Efeito da pirfenidona em modelo experimental de nefropatia crônica progressiva

Considerando-se a importância do mecanismo de fibrose na doença renal crônica progressiva, o presente projeto visou estudar o efeito da pirfenidona, droga potencialmente anti-fibrótica, na lesão renal em modelo experimental da nefropatia progressiva. Foi utilizado o modelo experimental de nefropatia crônica progressiva, através da inibição da síntese do óxido nítrico. A indução da doença renal foi feita utilizando-se ratos Wistar machos que receberam L-NAME (200 mg/L na água do bebedouro) e dieta hipersódica (3,2% Na) durante 30 dias. Após o período de tratamento (30 dias) foi avaliado o grau de lesão renal através da dosagem da albuminúria, creatinina sérica, grau de glomerulosclerose e grau de isquemia glomerular e fibrose intersticial, através de análise histológica. Foi também analisado o efeito da pirfenidona na inflamação local, através da avaliação dos componentes celulares (macrófagos, linfócitos e miofibroblastos) e da análise da atividade proliferativa (PCNA), através de imuno-histoquímica. Além disso, foi investigado o efeito da pirfenidona na atividade das metaloproteinases (MMP-2 e MMP-9) por zimografia. Além de analisar o efeito isolado da pirfenidona na nefropatia crônica progressiva experimental, o presente projeto se propôs a analisar o efeito da associação da pirfenidona ao losartan, micofenolato mofetil (MMF) e hidralazina, visando o bloqueio efetivo da progressão da doença renal. Os resultados obtidos demonstraram que em modelo experimental de nefropatia crônica a monoterapia com pirfenidona em dose alta apresenta um importante efeito renoprotetor. A associação desta droga, com efeito anti-fibrótico, ao losartan, MMF e hidralazina produziu efeito ainda mais marcante demonstrando que a associação de drogas que agem em diferentes mecanismos patogenéticos, podem representar uma importante alternativa de tratamento para a doença renal crônica progressiva

Considering it importance of the mechanism of fibrosis in the chronic progressive renal disease, the present project aimed at to study the effect of pirfenidone, potentially antifibrotic drug, in the renal injury in experimental model of progressive renal disease. The experimental model of progressive renal disease was used, through the chronic inhibition of the synthesis of nitric oxide. The induction of the renal disease was made using male Wistar rats that had received L-NAME (200 mg/L in the water of the water through) and high salt diet (3.2% Na+) during 30 days. After the period of treatment (30 days) the degree of renal injury measured by the albuminuria and serum creatinine, and the interstitial degree of glomerulosclerosis and fibrosis was evaluated. The effect of pirfenidone on metaloproteinases MMP2 e MMP9 was investigaed by zymography. The effect of pirfenidone in the local of inflammation was also analyzed by evaluating of the cellular components (macrophages, lymphocytes and myofibroblasts) and the analysis of the proliferative activity (PCNA), by means of immunohistochemistry. Besides analyzing the isolated effect of pirfenidone in the experimental chronic model of progressive renal disease, the present project analyzed the effect of the association of pirfenidone to losartan, mycophenolate mofetil (MMF) and hidralazine, aiming at the effective blockade of the progression of the renal disease. The results demonstrated that in experimental chronic progressive renal the monotherapy with pirfenidona presents an important renoprotector effect. The association of this drug, with antifibrotic effect, to losartan in, MMF and hidralazine presented a marked rennoprotector effect, demonstrating that the association of drugs that act in different mechanisms, can represent an important alternative of treatment for the progression of the renal disease
