Partindo de estudos científicos que relatam que a fissura labiopalatina é uma das malformações congênitas mais comuns na raça humana e que esta população quando comparada a sujeitos sem fissura labiopalatina apresenta maior ocorrência de perdas auditivas, complicações otológicas, disfunção velofaríngea e distúrbios articulatórios compensatórios na fala, os quais são indicadores de risco para o desenvolvimento do processo auditivo, da linguagem, da fala e de aprendizagem, este trabalho tem como objetivo verificar a associação entre a disfunção velofaríngea, o articulação compensatória compensatório e a perda auditiva periférica em sujeitos com fissura labiopalatina operados, visando averiguar se as alterações de fala ainda presentes em pacientes com fissura labiopalatina já operados podem ser justificadas pela perda da acuidade auditiva. Nesta pesquisa foi realizado um estudo retrospectivo de dados da avaliação da audição e de fala constante em prontuários de 60 pacientes, matriculados no HRAC-USP, com fissura transforame incisivo unilateral esquerda operada, com idade nas avaliações entre 4 e 5 anos. A escolha dos prontuários foi aleatória. Foram compostos 4 grupos, cada um com 15 pacientes, sendo G1 apresentando disfunção velofaríngea (DVF) e articulações compensatórias (AC´s), G2 sem DVF e com AC´s, G3 com DVF e sem AC´s e G4 sem DVF e sem AC´s. As otoscopias alteradas (positivas) estiveram prevalentes no grupo G1, com maior ocorrência da presença de retração e de opacificação de membrana timpânica. Nos quatro grupos investigados houve sujeitos que realizaram algum tipo de microtológica. Mais de 70% da amostra não apresentaram queixas auditivas. No histórico da audição obteve-se maior ocorrência de histórico negativo para perda auditiva. Um maior comprometimento da audição, evidenciado pela presença de perda auditiva, foi sinalizado para os grupos de pacientes com fissura labiopalatina com DVF ou com ACs presentes, porém sem significância estatística. O G4 apresentou maior ocorrência de sujeitos com audição normal (80%), enquanto que o G1 apresentou a maior ocorrência de perda auditiva (60%). A perda auditiva condutiva leve foi a mais ocorrente para os quatro grupos. O golpe de glote (mais ocorrente), a fricativa faríngea, a fricativa velar (menos ocorrente), a fricativa nasal posterior e a plosiva dorso-médio-palatal foram os distúrbios articulatórios compensatórios presentes. O estudo estatístico demonstra ausência de significância ao associar a presença de perda auditiva com a presença de articulação compensatória (p=0,05), assim como a presença de perda auditiva com a presença de disfunção velofarígea (p=0,12). Contudo, quando associado a presença de perda auditiva com a presença de DVF e AC concomitantemente houve significância estatística (p=0,025).Não foi encontrada associação entre a disfunção velofaríngea, articulações compensatórias e a perda auditiva periférica na população com fissura labiopalatina quando analisadas de forma isolada, porém as alterações de DVF e AC´s quando apresentadas pelo mesmo sujeito apontou significância estatística ao ser associado com a perda auditiva periférica. Sugere-se continuidade do estudo com aumento da amostra e verificando a associação da disfunção velofaríngea e articulações compensatórias com as habilidades auditivas centrais, visando esclarecer os pontos ainda obscuros no processo diagnóstico, fornecendo dados que favoreçam o processo de intervenção desta população.

Scientific studies report that cleft lip and palate is one of the most common congenital malformations in the human race. This population, when compared to those without cleft lip and palate, show higher occurrence of hearing loss, otologic complications, velopharyngeal dysfunction and compensatory articulation dysfunction in speech, which are risk indicators for the hearing development, language, speech learning processes. This study aims to verify the association between the velopharyngeal dysfunction, the compensatory articulation dysfunction and the peripheral hearing loss in people with operated cleft lip and palate. It inquires if speech alterations in patients with operated cleft lip and palate can be justified by the loss of hearing acuity. It consists of a retrospective study on the hearing and speech evaluation of 60 patients records with transforamen unilateral left cleft lip and palate, aged between 4 and 5 years at the time and registered in the HRAC-USP. Records were chosen randomly. Four groups, of 15 patients each, were organized. G1 presented velopharyngeal dysfunction (VPD) and compensatory articulation disorders (CADs), G2 without VPD and with CADs, G3 with VPD and without CADs and G4 without VPD and CADs. The otoscopies with positive alterations were prevalent in G1, with higher occurrence of retraction and opacification of the tympanic membrane. There were subjects submitted to some kind of microtologic procedure in all groups. More than 70% of the sample did not present auditory complaints. In the hearing history there was a higher occurrence of negative description for hearing loss. Groups of patients with cleft lip and palate with VPD or CAD's presented more hearing alterations, supported by the presence of hearing loss. It was no statistically significant, though. G4 presented higher occurrence of subjects with normal hearing (80%), whereas G1 presented the highest occurrence of hearing loss (60%). The conductive hearing loss presented the highest occurrence in all groups. The stroke of the glottis (more frequent), the pharyngeal fricative consonant, the velar fricative consonant (less frequent), the posterior nasal fricative consonant and the plosive mid-dorsum palatal were the compensatory articulation disorders. Results showed no significance when associating the presence of hearing loss with the presence of compensatory articulation disorders (p=0.05), as well as the presence of hearing loss with the presence of velopharyngeal dysfunction (p=0.12). However, when associated with hearing loss with the presence of compensatory articulation disorders and velopharyngeal dysfunction concomitantly statistical significance has been found (p=0,025). Association between the velopharyngeal dysfunction, the compensatory articulation disorders, and the peripheral hearing loss in the population with cleft lip and palate, when isolation analyzed, was not found. When the velopharyngeal dysfunction and compensatory articulation disorders showed by the same person, sta Statistical significance was found when associate with the presence of hearing loss. Further studies with bigger samples are suggested. It is essential to examine the association of the velopharyngeal dysfunction disorder with the central hearing abilities, aiming at illustrating the unknown questions in the diagnostic process in the future. It may provide favorable data to the intervention process of this population.
