Câncer de cabeça e pescoço é um termo amplo comumente utilizado para definir neoplasias da cavidade oral, faringe e laringe. O tratamento com cirurgia, quimioterapia e radioterapia está associado a vários efeitos colaterais, dentre eles prejuízos na condição oral, capacidade mastigatória e quadros de desnutrição. O presente estudo teve como objetivo verificar a influência da condição de saúde oral sobre a capacidade mastigatória e o estado nutricional em indivíduos que realizaram tratamento para câncer de cabeça e pescoço. A condição oral foi avaliada de acordo com a classificação do índice CPOD, CPI e avaliação de uso e necessidade de prótese; a capacidade mastigatória com o uso da Escala Funcional Intraoral de Glasgow; e o estado nutricional por meio do R24h e antropometria (ASG-PPP, IMC, CB, PCT e CMB). Foram incluídos no estudo dados de 23 indivíduos (18 homens e 5 mulheres) após tratamento do câncer de cabeça e pescoço com idade média de 57 anos. De acordo com a avaliação da condição oral, o CPOD médio foi 24,3, 43,4% da amostra era edêntulo, 91,3% necessitavam de prótese e apenas 56,5% utilizavam prótese. Indivíduos idosos apresentaram maior índice de utilização de prótese. O estado de saúde oral foi correlacionado com a capacidade mastigatória, onde 78,2% da amostra relataram dificuldade de mastigação. Além disso, a maior necessidade de prótese e pior capacidade mastigatória correlacionaram-se com maior pontuação da ASG-PPP, e o maior CPOD com menor PCT. A ASG-PPP indicou que 86,9% da amostra apresentava risco ou algum grau de desnutrição, 73,9% disfagia, 65,2% xerostomia, 60,8% disgeusia e 60,8% problemas dentários. O IMC e a CB indicaram eutrofia em 69,5% e 47,8% da amostra respectivamente, e nesta pesquisa ambos não foram bons indicativos de estado nutricional. A avaliação de tecido muscular (CMB) e adiposo (PCT) indicaram desnutrição em 69,5% e 34,7% da amostra, respectivamente. Já a avaliação dietética mostrou que 52,1%, 34,7% e 65,2% dos participantes da pesquisa apresentavam ingestão de energia, proteína e lipídeo maior que a sua necessidade, e 65,2%, 86,9%, 91,3%, 82,6%, 60,8%, 60,8%, consumo de carboidrato, fibra, ácido fólico, cálcio, ferro e riboflavina abaixo da sua necessidade, respectivamente. A maior escolaridade correlacionou-se com menor consumo de alimentos fonte de selênio e zinco, e maior consumo de B12, assim como a pior saúde oral e capacidade mastigatória correlacionaram-se com maior quantidade de vitamina A, cálcio, vitamina D, riboflavina, B12, ácido fólico e zinco, e menor quantidade de carboidrato, ferro e niacina da dieta. Desta maneira, podemos concluir que a condição oral influenciou a capacidade mastigatória de indivíduos após tratamento de câncer de cabeça e pescoço, e esta, quando prejudicada, correlacionou-se com o maior risco nutricional e perda de tecido adiposo. Além disso, a condição oral prejudicada contribuiu para a inadequação na ingestão de macro e micronutrientes.

Head and neck cancer is a broad term commonly used to define tumors of the oral cavity, pharynx and larynx. Treatment with surgery, chemotherapy and radiation therapy is associated with several side effects, including damage to the oral condition, chewing ability and malnutrition frames. This study aimed to assess the effect of oral health condition of the masticatory capacity and nutritional status in patients who underwent treatment for head and neck cancer. The oral condition was evaluated according to the classification of the DMFT index, CPI and assessment of use and need for prostheses; chewing ability with the use of Functional Scale Intraoral of Glasgow; and nutritional status through R24h and anthropometry (PG-SGA, BMI, MC, TSF and MMC). The study included data from 23 subjects (18 men and 5 women) after treatment of head and neck cancer with an average age of 57 years. According to the assessment of oral conditions, the average was 24.3 DMFT, 43.4% of the sample was edentulous, 91.3% prosthesis needed and only 56.5% used prosthesis. Elderly individuals showed the highest rate of use of the prosthesis, the state of oral health was correlated with chewing ability, where 78.2% of the sample reported difficulty chewing. Moreover, the greatest need of prosthesis and worse chewing ability correlated with higher scores of PG-SGA, and the largest DMFT with lower TSF. The PG-SGA indicated that 86.9% of the sample had some degree of risk or malnutrition, dysphagia 73.9%, 65.2% xerostomia, dysgeusia 60.8% and 60.8% dental problems. The IMC and the CB indicated eutrophic in 69.5% and 47.8% of the sample respectively, and this research both were not good indicators of nutritional status. The evaluation of muscle tissue (MMC) and fat (TSF) indicated malnutrition in 69.5% and 34.7% of the sample, respectively. Since dietary evaluation showed that 52.1%, 34.7% and 65.2% of survey participants had intakes of energy, protein and lipid greater than their need, and 65.2%, 86.9%, 91 3%, 82.6%, 60.8%, 60.8%, carbohydrate consumption, fiber, folic acid, calcium, iron and riboflavin below its necessity, respectively. The higher education correlated with reduced consumption of selenium and zinc-rich foods, and greater consumer B12, as well as the worst oral health and masticatory ability correlated with higher vitamin A, calcium, vitamin D, riboflavin, B12 , folic acid and zinc and low-carbohydrate, dietary iron and niacin. Thus, we can conclude that the oral condition influenced the chewing ability of individuals following treatment of head and neck cancer, and that, when impaired, correlated with higher nutritional risk and loss of adipose tissue. Furthermore, impaired oral condition contributed to inadequate intake of macro and micronutrients.
