Desde o final da década de 1970, concentrações de cromo total vêm sendo detectadas nas águas do Sistema Aquífero Bauru (SAB). Por isso, a qualidade da água do SAB, utilizada pelos municípios situados no Planalto Ocidental Paulista, tem sido questionada. De acordo com o monitoramento da qualidade da água subterrânea da Companhia Ambiental do Estado de São Paulo (CETESB), parte do cromo total é composta pela forma hexavalente, que é reconhecida como carcinogênica para humanos pela International Agency for Research on Cancer (IARC). Assim, as concentrações de cromo em água subterrânea, foram consideradas como sendo um fator geológico capaz de expor a população investigada a um risco aumentado para o câncer de estômago. Para investigar a associação entre esse fator de risco e a mortalidade por câncer de estômago, foi realizado um estudo ecológico espacial em 326 municípios situados no Planalto Ocidental Paulista e nos limites dessa província geomorfológica, espacialmente coincidente com a área do SAB. Foram relacionados indicadores de exposição ao cromo, como sendo as variáveis substitutas (proxy): tipos de abastecimento, bacias hidrográficas com e sem anomalias de cromo, e municípios atendidos e não atendidos pela Companhia de Saneamento Básico do Estado de São Paulo (SABESP). O efeito adverso foi medido por meio da Razão de Mortalidade Padronizada (RMP) para o desfecho óbitos por neoplasias malignas do estômago na população entre 15 e 75 e mais anos, de ambos os sexos, no período compreendido entre 2004 e 2015. A população padrão foi a do estado de São Paulo. As medidas de exposição (variáveis independentes) e as medidas de desfecho (variável dependente) foram inseridas em um modelo de regressão linear para se analisar a associação entre elas, controladas pela variável Índice de Desenvolvimento Humano Municipal (IDHM) para lidar com um possível efeito de confusão devido à influência das diferenças socioeconômicas entre os municípios da área do estudo. De forma exploratória, foram realizadas subanálises para os municípios com RMP > 1, os abastecidos exclusivamente por água subterrânea e os atendidos somente pela SABESP. Os resultados mostraram não haver aumento da mortalidade por câncer de estômago para a população investigada. Todavia, 24% dos municípios investigados apresentaram RMP > 1. Os resultados do modelo de regressão para esses municípios sugerem risco em algumas exposições, porém sem significância estatística. Os resultados do modelo mostraram associação inversa estatisticamente significante (p = 0,022) entre a exposição ao cromo e a mortalidade por câncer de estômago na população feminina que consome água fornecida pela SABESP. Esse resultado permitiu concluir que as práticas de tratamento da água subterrânea parecem estar sendo eficazes para uma parte dos municípios investigados. No âmbito da prevenção de doenças e da promoção da saúde, como uma das práticas do Sistema Único de Saúde (SUS), esse estudo oferece subsídios para que as Secretarias de Saúde dos municípios investigados reconheçam o problema ambiental do cromo em água de consumo humano e estabeleçam o mapeamento da distribuição das doenças e agravos potencialmente associados ao cromo hexavalente nos municípios que apresentaram RMP > 1

Since the late 1970s, total chromium concentrations have been detected in the water of the Bauru Aquifer System (BAS). Therefore, the water quality of the BAS, used by the municipalities located in the Planalto Ocidental Paulista, has been questioned. According to the monitoring of the groundwater quality of the Environmental Company of the State of São Paulo (CETESB), part of the total chromium is composed by the hexavalent form, which is recognized as carcinogenic to humans by the International Agency for Research on Cancer (IARC). Thus, concentrations of chromium in groundwater were considered a geological factor capable of exposing the investigated population to an increased risk for stomach cancer. To investigate the association between this risk factor and mortality from stomach cancer, a spatial ecological study was conducted in 326 municipalities located in the Planalto Ocidental Paulista and within the limits of this geomorphological province, spatially coincident with the BAS area. Proxy indicators of chromium exposure included: water supply types, hydrographic basins with and without chromium anomalies, and municipalities served and not served by the São Paulo State Basic Sanitation Company (SABESP). The adverse effect was measured using the Standardized Mortality Ratio (SMR) for the outcome of deaths from malignant neoplasms of the stomach in the population between 15 and 75 years and older, of both sexes, in the period between 2004 and 2015. The standard population was that of the state of São Paulo. Exposure measures (independent variables) and outcome measures (dependent variable) were examined in a linear regression model to analyse the association between them, controlled by the Municipal Human Development Index (MHDI) to deal with a possible confounding effect due to the influence of socioeconomic differences between the municipalities of the study area. In an exploratory way, sub-analyses were performed for municipalities with SMR > 1, those supplied exclusively by groundwater and those served only by SABESP. The results showed that there was no increase in the mortality due to stomach cancer in the population under investigation. However, 24% of the municipalities investigated showed SMR > 1. The results of the regression model for these municipalities suggest a risk for some exposures, however, without statistical significance. The results of the model showed a statistically significant inverse association (p = 0.022) between exposure to chromium and mortality from stomach cancer in the female population consuming water provided by SABESP. This result allowed to conclude that the groundwater treatment practices seem to be effective for part of the municipalities investigated. In the context of disease prevention and health promotion, as one of the practices of the Unified Health System, this study offers subsidies for the health departments of the municipalities investigated to recognize the environmental problem of chromium in water for human consumption and establish the mapping of the distribution of diseases and illness potentially associated with hexavalent chromium in the municipalities that presented SMR > 1
