A Doença Renal em Estágio Avançado (DREA) é marcada pela perda irreversível da função renal, resultando na incapacidade de manter as funções regulatórias, excretórias e endócrinas. O tratamento da Doença Renal Crônica (DRC) objetiva retardar a progressão da lesão renal e reduzir as complicações associadas; porém, à medida que a perda funcional renal avança, torna-se necessária adoção de medidas mais invasivas para viabilizar o tratamento, chamada de Terapia Renal Substitutiva, que pode ser a hemodiálise (HD), a diálise peritoneal (DP) e/ou o transplante renal (TX Renal). A abordagem terapêutica da DRC na infância constitui um desafio, devido às características de cada fase do crescimento e do desenvolvimento. O esquema terapêutico implica mudanças no bem-estar da criança, impondo inúmeras restrições físicas e sociais ao paciente e aos cuidadores primários (CP). O objetivo principal deste trabalho é descrever a percepção de qualidade de vida relacionada à saúde de crianças e de adolescentes portadores de DREA nos estágios 4 ou 5 da doença renal, bem como a de seus CP, e compará-la com um grupo de pares saudáveis, também avaliando a relação entre qualidade de vida dos pacientes com alvos de tratamento selecionados. Como objetivo secundário, propõe-se a análise da relação entre os parâmetros de QV dos pacientes e alvos de tratamento selecionados (hemoglobina, albumina e bicarbonato no sangue). Trata-se de um estudo descritivo, prospectivo e comparativo, utilizando os questionários Peds QL® 4.0 - relato da criança/adolescente e relato dos pais e Short Form-36 nos CP. O grupo Pacientes foi constituído de 64 crianças/adolescentes, acompanhados no Serviço de Nefrologia Pediátrica do Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, e seus respectivos CP, e o grupo Controles, constituiu-se de 129 pares saudáveis e seus respectivos CP. A faixa etária predominante entre os entrevistados foi de 8 a 12 anos de idade. A análise dos questionários Peds QL® 4.0 revelou maior comprometimento dos pacientes na capacidade física, no aspecto social e na atividade escolar, segundo os relatos das crianças/adolescentes e seus CP. O impacto nos domínios citados, inclusive no aspecto emocional, se tornou mais pronunciado com o aumento da idade do paciente. Dentre as modalidades terapêuticas, a HD, seguida da DP, tiveram maiores impactos negativos na QV dos pacientes. Demonstrou-se relação entre a magnitude das alterações encontradas nos alvos de tratamento e a piora da pontuação na capacidade física, aspectos emocional e social, segundo relatos dos CP, e nos aspectos emocionais, segundo relato dos pacientes. A análise do SF-36 não apresentou diferenças significativas entre os dois grupos de CP Demonstrou-se relação entre a magnitude das alterações encontradas nos alvos de tratamento e o estado geral de saúde do CP no SF-36. Este trabalho confirma que a DREA afeta negativamente a qualidade de vida dos pacientes, em graus variáveis, dependendo da idade e da modalidade terapêutica utilizada e que os seus CP tendem a ter um ponto de vista diferente daquele de seus filhos, principalmente nos aspectos emocionais e sociais. Apontamos também para a associação entre piora de parâmetros de QVRS e controle inadequado de alvos reconhecidos de tratamento da DREA

End-stage renal disease (ESRD) is characterized by the irreversible loss of kidney function and the impossibility to maintain regulatory, excretory and endocrine functions. Treatment of chronic kidney disease (CKD) aims to prevent the progression of kidney damage and to reduce associated complications. As the loss of renal functional progresses, it becomes necessary to adopt more invasive therapeutic alternatives known as Renal Replacement Therapy, which can be hemodialysis (HD), Peritoneal Dialysis (PD) and/or Kidney transplant (Kidney TX). The choice of therapeutic approach for pediatric CKD is a challenge, due to the characteristics of each phase of growth and development. The therapy motivates changes in the child´s life, imposing physical and social patient and primary caregivers (PC) restrictions. This study aims to describe the perception of health-related quality of life of children and adolescents with stages 4 or 5 ESRD, as well as their PC, and compare it with healthy peers. As a secondary objective, an analysis of the relationship between patients' QOL parameters and selected treatment targets (hemoglobin, albumin and bicarbonate in the blood) is proposed. This is a descriptive, prospective, comparative, using Peds QL ® 4.0 - report of child / adolescent and parent report questionnaires, and Short Form-36 for PC evaluation. The Patients group consisted of 64 children / adolescents followed at the Pediatric Nephrology Unit of the Children's Institute of Hospital das Clinicas of the University of São Paulo Medical School, and their PCs and the Control group consisted of 129 healthy peers and their PCs. The predominant age range among respondents was 8 to 12 years old. Analysis of the Peds QL ® 4.0 questionnaires revealed a greater negative impact in physical capacity and in social and school activities, in the patients group, according to the reports of children / adolescents and their PCs. This impact became more pronounced with increasing patient age, and affected the emotional aspect as well. Among the therapeutic modalities, the HD, seconded by PD, had a more negative impact on patients QoL. An association was demonstrated between the magnitude of inadequacies found in the selected treatment targets and worsening of physical, emotional and social aspects scores, according to reports of PCs and in emotional aspects according to patients' reports. The analysis of SF-36 scores did not differ between the two groups of PCs A relationship was demonstrated between the magnitude of the inadequacies found in the treatment targets and SF-36 PCs' general health scores. This work confirms that the ESRD negatively affects patients' HRQoL in different degrees depending on variables such as age and treatment modality, and that their PCs tend to have a different point of view from that of their children, especially regarding emotional and social aspects. It also points to the association between worsening of HRQoL parameters and inadequate control of recognized therapeutic ESRD treatment targets
