Approximately one-third of the population over 65 suffers from at least one fall a year, which can result in severe physical and psychological damage or death. These accidental falls are a result of the interaction of intrinsic, extrinsic and behavioral factors. About 30-50% of falls occur in the home environment of the elderly, and there is a consensus in the literature regarding the difficulty in identifying and modifying risk factors in the home. As a way of assessing the environment in which the elderly live, there are screening tools that can be used by a health professional. However, there is still no scale of self-evaluation translated and adapted for the brazilian population. The aim of this study is to translate, cross - culturally adapt and validate a self - screening scale for the risks of falling in the home environment and, secondarily, to improve self - perception of risks in order to reduce the number of falls of older adults residents in the community. This is a psychometric study, developed in two phases: the first is the stages of translation into the Portuguese language; back translation; evaluation by a expert committee regarding the semantic, idiomatic, conceptual and cultural equivalence of the items and pre-test. The second phase contemplated the validation through psychometric properties, through validation and the reliability of the instrument. The instrument was applied to a group of 90 elderly residents in the community and the evaluations of equivalences by the group of judges presented a 98% Compliance validity Ratio (CVR) and a Content Validity Index (CVI) of 94%. When we compared the answers of the instrument answered by the elderly with the answers obtained by a health professional, only in 8 12/61 items we had Kappa value > 0.60 and in the test-retest only 11/61 items with Kappa > 0.60. The internal consisty, measured by Cronbach's alpha for the nine domains ranged from 0.73 to 0.77. The instrument presented evidence of content validity and internal consistency and the elderly had a better perception of risk in the second evaluation
