A utilização de anestésicos locais associados a substâncias vasoconstritoras para tratamento odontológico de rotina de pacientes cardiopatas ainda é controversa, devido ao risco de efeitos cardiovasculares adversos. Objetivo: avaliar possíveis alterações hemodinâmicas relacionadas ao uso de anestésico local com vasoconstritor não adrenérgico em pacientes portadores de arritmias cardíacas comparando ao uso de anestésico sem vasoconstritor. Métodos: Estudo prospectivo randomizado, avaliou 33 pacientes com sorologia positiva para Doença de Chagas e 32 pacientes com Doença Arterial Coronariana, portadores de arritmia ventricular complexa ao Holter (>10 EV/h e TVNS), 21 do sexo feminino; idade 54,73 ± 7,94 anos, submetidos a tratamento odontológico de rotina com anestesia pterigomandibular, divididos em dois grupos: Grupo I -utilizando-se prilocaína a 3% com felipressina 0,03 UI/ml e Grupo II -lidocaína 2% sem vasoconstritor. Foram avaliados o número e complexidade de extrassístoles, a freqüência cardíaca e a pressão arterial sistêmica dos pacientes no dia anterior, uma hora antes, durante e uma hora após o procedimento odontológico. Resultados: Não foram observadas alterações hemodinâmicas relacionadas ao anestésico utilizado, nem diferença estatisticamente significativa entre os grupos. Conclusão: Prilocaína a 3% com felipressina 0,03 UI/ml pode ser utilizada com segurança em pacientes chagásicos e coronarianos com arritmia ventricular complexa.

The use of a local anesthetic solution with a vasoconstrictor agent for routine dental treatment in patients with cardiovascular disease is still controversial. Objetive: to evaluate the possible hemodynamics effects related to the use of a local anesthetic solution associated with a non-adrenergic vasoconstrictor agent, comparatively to another local anesthetic solution without a vasoconstrictor agent, in patients with cardiac arrhythmia. Methods: Prospective, randomized study, evaluated 33 patients with Chagas´s Cardiomyopathy and 32 with Coronary Artery Disease, 21 female; mean age 54,73 ± 7,94 of years old and complex ventricular arrhythmia detected by the 24 hours Holter monitoring. The patients underwent a routine dental treatment with pterygomandibular anesthesia, divided into two groups: Group I - received local anesthesia with 3% prilocaine associated with felypressin 0,03UI/ml and Group II - received 2% lidocaine without vasoconstrictor. Heart rate, number and complexity of premature ventricular beats and systemic blood pressure were monitored the day before, one hour before, during and one hour after dental treatment. Results: There were no changes in the homodynamic parameters related to the anesthetic solution and no significant differences were observed between the two groups. Conclusion: These findings suggest that 3% prilocaine with felypressin 0,03 UI/ml can be safely used in patients with complex ventricular arrhythmia associated with Chagas´s and coronary artery desease.
