Introdução: Pacientes com doença renal crônica (DRC) apresentam com frequência hipertensão e como a retenção de líquidos é um dos principais mecanismos fisiopatológicos, o uso de diuréticos é comum nesta população. Enquanto os diuréticos tiazídicos estão associados à redução da calciúria, redução/manutenção dos níveis paratormônio e aumento da densidade óssea, os diuréticos de alça apresentam efeito oposto. Estes efeitos, embora conhecidos na população geral, não foram suficientemente avaliados em pacientes com DRC. Objetivo: Este estudo propõe a avaliação da furosemida e hidroclorotiazida no distúrbio mineral e ósseo da doença renal crônica (DMO-DRC). Métodos: Ensaio clínico randomizado para comparar o efeito de duas intervenções, o uso de diurético tiazídico (grupo HIDRO - hidroclorotiazida 25mg/dia) e o uso de diurético de alça (grupo FURO - furosemida 40mg/dia) sobre o DMO-DRC. Incluídos pacientes com DRC estágio 3, estáveis, em acompanhamento regular ambulatorial. Após um período de wash-out, os pacientes foram randomizados para o grupo HIDRO ou FURO e seguidos prospectivamente a cada 3 meses por 1 ano, pelo mesmo observador, cego para a randomização. Foram avaliadas alterações bioquímicas séricas e urinárias no momento da inclusão e a cada 3 meses; paratormônio e vitamina D foram avaliados na inclusão e a cada 6 meses; marcadores de remodelação óssea (CTX e P1NP), densitometria (absortometria radiológica com dupla energia- DXA) e tomografia computadorizada quantitativa periférica de alta resolução (HRpQCT) foram avaliados na inclusão e após 1 ano. Resultados: foram incluídos 40 pacientes [61,8 (54,73) anos, 65% homens e 70% hipertensos], sendo 20 randomizados para cada grupo, os quais apresentaram características semelhantes após a randomização. Houve redução da calciúria e fração de excreção de cálcio no grupo HIDRO e aumento no grupo FURO, além de uma tendência ao efeito da medicação sobre a concentração de cálcio total, maior no grupo HIDRO. Não foi observada diferença nos níveis de paratormônio e 25-hidroxi-vitamina D, mas houve tendência à elevação de 1,25 dihidroxi-vitamina D no grupo FURO e redução no grupo HIDRO. Houve elevação de CTX, P1NP e fosfatase alcalina no grupo FURO e redução no grupo HIDRO. Não foi encontrada diferença significativa na mudança percentual da densidade óssea aferida pela DXA, apenas uma tendência a maior perda no 1/3 proximal do rádio distal, mais acentuada no grupo FURO. Na HRpQCT, no grupo FURO houve aumento da área cortical e espessura cortical, além de tendência ao aumento da densidade total e da densidade cortical, mas com redução da área trabecular no rádio, enquanto no grupo HIDRO o efeito oposto foi observado. Resultados da HRpQCT devem ser interpretados com cautela devido a dados incompletos. Conclusão: Furosemida e hidroclorotiazida tiveram efeitos opostos sobre o DMODRC em médio prazo. Embora o paratormônio não tenha se alterado significativamente, a furosemida se relacionou com um aumento e a hidroclorotiazida com redução da remodelação óssea, evidenciada pela CTX e P1NP

Introduction: Patients with chronic kidney disease (CKD) often have hypertension and since fluid retention is a main pathophysiological mechanism, the use of diuretic is common in this population. While thiazide diuretics are associated with reduced calciuria, reduction/maintenance of parathyroid hormone levels and increased bone density, loop diuretics have the opposite effect. Although these effects are known in the general population, they have not been sufficiently evaluated in patients with CKD. Objective: This study intends to evaluate the effects of furosemide and hydrochlorothiazide in chronic kidney disease-mineral and bone disorder (CKD-BMD). Methods: This was a randomized clinical trial comparing the effect of two drugs, thiazide diuretic (hydrochlorothiazide 25mg/day - HIDRO group) and loop diuretic (furosemide 40mg/day - FURO group) on the CKD-MBD. We included patients with stage 3 CKD, stable, in regular outpatient follow-up. After a washout period, patients were randomized to either the HIDRO or FURO group and followed prospectively every 3 months for 1 year, by the same observer, blinded to randomization. Serum and urinary biochemical changes were evaluated upon the inclusion and every 3 months; parathyroid hormone and vitamin D were evaluated at the inclusion and every 6 months; bone remodeling markers (CTX and P1NP), densitometry (DXA) and high-resolution peripheral quantitative computed tomography (HRpQCT) were evaluated at the inclusion and after one year. Results: 40 patients [61,8 (54,73) years, 65% male and 70% hypertensive) were included, and 20 were randomized to each group that presented similar characteristics after randomization. There was a reduction of calciuria and fraction of calcium excretion in the HIDRO group and an increase in the FURO group. There was a tendency of the medication effect in the total calcium concentration, which was higher in the HIDRO group. There was no difference in parathormone and 25-hydroxy-vitamin D levels, albeit there was a tendency to increase of 1,25 dihydroxy-vitamin D in the FURO group and a reduction in the HIDRO group. CTX, P1NP and alkaline phosphatase increased in the FURO group and reduced in the HIDRO group. No significant difference was found in the percentage change of bone density measured by DXA, only a tendency to greater loss in the proximal 1/3 of distal radius, more pronounced in the FURO group. Regarding HRpQCT, we found an increase in cortical area and cortical thickness, in addition to a tendency to increase of total density and cortical density, but with reduction of the trabecular area in the radius, in the FURO group, whereas in the HIDRO group the opposite effect was observed. Conclusion: Furosemide and hydrochlorothiazide had opposite effects on the CKD-MBD in the medium-term follow-up.
