INTRODUÇÃO: A epidemia pelo HIV/Aids é complexa, dinâmica e configura-se como um mosaico de subepidemias regionais. O aumento do número de casos em pequenos municípios brasileiros representa um desafio para o SUS. Conselheiro Lafaiete é uma cidade de 116.512 habitantes, sendo polo microrregional do estado de Minas Gerais. Possui um SAE que atende pacientes com HIV/Aids em toda a microrregião. Souto conduziu um levantamento do perfil epidemiológico dos pacientes deste SAE no ano de 2001. Este projeto se justifica pela necessidade de se conhecer melhor as tendências epidemiológicas da infecção pelo HIV/Aids nesta cidade. OBJETIVOS: Descrever os perfis demográfico, socioeconômico, relativos ao modo de exposição ao HIV, ao estado imunológico, ao uso da TARV, aos hábitos de vida, à taxa de letalidade e às comorbidades. Estabelecer um paralelo com alguns dos dados de Souto. MÉTODOS: Estudo descritivo, série de casos, de corte transversal utilizando dados institucionais, obtidos dos prontuários. Incluídos todos os 163 pacientes que passaram por pelo menos uma consulta no período de 15/01/2008 a 30/01/2009. RESULTADOS: Tempo de infecção, em média: 5,23 anos; 57,1% dos pacientes eram homens, razão homem/mulher 1,33:1; média de idade 38,5 anos; 86,7% dos indivíduos residiam em meio urbano; 63,2% tinham menos de oito anos de estudo; 62,5% recebiam menos de um salário-mínimo/mês, sendo esta renda menor entre as mulheres (p<0,001); 58% dos pacientes exerciam ocupações remuneradas e mais homens (75%) recebiam benefício previdenciário (p=0,037). Adquiriram o HIV por prática heterossexual 71,8%, com múltiplas parcerias mais frequente entre os homens (p<0,001) e parceria única entre as mulheres (p<0,001); 94,1% referiram a não utilização do preservativo antes do diagnóstico e 74,4% referiram que após o diagnóstico passaram a utilizá-lo (p<0,001); 21,2% referiram DST antes do diagnóstico, havendo associação desta variável com prática bissexual (p=0,039) e uso de drogas ilícitas (p=0,006); 3% eram VDRL/FTA-ABS positivos; 10,4% referiram uso de drogas ilícitas inalatórias, principalmente entre os residentes em áreas urbanas (p=0,045). Quanto aos marcadores das hepatites virais 1,5% eram AgHBs positivo e 1,5% VHC positivos. Consumo de tabaco foi referido por 37,9% e de álcool por 41% dos pacientes. Entre as mulheres, 32,9% tiveram o diagnóstico estabelecido em função da gestação, com média de idade mais baixa (p<0,001) e aferição de linfócitos T CD4+ mais alta (p<0,001) em relação ao restante das mulheres. Dos pacientes, 77,3% usavam TARV, com 81% de boa adesão; o autorrelato de adesão esteve relacionado à carga viral (CV) indetectável em 60,2% dos pacientes (p=0,017). Baixa adesão à TARV (p=0,029) e ao acompanhamento (p=0,003) esteve relacionada ao consumo referido de etílicos. A taxa de letalidade foi de 3,1%. CONCLUSÕES: População predominantemente de adultos jovens, do sexo masculino, heterossexual, urbana, de baixo nível de escolaridade e renda, embora economicamente ativa. A gestação contribuiu para diagnóstico mais precoce e em situação imunológica menos comprometida entre as mulheres e quando comparadas aos homens. A via de infecção na mulher foi provavelmente por relação heterossexual com parceiro único. A adesão ao uso de preservativo previamente ao conhecimento da doença foi baixa e independente dos hábitos sexuais, com aumento referido da adesão após o diagnóstico. O consumo de drogas ilícitas foi baixo e esteve relacionado à moradia em região urbana. Adesão terapêutica e ao acompanhamento foram satisfatórias, exceto nos pacientes que referiram consumo de etílicos. Em relação aos dados de Souto, o presente estudo revela um estreitamento da razão de sexo, pior rendimento mensal individual, melhora do padrão de escolaridade, menor ocorrência de DST, maior proporção de uso de TARV e maior consumo referido de etílicos. Constatou-se a necessidade de ampliação da área de abrangência do Centro de Testagem e Aconselhamento para toda a microrregião de Conselheiro Lafaiete

INTRODUCTION: The HIV/Aids epidemic is complex and dynamic and it spreads like a regional sub-epidemic mosaic. The increasing number of cases in small Brazilian municipal districts is a challenge to SUS. Conselheiro Lafaiete has 116,512 inhabitants and it is an micro-region center in the state of Minas Gerais. It has a SAE which attends HIV/Aids patients from the whole micro-region. Souto conducted a survey of the epidemiological profile of the patients of that SAE in 2001.The present project was designed because of the need of knowing better the epidemiological tendencies of the infection by HIV/Aids in that city. OBJECTIVES: To describe the social-economic and demographic profiles in relation to HIV exposure, immunological condition, use of TARV, way of life, lethality rate and the co morbidities; to draw a parallel with some of Soutos data. METHODS: Descriptive study, series of cases with transversal section utilizing institutional data. It were included all the 163 patients attended between 01/15/2008 and 01/30/2009. RESULTS: Period of infection, in average: 5.23 years; 57.1% of the patients were men, man/woman ratio: 1.33:1; average age: 38.5 years; 86.7% lived in the urban area; 63.2% had attended school for less than eight years and 62.5% earned less than the minimum salary/month, the womens income were smaller (p<0.001); 58% of the patients were regular workers and more men (75%) received health welfare benefits (p=0.037); 71.8% were infected by heterosexual practice, multiple partners being more frequent among men (p<0.001) and among women, an only partner (p<0.001); 94.1% mentioned not to have used condom before the diagnosis and 74.4% mentioned to have started using it after the diagnosis (p<0.001%); 21.2% referred to STD (sexually transmitted diseases) before the diagnosis and this variable was associated to bisexual practice (p=0.039) and use of illicit drugs (p=0.006); 3% were VDRL/FTA-ABS positive; 10.4% reported the use of inhalable illicit drugs, especially the ones that lived in urban areas (p=0.045). In relation to the viral hepatitis markers, 1.5% had AgHB positive and 1.5% VHC positive. Consumption of tobacco was reported by 37.9% of the patients and of alcohol by 41% of them. Among women, 32.9% had the diagnosis established due to their pregnancy; the average age being lower and the CD4+ lymphocyte count higher (p<0.001) than the remaining women. Among the patients, 77.3% used TARV, with 81% of good adherence; the self report of adherence was related to the viral charge undetectable in 60.2% of the patients (p=0.017). Low adherence to TARV (p=0.029) and to the follow up (p=0.003) was related to the consumption reported of ethylics. Lethality rate was 3.1%. CONCLUSIONS: The population studied was predominantly of young male heterosexual adult, living in urban areas, with low level of education and low income, though economically active. Pregnancy contributed to an early diagnosis, in a better immunological condition among women and when compared to men. Infection in women happened probably through heterosexual relationship with an only partner. Low frequency in the use of condom before the disease diagnosis, independent from sexual habits, with subsequent increase in its use after the diagnosis. The consumption of illicit drugs was low and related to living in an urban area. The adherence to TARV and the follow-up were satisfactory, with the exception of the consumption reported of ethylics. In relation to Soutos data, the present study show a narrowing ratio among sex, lower individual monthly income, better schooling pattern, lower occurrence of STD, increasing use of TARV and of referred consumption of ethylics. It was made clear the necessity of expand the coverage area of the CTA - HIV Testing and Counseling Center to include the whole micro-region of Conselheiro Lafaiete
