Um sistema mais adequado de fixação interna no tratamento de fraturas mandibulares é essencial para um bom resultado. Foi realizado um estudo comparativo da utilização do sistema de fixação 2.0-mm locking com sistemas convencionais no tratamento de fraturas mandibulares. Para este estudo, 87 pacientes consecutivos com 134 fraturas mandibulares foram divididos aleatoriamente para receber placas 2.0-mm locking (n = 45) ou convencionais 2.0-mm ou 2.4-mm (n = 42) e tiveram um seguimento mínimo de 6 meses. Foram obtidos dados pessoais, presença de doenças sistêmicas, abuso de substâncias, etiologia, politraumatismo e as localizações. As fraturas foram classificadas com base no grau de deslocamento e de complexidade. Foi mensurada a amplitude da abertura bucal. Análises estatísticas foram feitas na comparação entre grupos (p<= 0,050). Houve maior faixa etária (p = 0,010) e mais distúrbios gerais (p< 0,001) no Grupo Locking. Houve maior gravidade das fraturas (p= 0,004) no Grupo Convencional. Houve diferença significante para a localização das fraturas (p = 0,009), com mais casos na região de corpo no Grupo Locking. Houve aumento progressivo dos valores da abertura bucal, com diferença significante em ambos os grupos (p<= 0,050), sendo que o convencional apresentou melhor recuperação (47,2 ± 8,9 mm). Um caso de complicação pós-operatória maior ocorreu no Grupo Locking (2,2%) e três casos no Grupo Convencional (7,1%). Foi observado que o sistema 2.0-mm locking mostrou melhores resultados clínicos, pois ocorreu apenas um caso de complicação maior, porém o grupo convencional mostrou recuperação mais rápida da abertura bucal e, que também foi constatado que o sistema 2.0-mm locking foi efetivo no tratamento de fraturas desfavoráveis que necessitariam do sistema 2.4-mm convencional.

A more adequate system of internal fixation in the treatment of mandibular fractures is essential for a good result. A comparative study of the use of the 2.0-mm locking fixation system with conventional systems in the treatment of mandibular fractures was performed. For this study, 87 consecutive patients with 134 mandibular fractures were randomized to receive 2.0-mm locking plates (n = 45) or conventional 2.0-mm or 2.4-mm plates (n = 42) and had a minimum follow-up of 6 months. Fractures were classified based on the degree of displacement and complexity. The amplitude of mouth opening was measured. Statistical analyses were performed to compare groups (p <= 0.050). There were older age (p = 0.010) and more substance abuse (p = 0.001) in the Locking group. There was more complex fractures (p = 0.017) and teeth in the fracture line (p = 0.044) in the conventional group. There was a significant difference for the location of fractures (p = 0.009), with more cases in the body region in the Locking group. There was a progressive increase in the values of mouth opening, with a significant difference in both groups (p <= 0.050), whereas the conventional group showed better recovery (47.2 ± 8.9 mm). A case of major postoperative complication occurred in the Locking Group (2.2%) and three cases in the conventional group (7.1%). The 2.0-mm locking system showed better clinical outcomes because there was only one case of major complications and was effective in the treatment of unfavorable fractures that would require the conventional system 2.4-mm but the group treated with conventional systems showed faster recovery of mouth opening.
