A Sindrome Congenita do Zika (CZS) e uma condicao que afeta o desenvolvimento infantil quando o Virus Zika (ZIKV) infecta uma mae gestante. Esta condicao e caracterizada pela microcefalia e por uma serie de outras manifestacoes neurologicas e oftalmologicas. Atrofias coriorretinianas, manchas focais pigmentadas, estrabismo e infeccao em estruturas do sistema nervoso central relacionadas a visao sao todos achados ja identificados em pacientes com CZS ou verificados em modelos experimentais. Devido a multiplicidade de formas pelas quais o ZIKV pode prejudicar a funcao visual no inicio da vida, nos propusemos a estudar esta condicao pela avaliacao da acuidade visual (AV) longitudinalmente em uma amostra de criancas infectadas, independente da presenca de manifestacoes clinicas da sindrome. O presente trabalho objetiva avaliar o desenvolvimento da AV de criancas infectadas por ZIKV durante a gestacao e relacionar as potenciais manifestacoes da CZS a quaisquer perdas de AV identificadas. Criancas em um Grupo Controle (N=27), um grupo cuja confirmacao de exposicao ao ZIKV veio por uma amostra da mae (ZE; N=22), e um grupo cuja confirmacao de exposicao ao ZIKV veio por uma amostra da crianca (ZI; N=11) tiveram sua AV medida pelo metodo dos Cartoes de Acuidade de Teller (TAC) de 2 a 4 vezes durante os primeiros 30 meses de vida. A AV se desenvolveu normalmente em pacientes que nao apresentaram sintomas clinicos da CZS, com raras excecoes. Em uma subamostra do grupo ZI que apresentou sintomas clinicos consistentes com a CZS, a AV estava dentro dos valores normativos, com excecao de uma crianca na qual foram identificadas atrofias coriorretinianas. A presenca de sintomas oftalmologicos parece ser preditiva de perdas visuais de alta magnitude. Em casos de criancas infectadas pelo ZIKV em que sintomas clinicos estao ausentes ou se apresentam de forma sutil, a AV parece estar se desenvolvendo normalmente, mas acompanhamento mais completo a longo prazo e sugerido para estes casos

Congenital Zika Syndrome (CZS) is a developmental condition resulting from Zika Virus (ZIKV) maternal infection during pregnancy. This condition is characterized by microcephaly and a series of other neurological and ophthalmological outcomes. Chorioretinal atrophies, focal pigmentary mottling, strabismus and infection in vision-related central nervous system structures have all been identified in CZS patients or verified in experimental models. Due to the multiple ways in which ZIKV might impair visual function in early life, we studied this condition through the assessment of visual acuity (VA) longitudinally in a sample of infected children, irrespective of the presence of CZS symptoms. The present work aims to follow up the VA development of children infected by ZIKV during pregnancy and relate the potential CZS symptoms to any identified losses. Children in a Control Group (N=27), in a group in which confirmation of exposure came from a sample from the mother (ZE; N=22) and a group in which confirmation of exposure came from a sample from the child (ZI; N=11) had their VA measured via the Teller Acuity Cards (TAC) method on 2 to 4 occasions during the first 30 months of life. VA developed normally in patients that did not present CZS outcomes, with rare exceptions. In a sub-sample of the ZI group which presented clinical symptoms consistent with CZS, VA was within normative values, with the exception of one child that presented chorioretinal atrophies. The presence of ophthalmological outcomes seems to be predictive of visual function losses of high magnitude. In cases of ZIKV-infected children without symptoms or presenting only mild symptoms, VA seems to be developing normally. A more long-term follow up is suggested for such cases
