O interesse em compreender a epidemiologia da malária no Brasil, fornecendo subsídios para seu controle, decorre principalmente de sua elevada morbidade em populações expostas continuamente ao risco de infecção. Entre março de 2010 e abril de 2011, foram realizados quatro cortes transversais em um assentamento agrícola localizado no estado do Amazonas, com o recrutamento de 395 indivíduos de idades entre dois meses e 66 anos. Considerando os resultados moleculares obtidos por PCR em tempo real em 731 amostras sanguíneas, obteve-se uma taxa de positividade de 17,8%, aproximadamente três vezes maior do que a detectada pelo exame microscópico, com a detecção de infecções mistas apenas pela técnica molecular. Cerca de dois terços das infecções diagnosticadas eram assintomáticas. Os resultados enfatizam a importância de infecções assintomáticas infecções, muitas vezes submicroscópicas, na manutenção da transmissão de plasmódios e caracterizam o papel de aspectos comportamentais ligados à idade e à exposição cumulativa à malária.

Understanding the epidemiology of malaria in Brazil, to support malarias control, has attracted interest mainly due to its high morbidity in populations continuously exposed to the risk of infection. Between March 2010 and April 2011, four cross-sectional were performed in a farming settlement located in the state of Amazonas, with the recruitment of 395 patients aged between two months and 66 years. Considering the molecular results obtained by real time PCR in 731 blood samples, we obtained a positivity rate of 17.8%, nearly three times greater than that detected by microscopic examination, with the detection of mixed infections only by molecular technique. About two-thirds of diagnosed infections were asymptomatic. The results emphasize the importance of asymptomatic infections, often submicroscopic, to the maintenance of the transmission of parasites and characterize the role of behavioral issues related to the age and cumulative exposure to malaria.
