A hialuronidase é uma enzima utilizada em anestesias oftalmológicas com o intuito de melhorar a eficácia anestésica. No entanto, como a hialuronidase aumenta a difusão do anestésico local injetado tanto para o feixe nervoso quanto para a circulação sistêmica, podendo causar alterações nos parâmetros cardiovasculares. O objetivo deste estudo controlado, duplo-cego, boca dividida e parcialmente randomizado, foi avaliar os efeitos cardiovasculares induzidos pelo bloqueio do nervo alveolar inferior com o anestésico local (AL) cloridrato de lidocaína, associado à epinefrina concomitantemente à injeção de hialuronidase (H) 75 UTR/ml ou placebo (PL), para a realização de cirurgias de terceiros molares inferiores bilaterais e simétricos em 20 pacientes. Os parâmetros cardiovasculares pressão arterial sistólica (PS), diastólica (PD), média (PM), e freqüência cardíaca (FC), foram monitorados pelos métodos oscilométrico e fotopletismográfico, respectivamente, e avaliados em 8 etapas clínicas: T(-5´), T (AL+H ou PL), T(5´), T(incisão´), T(luxação), T(avulsão), T(sutura) e T(repouso). A hialuronidase injetada concomitante ao anestésico local, não induziu alterações significativas de PS, PD, PM e FC (p>0,01), comparada ao placebo. Os valores médios das variáveis não diferiram (p>0,01) entre as etapas clínicas. O uso do AL concomitante à injeção de hialuronidase mostrou-se seguro para essa dose e via de administração.

The hyaluronidase is an enzyme that has been employed in ophthalmology to improve the effectiveness of anesthesia and to avoid supplementary anesthesia. However, the hialuronidase can increase the diffusion of local anesthetic to the nervous fibers and to the blood circulation as well, which could induce changes in the cardiovascular dynamic. The purposes of this controlled, double-blind, splint mouth and partially randomized trial were to evaluate the cardiovascular effects induced by alveolar inferior nerve block of local anesthetic (LA) 2% lidocaine hydrochloride with 1:100 000 epinephrine, injected concomitantly to hialuronidase 75 TRU/ml or placebo, on symmetric bilateral lower third molar surgery in 20 health outpatients. The cardiovascular parameters systolic (SP), diastolic (DP) and mean (MP) pressures and heart rate (HR) were monitored by oscillometric and photopletismographic methods respectively, in 8 clinical steps of the procedure: T(-5´), T (LA+H or PL), T(5´), T(incision), T(shifted tooth), T(dental avulsion), T(suture) e T(rest). The hyaluronidase injected concomitantly to the LA not induce significant cardiovascular changes (p>0.01) comparing to placebo. The mean values of the variables did not differ significantly among the clinical steps. The use of hyaluronidase injected concomitantly to the LA shows to be safe for this dose and way of administration.
