O propósito deste estudo foi contribuir para a mudança do paradigma odontológico, centrado na doença, por meio da conversão dos resultados dos Levantamentos Epidemiológicos de cárie dentária no índice T-Health modificado, para analisá-los do ponto de vista de estrutura dentária saudáve e discutir o papel da epidemiologia no contexto de promoção da saúde. Esta pesquisa caracteriza-se como um estudo epidemiológico longitudinal retrospectivo, produzido a partir de dados de levantamentos epidemiológicos operacionalizados no município de Bauru-SP, em 1976 (n=261), 1984 (n=321), 1990 (n=253), 1995 (n=377) e 2001 (n=211), relativos aos escolares de 12 anos de idade. Os resultados apontaram que o CPOD passou de 9,89, em 1976, para 1,44, em 2001, alcançando redução percentual da ordem de 84,53%. Em contrapartida, o T-Health evoluiu de 82,05 para 107,63 - aumento de 30,84%. Esta diferença se deve ao fato da variação do T-Health ser calculada em relação à totalidade dos dentes, e não somente aos acometidos por cárie, que é o que acontece com o CPOD. Concluiu-se que o T-Health modificado demonstrou ser um indicador mais sensível da variação do estado de saúde bucal quando comparado ao CPOD, sendo recomendado que seja incluído nos estudos epidemiológicos de maneira complementar ao CPOD, no intuito de permitir comparações de saúde bucal de vários grupos populacionais e embasar os atores da saúde no planejamento de ações de saúde.

Proposal of utilization of the sound-equivalent teeth index for analysis of the oral health condition of school children aged 12 years in the city of Bauru, SP, from 1976 to 2001

This study aimed contributing to a change in the dental paradigm, focused on the disease, by conversion of the results of epidemiological surveys of dental caries to the modified T-Health index, in order to analyze them from the viewpoint of a healthy tooth structure and discuss the role played by epidemiology in the context of health promotion. This study is characterized as a retrospective longitudinal epidemiological study, based on secondary data of epidemiological surveys conducted at the city of Bauru-SP in 1976 (n=261), 1984 (n=321), 1990 (n=253), 1995 (n=377) and 2001 - (n=211), related to 12-years-old students. The results indicated that the DMFT was reduced from 9.89, in 1976, to 1.44, in 2001, reaching a percent reduction of 84.53%. On the other hand, the T-Health was increased from 82.05 to 107.63 - a 30.84% increase. This difference occurred because the variation in the T-Health index is calculated in relation to the total number of teeth, and not only of teeth affected by caries, as the DMFT. It was concluded that the modified T-Health index was a more sensitive indicator of variation in the oral health status compared to the DMFT. Thus, its inclusion in epidemiological studies is recommended as a complementary index to the DMFT, with a view to allow comparisons of oral health between several populations and provide subsidies to health professionals when planning health actions
