A morfologia facial de crianças nascidas com fissura labiopalatina unilateral completa (FLP) apresentam comprometimento dos tecidos duros e moles do complexo nasomaxilar. A proposta desta pesquisa foi avaliar as estruturas ósseas do terço médio da face de pacientes portadores de FLP unilateral completa por meio da tomografia computadorizada por feixe cônico (TCFC) utilizando o software OsiriXTM. A amostra foi composta por TCFC de 28 crianças portadoras de fissura labiopalatina unilateral completa sendo 19 do lado esquerdo e 9 do lado direito, com média de idade de 9 anos e 6 meses. Todas TCFC foram obtidas imediatamente antes do enxerto ósseo alveolar secundário após a expansão maxilar. A análise das estruturas do terço médio da face foi realizada com o emprego do software OsiriXTM DICOM Viewer, rodado em um computador pessoal MacBook Pro, sistema operacional MAC OS X Versão 10.6.8 da Apple Inc. As mensurações hemifaciais foram realizadas por meio da demarcação de sete pares de pontos bilaterais (Zf, Zy, Nm, Cn, J, Or e Po) e pela definição dos planos sagital mediano e coronal. A assimetria facial foi verificada tomando-se o lado sem fissura como controle comparando-o com o lado fissurado, calculando-se a distância entre cada ponto aos referidos planos para as dimensões transversais e sagitais, sendo que a vertical foi estimada pela localização de cada ponto no referido corte axial. Os dados foram tabulados e submetidos à analise estatística com o Teste dos Postos Sinalizados de Wilcoxon, com nível de significância p0,050. Os resultados mostraram uma tendência do lado fissurado ser menor que o não fissurado nas avaliações transversais. Quanto a lateralidade da fissura observou-se que as fissuras do lado esquerdo apresentam maior grau de assimetria transversal para as variáveis Zf, Nm, Or e Zy. Na avaliação sagital apenas a variável Cn apresenta diferença estatisticamente significante entre o lado fissurado e o não fissurado independente da lateralidade da fissura. Ao utilizar mensurações estimadas pelos cortes axiais, devido à limitação do software OsiriXTM, também Cn foi a única variável a mostrar significância estatística na avaliação vertical, independente da lateralidade da fissura. Pode-se concluir que o software OsiriXTM apresenta limitações quanto às avaliações verticais. Os pacientes com FLP unilateral completa apresentam assimetrias transversais relacionadas à lateralidade da fissura, com predominância do lado esquerdo. As assimetrias sagitais e verticais se concentram na região nasal, independente da lateralidade da fissura.

The facial morphology of children born with complete unilateral cleft lip and palate (CLP) have injuries of soft and hard tissues of nasomaxillary complex. The purpose of this research was to evaluate the bony structures of the midface of patients with complete unilateral CLP by cone beam computed tomography (CBCT) using the OsiriX software. The sample consisted of CBCT in 28 children with complete unilateral cleft lip and palate in which 19 on the left side and 9 on the right side, with a mean age of 9 years and 6 months. All CBCT were obtained immediately before the secondary alveolar bone graft after maxillary expansion. The structures analysis of the midface was performed using the OsiriXTM DICOM Viewer software, run on a personal computer MacBook Pro, with the operation system MAC OS X Version 10.6.8 Apple Inc. The hemifacial measurements were performed through the demarcation of seven pairs of bilateral points (Zf, Zy, Nm, Cn, J, Or and Po) and by the definition of the median sagittal and coronal planes. The facial asymmetry was verified becoming th side without cleft like the control side and the side cleft like the study side. The distance between each point was calculated in the referred plans for the sagittal and transverse dimensions, which the vertical was estimated by each point in the axial section. The data were tabulated and submitted to statistical analysis by test the Wilcoxon signed-stations, with a significance level p0.050. The results showed a tendency of the cleft side be smaller than the noncleft for transverse evaluations. About the laterality of the cleft it was verified that the clefts on the left show higher degree of asymmetry transverse to the variables Zf, Nm, Or and Zy. In the sagittal assessment, only Cn variable shows a statistically significant difference between the cleft side and noncleft, regardless of laterality of the cleft. Using estimated measurements with axial slices, due to the limitation of the software OsiriXTM, Cn also was the only variable to show statistical significance in the vertical assessment, independent of laterality of the cleft. It can be concluded that the software OsiriXTM has limitations in the assessments vertical. Patients with complete unilateral CLP had transverse asymmetries related to laterality of the cleft, mainly on the left side. The sagittal and vertical asymmetries were focused in the nasal region, independent of laterality of the cleft.
