INTRODUÇÃO: Pacientes com Doença Renal Crônica (DRC) apresentam várias comorbidades, que podem aumentar o risco de morte tanto em lista de espera como após o transplante renal. O objetivo principal do presente estudo foi determinar o impacto das comorbidades mais importantes e prevalentes nos pacientes com DRC modificando o benefício do transplante comparado à manutenção em lista de espera em curto e longo prazo. Os objetivos secundários foram: 1) comparar a sobrevida dos pacientes em lista de espera por faixa etária, comorbidades e tipo de tratamento, 2) determinar os fatores de risco para morte em lista de espera, 3) elaborar e validar um índice para avaliar o risco de morte dos pacientes inscritos em lista de espera para transplante renal. MATERIAL E MÉTODOS: dados dos pacientes inscritos em lista de espera para transplante renal no Serviço de Transplante Renal do Hospital das Clinicas da Universidade de São Paulo, no período compreendido entre 01 de janeiro de 2007 até 31 de dezembro de 2016, separados em dois grupos, foram analisados retrospectivamente. Análises estatísticas convencionais foram realizadas. Regressão de COX uni e multivariada com intervalo de confiança (IC) de 95% foi realizada para determinar o risco inicial e em longo prazo de morte após a exposição ao transplante renal comparado com a manutenção em lista de espera. Esta análise foi realizada para todos os pacientes e para cada comorbidade estudada. Para a elaboração do índice de morte em lista de espera foi utilizado regressão logística com IC de 95%. RESULTADOS: 1692 pacientes do grupo 1 e 1578 pacientes do grupo 2 foram estudados. Pacientes com Diabetes Mellitus (DM), doença arterial coronariana (DAC), doença arterial obstrutiva periférica (DAOP) e insuficiência cardíaca congestiva (ICC) apresentam diferentes riscos de morte inicial após o transplante e tempos diferentes para igualar o risco à lista de espera, entretanto, todos acabam com risco reduzido a longo prazo após o transplante renal, evidenciando o benéfico do transplante nestas comorbidades. A sobrevida dos pacientes em lista de espera se reduz progressivamente ao longo do seguimento, pacientes com idade > 60anos e com as seguintes comorbidades: DM, ICC e DAC, apresentam menor sobrevida em lista de espera nos dois grupos do estudo. O índice para morte em lista de espera foi criado e validado com a combinação de quatro variáveis clinicas: idade, ICC, DM e DAOP, mostrando-se eficaz em predizer a probabilidade de morte em 3 anos. CONCLUSÕES: A utilização das ferramentas aqui apresentadas poderão servir como estratégias objetivas para quantificar e esclarecer o risco individual no momento de listar um paciente e discutir sobre as melhoras opções terapêuticas no momento da oferta do órgão

BACKGROUND: Patients with End Stage Renal Disease (ESRD) have several comorbidities, which may increase the risk of death both on the waitlist and after kidney transplantation (KTx). The main objective of the present study was to determine the impact of the most important and prevalent comorbidities on the benefit of KTx over remaining on the waitlist, in a short and long-term of followup. Secondary objectives were: 1) to compare waitlist survival by age, comorbidities and type of treatment, 2) to determine risk factors for death on the waitlist, 3) to develop and validate a score to predict the probability of death on the waitlist. MATERIAL AND METHODS: data from patients enrolled on the waitlist for KTx on the Renal Transplant Service of Hospital das Clínicas, of São Paulo University from January 1, 2007 to December 31, 2016, separated into two groups, were retrospectively analyzed. Conventional statistical analyzes were performed. Univariate and multivariate COX regression with 95% confidence interval (CI) was performed to determine the initial and long-term risk of death after time-varying exposure to KTx. Such analyses were performed for all patients and for each comorbidity group of patients. Logistic regression was used in order to develop the score for probability of death on the waitlist. RESULTS: 1692 patients from group 1 and 1578 patients from group 2 were studied. Patients with Diabetes Mellitus (DM), Congestive heart failure (CHF), Coronary artery disease (CAD) and Peripheral vascular disease (PVD), have different initial risk of death after transplantation and different times for equal risk. Finally, they all ended-up with a reduced long-term risk throughout the study period, highlighting the benefit of transplantation in patients with these comorbidities. The survival on the waitlist was reduced progressively throughout the follow-up. Patients older than 60 years and with the following comorbidities: DM, CHF and CAD, had lower waitlist survival in the two study groups. The score for death on the waitlist was created and validated by combination of four clinical variables: age, CHF, DM and PVD, being effective in predict the probability of death at 3 years. CONCLUSIONS: The use of these results could help to quantify and clarify the individual risk of death when wait-listing a patient and were useful when counseling patients about their treatments options at the time of organ offer
