O frênulo lingual pode limitar os movimentos da língua dependendo da porção de tecido residual que não sofreu apoptose durante o desenvolvimento embrionário, podendo justificar a variação anatômica do frênulo. Diferenciar essas variações requer conhecimento da anatomia da língua e do assoalho da boca para identificar se esses achados podem comprometer a movimentação da língua e as funções orofaciais. A literatura contempla um protocolo não validado para avaliação do frênulo de língua em bebês até seis meses de idade, existindo críticas quanto à sua exequibilidade e complexidade. Este trabalho teve por objetivo verificar se as características anatômicas do frênulo da língua influenciam na sucção e deglutição de bebês nascidos a termo, com a finalidade de propor um protocolo de avaliação do frênulo lingual para bebês. Para isso foi elaborado um protocolo específico dividido em história clínica, avaliação anatômica e avaliação funcional, o qual foi aplicado em 100 bebês saudáveis, nascidos a termo. A avaliação foi realizada por duas fonoaudiólogas especialistas em motricidade orofacial, com experiência em avaliação de frênulo lingual por meio da análise das filmagens realizadas durante a aplicação do protocolo. Para o tratamento estatístico foi aplicado o teste Qui-quadrado seguido do teste exato de Fisher, além da análise de variância, considerando os dados qualitativos e quantitativos, respectivamente. Os resultados obtidos a partir do consenso entre as duas avaliadoras e a análise estatística mostraram que houve relação entre a tendência do posicionamento da língua durante o choro e o tempo entre as mamadas; entre a forma da língua quando elevada e o cansaço para mamar; bem como entre a fixação do frênulo na língua e o movimento da língua na sucção não nutritiva. Pela análise dos dados dos 16 bebês com alteração de frênulo lingual, foi possível chegar às características indicativas de alteração. Na história clínica, os dois itens indicativos foram morde o mamilo e o tempo entre as mamadas. Na avaliação anatomofuncional, as características indicativas foram a tendência do posicionamento da língua durante o choro, a forma da ponta da língua quando elevada e a fixação do frênulo na língua. Na avaliação da sucção não nutritiva todos os 16 bebês apresentaram movimentos de língua incoordenados. Na sucção nutritiva durante a amamentação, a análise mostrou que bebês com alteração do frênulo apresentaram poucas sucções com pausas longas, incoordenação sucção/deglutição/respiração, estalos de língua assistemáticos ou frequentes, bem como mordem o mamilo. Tais achados possibilitaram a adequação do protocolo inicial, e a atribuição de escores, incluindo a pontuação indicativa de alteração do frênulo. Concluiu-se que a forma da língua quando elevada durante o choro e a fixação do frênulo na língua são as características anatômicas do frênulo lingual que influenciam o movimento da língua durante a sucção não nutritiva e o ritmo da sucção durante a amamentação. A partir dos resultados e da experiência da aplicação foi possível propor um protocolo de avaliação do frênulo da língua com escores para bebês, sensível para diagnosticar as características anatômicas do frênulo lingual que podem interferir nas funções de sucção e deglutição durante a amamentação.

The lingual frenulum may restrain the tongue movements depending on the amount of residual tissue that did not undergo apoptosis during embryonic development, what may justify the anatomical variations of the lingual frenulum. Differentiating the anatomical variations requires extensive knowledge of the anatomy of the tongue and the floor of the mouth in order to identify whether the findings may compromise the tongue movements and the orofacial functions. In the literature one protocol was identified for the assessment of the lingual frenulum in infants up to six months of age. That protocol is not validated, and there are controversies on its feasibility and complexity. The aims of this study were to verify whether the anatomical characteristics of the lingual frenulum interfere with the sucking and swallowing in full-term infants and propose a lingual frenulum evaluation protocol for infants. A specific protocol including clinical history, anatomical and functional evaluations was designed and administered to 100 healthy full-term infants. Two speech-language pathologists, specialists in orofacial myology and experienced in lingual frenulum evaluation, performed the assessment by analyzing the recordings and photographs.taken during the administration of the protocol. For statistical treatment the chi-square test was performed followed by Fisher's exact test, besides the analysis of variance, considering the qualitative and quantitative data, respectively. The results obtained from the agreement between the two evaluators and the statistical analyses demonstrated that there was a relationship between the tendency of tongue position during crying and the time between feedings; between the shape of the tongue when elevated and fatigue during breastfeeding; and between the lingual frenulum attachment to the tongue and the tongue movement in non-nutritive sucking. By analyzing the data from the 16 infants with lingual frenulum alteration, it was possible to verify the characteristics that indicate alteration. In the clinical history part, the two indicators were bite nipple and time between feedings. In the anatomo-functional evaluation the indicators were the tendency of tongue position during crying, the shape of the tip of the tongue when elevated, and the lingual frenulum attachment to the tongue. In the non-nutritive sucking evaluation all 16 infants demonstrated uncoordinated tongue movements. By evaluating nutritive sucking during breastfeeding, the analysis demonstrated that the infants with frenulum alteration had a few sucks with long pauses, uncoordinated sucking/ swallowing/breathing, unsystematic or frequent tongue clicking as well as nipple biting. Those findings allowed the adequacy of the initial protocol and the assignment of scores, including scores for indication of lingual frenulum alteration. In conclusion the shape of the tongue when elevated during crying and the lingual frenulum attachment to the tongue are anatomical characteristics of the lingual frenulum that interfere with the movement of the tongue during non-nutritive sucking and the rhythm of sucking during breastfeeding. From the results and the experience of administrating the protocol it was possible to propose a lingual frenulum evaluation protocol with scores for infants, which is effective to diagnose the anatomical characteristics of the lingual frenulum that may interfere with the functions of sucking and swallowing during breastfeeding.
