Esta dissertação aborda as consequências do intercâmbio teórico e prático entre a terapia ocupacional em saúde mental e a psicanálise. Para tanto, descreve historicamente a criação da profissão de terapeuta ocupacional, seu fundamento filosófico, o funcionalismo e a perda, posteriormente, de seu embasamento teórico. A contextualização histórica abarca também o uso terapêutico da ocupação no Brasil e a fundação dos primeiros cursos de terapia ocupacional no país. No capítulo reservado à psicanálise, o percurso histórico descreve sucintamente seus conceitos principais e seu desenvolvimento teórico e técnico a partir da confrontação com os chamados casos difíceis, pacientes psicóticos, bordelines e neuróticos graves. São discutidas as contribuições para a questão realizadas por SándorFerenczi, Melanie Klein, D.W. Winnicott e Marion Milner. A contribuição desta última se dá, principalmente, através de seu conceito de meio maleável, relevante para a discussão em torno da fundamentação da prática da terapia ocupacional. O trabalho discute de que modo os conceitos psicanalíticos impactaram a criação de uma terapia ocupacional psicodinâmica, inclusive no Brasil, e também como, mais recentemente, autores franceses como Roussillon e Brun vem se ocupando do estudo das terapias que se utilizam de mediações no atendimento de pacientes com questões no processo de simbolização, utilizando como eixo principal o conceito de meio maleável

This paper discusses the consequences of the theoretical and practical exchange between occupational therapy in mental health and psychoanalysis. In order to achieve that, it historically describes the creation of the profession of occupational therapist, its philosophical foundation, functionalism, and subsequently the loss of its theoretical basis.The historical context also includes the therapeutic use of occupation in Brazil and the development of the first occupational therapy courses in the country. The chapter dedicated to psychoanalysis outlines historically its key concepts and theoretical and practical developments by confronting them to the so-called "difficult cases", psychotics, borderlines and severe neurotics. It also discusses the contributions made by SándorFerenczi, Melanie Klein, DW Winnicott and Marion Milner to the matter. The contribution of Marion Milner occurs mainly through her concept of malleable medium, a relevant concept to the discussion of the reasoning of the occupational therapy practice. The paper discusses how psychoanalytic concepts impacted the creation of a psychodynamic occupational therapy in Brazil. It also discuss how, more recently, French authors such as Roussillon and Brun has been studying therapies that utilize mediation in patients with issues in the symbolization process using the concept of malleable medium
