INTRODUÇÃO: Atualmente a hepatite C é um importante problema de saúde pública em todo o mundo. Uma característica dessa infecção é o fato de que menos da metade dos indivíduos infectados conhece seu status de portador, consequência de fatores associados às dificuldades de diagnóstico e de acesso à assistência médica e ao tratamento. Por sua vez, a triagem sorológica realizada no momento da doação de sangue possibilita a oportunidade de identificar indivíduos assintomáticos e incluí-los num seguimento. A Fundação Pró-Sangue, do Hemocentro de São Paulo, realiza triagem para hepatite C desde 1992. Nos últimos 20 anos, cerca de três mil doadores foram diagnosticados pela triagem, porém pouco se sabe sobre o acompanhamento médico recebido por estes indivíduos e seus desfechos clínicos. OBJETIVOS: Os objetivos do presente estudo foram: descrever as características do seguimento clínico de pacientes identificados como infectados pelo vírus da hepatite C através da doação de sangue em um serviço de referência; descrever as características sociodemográficas, clínicas e laboratoriais destes doadores; estimar a proporção de doadores de sangue que não receberam acompanhamento clínico adequado após o diagnóstico; identificar os fatores associados ao não acompanhamento clínico adequado dessa população. MÉTODOS: Desenvolveu-se um estudo descritivo de pacientes infectados pela hepatite C identificados através de doação de sangue da Fundação Pró-Sangue. O diagnóstico da infecção pelo VHC foi realizado através de identificação de anticorpo anti-VHC por técnica imuno-enzimática (ELISA) e confirmação através de teste de imunoblot (RIBA), em segunda amostra coletada, no período compreendido entre 1994 a 2012. Os pacientes foram convidados a participar do estudo através do envio de cartas e de telefonemas. As variáveis analisadas durante as entrevistas, por meio de questionário padronizado, foram: demográficas, uso prévio de drogas, transfusão de sangue, consumo de álcool, hábitos sexuais, comorbidades e variáveis relativas ao conhecimento do status sorológico da hepatite C, ao seguimento clínico e à terapia medicamentosa. Análise multivariada foi realizada para identificar fatores associados à falta de encaminhamento para consulta ou perda de seguimento clínico. RESULTADOS: Entre os 2952 doadores de sangue convidados, 781 (26%) concordaram em participar: 57% do sexo masculino, idade média de 47 anos. A principal via de aquisição da hepatite C foi transfusão de sangue anterior (32%). Um total 282 pacientes (36% da coorte estudada [IC 95% 32,7 - 39,6]) não receberam adequado seguimento clínico após o diagnóstico na doação. As principais causas do não seguimento clínico foram a falta de compreensão da necessidade desse acompanhamento (para 71%) ou dificuldades de acesso a serviços de saúde. Após análise multivariada, estiveram associados ao não seguimento adequado: sexo masculino (p < 0,001), cor de pele não branca (p < 0,001), idade abaixo de 50 anos (p < 0,001) e menor escolaridade (p=0.024). CONCLUSÕES: Uma proporção significativa de pacientes com hepatite C, identificados após doação, não recebeu acompanhamento clínico adequado após o diagnóstico e não foi encaminhada para tratamento específico. Os motivos mais frequentes foram: a falta de compreensão, pelos pacientes, da necessidade de cuidados clínicos e falta de acesso aos serviços de saúde. Acreditamos que nossos resultados reforçam a necessidade e a importância de aconselhamento médico adequado no momento do diagnóstico. Reforçam também a necessidade de implementação de estratégias educacionais e de políticas públicas que ampliem o acesso dos pacientes a serviços e tratamentos disponíveis

BACKGROUND: Hepatitis C is a major public health problem worldwide. A characteristic of this infection is the fact that less than half of infected people know their carrier status, as a result of factors associated with barriers to diagnosis and access to healthcare services and specific treatment. On the other hand, serological screening at the time of blood donation provides the opportunity to identify asymptomatic individuals and include them in a follow-up. The Fundação Pro-Sangue de São Paulo , conducts screening for hepatitis C since 1992. Over the past 20 years, about three thousand donors were diagnosed by serological screening, but little is known about the medical care received by these individuals and their clinical outcomes. OBJECTIVE: The objectives of the present study were as follows: to describe the characteristics of the clinical follow-up of patients identified as infected with hepatitis C through blood donation in a reference service; to describe the demographic and clinical characteristics of this group of patients; to estimate the proportion of blood donors who did not receive proper clinical follow-up after diagnosis; to identify the factors associated with inadequate follow-up. METHODS: A descriptive study of patients infected with hepatitis C identified through blood donation at Fundação Pro-Sangue de São Paulo. Patients were identified as anti-HCV positive using enzyme-linked immunosorbent assay (ELISA) and confirmed by immunoblot recombinant assay (RIBA), from 1994 to 2012. Patients were invited to participate in the study by letters and phone calls. Variables analyzed during the interviews to patients through a standardized questionnaire were: demographic, previous use of drugs, blood transfusion, alcohol consumption, sexual behavior, comorbidities, and variables related to awareness of hepatitis C serological status, clinical follow-up and drug therapy. Multivariate analysis was conducted to identify factors associated with the lack of medical consultation or loss to clinical follow-up. RESULTS: Among 2952 invited blood donors, 781 (26%) agreed to participate: 57% male, mean age of 47 years. The main route of acquisition of hepatitis C was the previous blood transfusion (32%). A total 282 patients (36% of the study cohort [95% CI 32.7 to 39.6]) did not receive proper follow-up or treatment after diagnosis. The main reasons for interruption of clinical follow-up were not understanding the need of medical care (to 71%) or obstacles to access to healthcare services. In multivariate analysis, were associated with inadequate follow-up: male gender (p < 0.001), non-white skin color (p < 0.001) and age below 50 years (p < 0.001) and low education level (p=0.024). CONCLUSIONS: An important proportion of patients identified through blood donation did not receive proper clinical follow-up after diagnosis and were not referred for specific treatment. The most frequent reasons were: lack of understanding, by patients, of the need for clinical care and barriers to access health services. We believe that our results reinforce the need and the importance of proper medical advice at diagnosis of this infection. Our findings also reinforce the need to implement educational strategies and public policies that increase patient access to available services and treatments
