografia rotacional tridimensional, que foi de 64,5% com AUC=84%. O valor preditivo negativo (VPN) da angiografia rotacional tridimensional para 50% de estenose foi de 100%, a razão de verossimilhança negativa (RVN) e a probabilidade pós-teste (Ppós) < 0,1%. Considerando 64,5% como resultado positivo para estenose significativa na angiografia rotacional tridimensional temos VPN 92%, RVN 0,15 e Ppos 5%. CONCLUSÃO: A angiografia rotacional tridimensional, quando comparada a flebografia transfemoral, para o diagnóstico de estenose da veia ilíaca comum esquerda, mostrou ser mais sensivel e possuir um alto valor preditivo negativo, além de ser mais barato e menos invasivo que o exame considerado padrão ouro atualmente

The most commonly used test (transfemoral phlebography) does not allow quantification of stenosis, while intravascular ultrasound, currently gold standard to quantify stenosis, besides being expensive and more invasive, does not allow the evaluation of collaterals and venous flow in the pelvis. The technological evolution of the angiographic devices allows the realization of three-dimensional rotational angiography (tomography of conic bundles) that enables the angiographic evaluation of the pelvis and reconstruction of a three-dimensional mold capable of defining vascular stenoses and dilatations in more detail.Therefore, our main objective was to compare three-dimensional rotational angiography with transfemoral phlebography for the diagnosis of left common iliac vein compression. METHODS: Patients with suspected iliac vein compression who were referred for transfemoral phlebography at the Hospital Nove de Julho were invited to participate in the study from 2013 to 2017. Those who accepted were also submitted to three-dimensional rotational angiography. Once the tests were performed, the films were analyzed by 4 independent examiners and the conclusions compared to the result of three-dimensional rotational angiography. It was considered positive when stenosis greater than or equal to 50%. RESULTS: A total of 96 patients were included in the analysis, 69 women and 27 men, who underwent both examinations. Transfemoral phlebography was positive in 34.3% of cases and three-dimensional angiography in 80.0% of patients (p < 0.001). A ROC curve was calculated to evaluate the best cutoff point for the diagnosis of stenosis for three-dimensional rotational angiography, which was 64.5% with AUC = 84%. The negative predictive value (NPV) of three-dimensional rotational angiography for 50% stenosis was 100%, the negative likelihood ratio (RVN) and the post-test probability (Ppos) < 0.1%. Considering 64.5% as a positive result for significant stenosis in threedimensional rotational angiography we have NPV 92%, RVN 0.15 and Ppos 5%. CONCLUSION: Three - dimensional rotational angiography, when compared to transfemoral phlebography, for the diagnosis of stenosis of the left common iliac vein, showed to be more sensitive and have a high negative predictive value, besides being cheaper and less invasive than the currently gold standard test
