A valorização da estética não ocorre apenas no indivíduo adulto, mas também na criança. A clareação dentária externa realizada é uma alternativa viável e segura de tratamento estético desde que realizada pelo profissional e respeitados requisitos importantes como proteger os dentes e tecidos moles para que os riscos sejam os menores possíveis. Por ser o ponto frágil da estrutura dentária o conhecimento sobre a morfologia da junção amelocementária é muito importante durante alguns procedimentos clínicos e especialmente na clareação dentária. O objetivo deste estudo foi analisar os tipos morfológicos de inter-relação tecidual na região cervical dos dentes decíduos e compará-los nos diferentes grupos dentários, além de avaliar as alterações morfológicas da junção amelocementária induzidas pela clareação dentária externa a laser. Com esses objetivos, analisou-se a junção amelocementária de 120 dentes decíduos à microscopia eletrônica de varredura. Destes, 105 eram não clareados e subdivididos em grupos de 15 espécimes por grupo dentário e outros 15 foram clareados. Ao término das análises estabeleceram-se os padrões morfológicos da junção amelocementária decídua e compararam-se com os encontrados nos dentes clareados. Os três tipos de relação entre tecidos mineralizados na junção amelocementária foram cemento recobrindo esmalte, esmalte e cemento topo-a-topo e a presença de "gap" entre o esmalte e o cemento, expondo áreas de dentina, não detectando-se qualquer predominância quanto aos grupos dentários. Em todos os dentes, apresentaram-se os três tipos morfológicos de junção amelocementária. O uso da técnica de clareação externa tendo-se o laser como ativador acentuou o contorno do esmalte e do cemento deixando-os mais nítidos e os túbulos dentinários nos "gaps" se apresentaram mais abertos e em maior número, indicando uma ampliação na área dos "gaps". Por apresentar forma irregular e presença de "gaps", a junção amelocementária decídua pode ser uma região mais frágil para a instalação de reabsorções cervicais externas em casos de traumatismos e colocação inadequada de grampos para isolamento absoluto. A expansão dos "gaps" pelos agentes clareadores pode estar associada a uma maior sensibilidade dentinária após a clareação dentária externa tal como foi descrita em dentes permanentes.

Morphology of the cementoenamel junction (CEJ) of deciduous human teeth on scanning electron microscopy (SEM) and the effects of external bleaching

Esthetics is not only important for adults but also for children. External dental bleaching is a feasible and safe alternative of esthetic treatment, as long as performed by the professional under some important required conditions, such as, adequate protection of the teeth and soft tissues, so that risks are as reduced as possible. Since the cementoenamel junction (CEJ) is the fragile spot of the dental structure, previous knowledge of its morphology is essential in order to perform certain clinical procedures and especially dental bleaching. The goal of this study was to analyze the morphological types of intertissues relation on the cervical area of deciduous teeth, comparing them between different dental groups and evaluating the morphological alterations of the cementoenamel junction (CEJ) induced by dental external laser bleaching. The sample comprised 120 deciduous teeth analyzed by scanning electron microscopy (SEM). Out of the 120 deciduous teeth, 105 remained unbleached and were divided in groups of 15 specimens for each dental group. The last 15 teeth were submitted to laser bleaching. The analysis performed allowed us to establish the morphological patterns of the cementoenamel junction (CEJ) in deciduous teeth with and without bleaching. The three types of interrelations between the mineralized tissues at the cementoenamel junction (CEJ) were: enamel overlapped by cementum; enamel and cementum edge-to-edge; and a gap, revealing a strip of exposed dentin. No predominance of any of the three types of interrelations between groups was found, whatsoever. All groups of teeth presented the three morphological types of cementoenamel junction (CEJ). The technique of external bleaching activated by laser enhanced the outline of the enamel and cementum making them more noticeable. The dentin tubule at the areas of gap were widened and more numerous indicating amplification of the gap areas after bleaching. Since the cementoenamel junction (CEJ) of deciduous teeth also presents irregularities and gaps, it may be a more fragile and susceptible area to the occurrence of cervical external root resorption in case of traumatisms or due to the inadequate installation of rubber dam clips. The expansion of the gaps by bleaching agents may be associated with increased dentinal sensitivity after external dental whitening, such as described in permanent teeth.
